0e6400c7f0 - Updated to Luigi's 2-15-98 code. The code in 2.2 is the same except for
select/poll and DEVFS changes, which are limited to an include/define
  in sound.h and the actual select/poll implementation in sound.c

[ This commit is blind, but the code is similar enough that there will
  hopefully be no problems. ]

a021db1639 - If a PCCARD serial device is removed from the system, let the serial
driver 'break out' of the infinite loop waiting for a response from
  it.  This is a bad thing, but no worse than having the kernel hang.

a4daaa09f2 Implement the spirit but not the letter of Terrys hot-char patch.
The differences Terrys patch and this patch are:
 * Remove a lot of un-needed comments.
 * Don't put l_hotchar at the front of stuct linesw, there is no need to.

5883bcc9d2 Don't (conditionally) override the kernel's min() and max() functions
with macros.  This breaks if the functions are replaced by macros with
unsuitable semantics.   Define a MAX() macro unconditionally instead.
max() is unsuitable since we need a constant expression.  Don't define
MIN() - we never used min().

1d804e7925 Update timer0_prescaler_count before calling hardclock() while timer0
is "acquired".  This fixes a TSC biasing error of about 10 msec when
pcaudio is active.

Update `time' before calling hardclock() when timer0 is being released.
This is not known to be important.

Added some delays in writertc().  Efficiency is not critical here, unlike
in rtcin(), and we already use conservative delays there.

Don't touch the hardware when machdep.i8254_freq is being changed but
the maximum count wouldn't change.  This fixes jitter of up to 10 msec
for most small adjustments to machdep.i8254_freq.  When the maximum
count needs to change, the hardware should be adjusted more carefully.

8a69c85a7e Add support for VESA mode 0x102 (800x600x4) in syscons. You can activate
this using option "-b" to the boot blocks.  It is smartest to compile
a font into your kernel (See LINT), but not mandatory, but apart from
the cursor you will see nothing on the screen until you load a font.

This mode allows XF86_VGA16 to run in 800x600 mode on otherwise unsupported
graphics hardware.

A number of buglets in the cursor handling in syscons may become
visible this way.

045b6fefd8 Fixed initialization of the 4MB page. Kernels larger than about 2.75MB

cd59d49d07 Only use the i586-optimized copying and zeroing functions if they are
actually faster (more than 20% faster for zeroing 1 MB at boot time).
This fixes pessimized copying and zeroing on K6's and perhaps on other
CPUs that are misclassified as i586's.

1ee1681798 - Move the mouse cursor to the center of the screen after video
mode switch in ioctl.
Possibly related to PR: kern/4271

- A kludge: initialize scp->xpixel and ypixel even in the text mode.
If the console enters the `unknown' graphics mode via the ioctl KDSETMODE
(KD_GRAPHICS), these fields are not set (because syscons cannot know
the correct values), but set_mouse_pos() need to refer to these field
to adjust the mouse position.

- Turn off MOUSE_VISIBLE when switching video mode by ioctl.

- another new option: SC_MOUSE_CHAR
Define the first character code of four consecutive codes to be used for
the mouse cursor.  Default codes are 0xd0 through 0xd3.  Beware that
if you decide to use any codes outside the range of 0xc0-0xdf,
the mouse cursor may not look good, because of the way VGA displays
characters in 9-dot-wide character cells.
Requested by several people.

(This patch was tested by a person who recently reported, in the -current
ML, a page fault problem in the kernel (draw_mouse_iamge()) after
X server shutdown. The patch cured his problem.)

59f5eaf466 - A new semaphore: font_loading_in_progress.
Don't touch/update the screen while manipulating font data.
Possibly related to PR: kern/4271

- Set up VGA in alphanumeric mode rather than graphics mode when
loading font into video memory. This will drastically reduce flicker.

f8a35e9cef - Break scrn_timer() into two pieces. Now screen update is done in a
separate routine: scupdate() called from scrn_timer().

