1) Although C99 allows implementations to define additional SIG*
constants in scope, C90 does not; so, add namespace visibility conditionals to SIG*. 2) Define the extended __sighandler_t type only in BSD namespace. 3) Don't forward declare a struct for a prototype in <signal.h>. 4) Move location of SIG_* constants. 5) Move a forward declare into the correct namespace conditional.
