o add country codes from Atheros regulatory; these are not listed in

the ISO tables, mark them accordingly
o add sku's for handling 900MHz cards
o add opaque struct defs and change []'s to *'s so this file can be
  included w/o requiring all of net80211 to be pulled in
o make CTRY_DEBUG and CTRY_DEFAULT public