- Make sure that the screen is updated for the low-level console
routines sccngetc() and sccncheckc().  A new routine, sccnupdate(),
is introduced and will call scupdate() above.

- A new option: SC_DISABLE_REBOOT
The reboot key (usually Ctl-Alt-Del) will be ignored if this option
is defined. You may still have the reboot key defined in the keymap and
it won't cause error when the keymap is loaded, but it will be quietly
treated as nop.

f1353e56ee If the kernel was built with USERCONFIG_BOOT, and the '-c' (RB_CONFIG) flag
was supplied to the kernel when booting, ignore any 'quit' commands in the
userconfig script.

This provides an escape mechanism whereby a broken userconfig script may
be worked around.

I've worked to enhance the connect() patches.

I've just tested this with the Linux JDK appletviewer on an applet
that does a lot of connects, and it works as well as during my
previous tests.

The connect() patch is now a merge between my older patch and the
OpenBSD stuff. It ensures that any async error is returned by
connect() instead of getsockopt(SOL_SOCKET, SO_ERROR) as reasonnable
systems do.

There are also minor patches to implement IPPROTO_TCP for
get/setsocktopt(). These are also tested (with Linux Apache).

I would appreciate any feedback regarding these changes, as they'd
be very useful in 2.2.6.

95461b450d 1) Start using a cleaner and more consistant page allocator instead
of the various ad-hoc schemes.
2)	When bringing in UPAGES, the pmap code needs to do another vm_page_lookup.
3)	When appropriate, set the PG_A or PG_M bits a-priori to both avoid some
	processor errata, and to minimize redundant processor updating of page
	tables.
4)	Modify pmap_protect so that it can only remove permissions (as it
	originally supported.)  The additional capability is not needed.
5)	Streamline read-only to read-write page mappings.
6)	For pmap_copy_page, don't enable write mapping for source page.
7)	Correct and clean-up pmap_incore.
8)	Cluster initial kern_exec pagin.
9)	Removal of some minor lint from kern_malloc.
10)	Correct some ioopt code.
11)	Remove some dead code from the MI swapout routine.
12)	Correct vm_object_deallocate (to remove backing_object ref.)
13)	Fix dead object handling, that had problems under heavy memory load.
14)	Add minor vm_page_lookup improvements.
15)	Some pages are not in objects, and make sure that the vm_page.c can
	properly support such pages.

65fa467e59 Moved misplaced idempotency ifdef. Multiple inclusion was fatal since

919de96974 Fixed conflicts between <machine/console.h> and <machine/pcvt_ioctl.h>,
so that it is possible for kdump and truss to include them both.  Use
#defines from pcvt_ioctl.h in conflicting cases, since pcvt_ioctl.h
gives a hint about the bogus third arg to _IO().  Ifdef the common
typedefs.  Export `struct key_t' from pcvt_ioctl.h so pcvt_ioctl.h is
bug for bug compatible with console.h (now both are broken in C++
mode).

   radio available with in some bt848 based cards;additionally, wrote
   code to correctly recognized his bt848 card.
3. Roger Hardiman <roger@cs.strath.ac.uk> submitted various fixes to smooth
   out the microcode and made all modes consistent.

4. Added supported for yuv12 so we know can capture raw streams and feed it
   to mpeg_encoder . The upshot is that we can now mpeg encode more and save
   nearly 100 percent of the disk requirements previously for programs such
   as fxtv first save the raw video image to disk then converted to a
   format suitable for mpeg_encode.

b8c7cb30ab Don't set even parity uselessly in the line control register (parity
is not enabled).

Saved a few bytes by reusing the line control register bits and not
loading the speed in the wrong (cpu) register.

b163ce2f65 Fixed the search for free wd drives. The search was terminated
prematurely when there was a hole (for a cdrom or an unused
interface) in the sequence of wd drives.  This caused non-free
wd units to be probed as atapi drives.  There was no problem
provided the atapi probe failed correctly.

eaf13dd73a Change the busy page mgmt, so that when pages are freed, they
MUST be PG_BUSY.  It is bogus to free a page that isn't busy,
because it is in a state of being "unavailable" when being
freed.  The additional advantage is that the page_remove code
has a better cross-check that the page should be busy and
unavailable for other use.  There were some minor problems
with the collapse code, and this plugs those subtile "holes."

