718cf2ccb9 sys/dev: further adoption of SPDX licensing ID tags.
Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.

The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

fbbd9655e5 Renumber copyright clause 4
Renumber cluase 4 to 3, per what everybody else did when BSD granted
them permission to remove clause 3. My insistance on keeping the same
numbering for legal reasons is too pedantic, so give up on that point.

2c9b30a91f [ath_hal] use the correct revision information for QCA953x.
This probe/attaches correctly in my local branch and now displays
a useful message:

ath0: <Qualcomm Atheros QCA953x> at mem 0x18100000-0x1811ffff irq 0 on nexus0
...

2f544eedb3 Add device identification and probe/attach support for the QCA9565.
The QCA9565 is a 1x1 2.4GHz 11n chip with integrated on-chip bluetooth.
The AR9300 HAL already has support for this chip; it just wasn't
included in the probe/attach path.

Tested:

* This commit brought to you over a QCA9565 wifi connection from
  FreeBSD.
* .. ie, basic STA, pings, no iperf or antenna diversity checking just yet.

6479ef780d Bring over the initial 802.11n bluetooth coexistence support code.
The Linux ath9k btcoex code is based off of this code.

Note this doesn't actually implement functional btcoex; there's some
driver glue and a whole lot of verification that is required.

On the other hand, I do have the AR9285+BT and AR9287+BT NICs which
this code supports..