Also, the vfs_bio code wasn't checking correctly for PG_BUSY
pages.  I am going to develop a more consistant scheme for
grabbing pages, busy or otherwise.  For now, we are stuck
with the current morass.

c5b193bfba Retire LFS.
If you want to play with it, you can find the final version of the
code in the repository the tag LFS_RETIREMENT.

If somebody makes LFS work again, adding it back is certainly
desireable, but as it is now nobody seems to care much about it,
and it has suffered considerable bitrot since its somewhat haphazard
integration.

0621c31aaf Make the bounce buffer code a little more robust when space isn't
available.  If there isn't bounce space available, the bounce code
is disabled.  This will allow most large systems to run properly
when the bounce space is mistakenly allocated above 16MB.

c67d7e8aa2 Fix operation with the Iomega Zip 100 ATAPI.
All known versions of this drive (firmware 21.* and 23.*) will lock up
if presented with a read/write request of > 64 blocks.  In the presence
of such a unit, I/O requests of > 64 blocks are fragmented to avoid
this.

facf6a3fbe Check the status port after waiting for DRQ; some drives seem to be very
slow coming off the bus (eg. Iomega's ATAPI Zip).  Failure to do
this results in a false probe of an ATAPI device with garbage

88a92ff10e Even though BIOS writer's guide recommends cpuid instruction of Cyrix
6x86MX CPU is enabled (BIOS should not disable it), some BIOS disables
it via CCR4.  In this case, cpu variable becomes CPU_486 and
identblue() is called.  Because Cyrix 6x86MX has MSR and doesn't have
MSR1002, wrmsr instruction generates general protection fault.

2d8acc0f4a VM level code cleanups.
1)	Start using TSM.
	Struct procs continue to point to upages structure, after being freed.
	Struct vmspace continues to point to pte object and kva space for kstack.
	u_map is now superfluous.
2)	vm_map's don't need to be reference counted.  They always exist either
	in the kernel or in a vmspace.  The vmspaces are managed by reference
	counts.
3)	Remove the "wired" vm_map nonsense.
4)	No need to keep a cache of kernel stack kva's.
5)	Get rid of strange looking ++var, and change to var++.
6)	Change more data structures to use our "zone" allocator.  Added
	struct proc, struct vmspace and struct vnode.  This saves a significant
	amount of kva space and physical memory.  Additionally, this enables
	TSM for the zone managed memory.
7)	Keep ioopt disabled for now.
8)	Remove the now bogus "single use" map concept.
9)	Use generation counts or id's for data structures residing in TSM, where
	it allows us to avoid unneeded restart overhead during traversals, where
	blocking might occur.
10)	Account better for memory deficits, so the pageout daemon will be able
	to make enough memory available (experimental.)
11)	Fix some vnode locking problems. (From Tor, I think.)
12)	Add a check in ufs_lookup, to avoid lots of unneeded calls to bcmp.
	(experimental.)
13)	Significantly shrink, cleanup, and make slightly faster the vm_fault.c
	code.  Use generation counts, get rid of unneded collpase operations,
	and clean up the cluster code.
14)	Make vm_zone more suitable for TSM.

This commit is partially as a result of discussions and contributions from
other people, including DG, Tor Egge, PHK, and probably others that I
have forgotten to attribute (so let me know, if I forgot.)

This is not the infamous, final cleanup of the vnode stuff, but a necessary
step.  Vnode mgmt should be correct, but things might still change, and
there is still some missing stuff (like ioopt, and physical backing of
non-merged cache files, debugging of layering concepts.)

Move sigjmp_buf and jmp_buf structure definitions to machine/setjmp.h
so that i386 can continue to use int as the basic register type and
alpha can use long. Bruce was concerned about possible differing
alignment. I've left the definition of _JBLEN in machine/setjmp.h
even though Bruce's example used the number directly. I don't know if
any other code relies on _JBLEN, so I left it to avoid potential
breakage.

1d5e9e2255 Make INET a proper option.
This will not make any of object files that LINT create change; there
might be differences with INET disabled, but hardly anything compiled
before without INET anyway.  Now the 'obvious' things will give a
proper error if compiled without inet - ipx_ip, ipfw, tcp_debug.  The
only thing that _should_ work (but can't be made to compile reasonably
easily) is sppp :-(

This commit move struct arpcom from <netinet/if_ether.h> to
<net/if_arp.h>.

eb794faa90 Define integer types added by POSIX.1g. We already had int8_t, int16_t,
and int32_t -- this adds their unsigned counterparts uint8_t, uint16_t,
and uint32_t.  Bruce has noted that C9x also defines these types (in a
different file), so this may have to be revisited with some ifdefs at a
later date.

a3bd3dac25 Added accent (dead) key support to syscons and kbdcontrol.
With a keymap with accent key definitions loaded to syscons, you press
an accent key followed by a regular letter key to produce an accented
letter.  Press an accent key followed by the space bar to get the
accent letter itself.

Code is based on the ideas and work by jmrueda@diatel.upm.es and
totii@est.is.

95e5e988e0 Make our v_usecount vnode reference count work identically to the
original BSD code.  The association between the vnode and the vm_object
no longer includes reference counts.  The major difference is that
vm_object's are no longer freed gratuitiously from the vnode, and so
once an object is created for the vnode, it will last as long as the
vnode does.

When a vnode object reference count is incremented, then the underlying
vnode reference count is incremented also.  The two "objects" are now
more intimately related, and so the interactions are now much less
complex.

When vnodes are now normally placed onto the free queue with an object still
attached.  The rundown of the object happens at vnode rundown time, and
happens with exactly the same filesystem semantics of the original VFS
code.  There is absolutely no need for vnode_pager_uncache and other
travesties like that anymore.

A side-effect of these changes is that SMP locking should be much simpler,
the I/O copyin/copyout optimizations work, NFS should be more ponderable,
and further work on layered filesystems should be less frustrating, because
of the totally coherent management of the vnode objects and vnodes.

Please be careful with your system while running this code, but I would
greatly appreciate feedback as soon a reasonably possible.

ab9bca19b8 Removed unnecessary (and broken) wakeup code in rpstart(). There is no
need to do it directly, since ttwwakeup() is always called just before
returning from rpstart().  The brokenness was waking up the wrong address
after clearing TS_SO_OLOWAT.  It's not clear how processes waiting for
output to drain below low water ever got woken up.

Found by:	when I fixed longstanding warts in output watermark
		handling, this was the only driver that knew too much
		(anything) about the watermarks

e1d6dc656d This patch causes the "calltodo" timer list to be decremented by the amount
of time that the laptop was suspending.  Thus, select() calls that might have
suspended rather than firing  at 1hr + "time suspended" since the timer was
posted.

Adding:

    options  APM_FIXUP_CALLTODO

to the kernel config enables the patch.

This patch was slightly modified to use a consistant indent style and
I removed some unused local variables.  After this has been tested a
few weeks we'll make the options the default, so for now I'm now
documenting it in LINT.  Mike can later if he wants.

adbf9b6f86 Hopefully fix the problem with the TLB not being updated correctly.
Problem tracked down by bde@freebsd.org, but this is an attempted
efficient fix.

8277ac255e Make the class code checks in function pci_cfgcheck less strict.
It failed to recognize the PCI bus in a system that had only an
old chip-set (class code 000000) and a Cyclom multiport serial
card on PCI bus 0, but no VGA card or disk or network controller.

8cd077a7fd Augment $PATH to ensure searching of /sbin and /usr/sbin for sysctl
