328d36e934 Replace the unknown driver with a nomatch method in the isa driver.
This allows ISA PnP drivers to be dynamically loaded after the kernel
has booted.

4db9ae91ff New machine-independant /dev/random driver.
This is work-in-progress, and the entropy-gathering routines are not
yet present. As such, this should be viewed as a pretty reasonable
PRNG with _ABSOLUTELY_NO_ security!!

Entropy gathering will be the subject of ongoing work.

This is written as a module, and as such is unloadable, but there is
no refcounting done. I would like to use something like device_busy(9)
to achieve this (eventually).

f6011da8dc New machine independant /dev/null and /dev/zero driver. This device is
severely stripped down compared with its predecessor, and is measurably
a _lot_ faster.

Many thanks to Jeroen van Gelderen for lots of good ideas.

There is still a problem with this; it is written as a mudule, and as
such is theoretically unloadable. However, there is no refcounting done
as I would prefer to do that a'la device_busy(9), rather than some
"home-rolled" scheme. The point is pretty moot, as /dev/null is
effectively compulsory.

55b59b50f2 If swdpio1 doesn't clear, we have a reversed (or disconnected) cable. Change
the message to indicate that it could also be a disconnected cable, and
return okay from wx_hw_intialize *anyway*. This allows us to contineu to
set the station address and when we do get link up, we're ready to roll.

7e23685d60 Stupid me, I put the opt_tdfx.h underneath a test for TDFX_LINUX, which
resides in opt_tdfx. I also cleand up that large define. Compile, tell me
if it does, and I'll re-enable the tdfx entry in the makefile.

2a50a6d730 Add PnP probe methods to some common AT hardware drivers. In each case,
the PnP probe is merely a stub as we make assumptions about some of this
hardware before we have probed it.

Since these devices (with the exception of the speaker) are 'standard',
suppress output in the !bootverbose case to clean up the probe messages
somewhat.

c636255150 fix races in the uidinfo subsystem, several problems existed:
1) while allocating a uidinfo struct malloc is called with M_WAITOK,
   it's possible that while asleep another process by the same user
   could have woken up earlier and inserted an entry into the uid
   hash table.  Having redundant entries causes inconsistancies that
   we can't handle.

   fix: do a non-waiting malloc, and if that fails then do a blocking
   malloc, after waking up check that no one else has inserted an entry
   for us already.

2) Because many checks for sbsize were done as "test then set" in a non
   atomic manner it was possible to exceed the limits put up via races.

   fix: instead of querying the count then setting, we just attempt to
   set the count and leave it up to the function to return success or
   failure.

3) The uidinfo code was inlining and repeating, lookups and insertions
   and deletions needed to be in their own functions for clarity.

d2693dbbc4 Add code so that the udata field is preserved across a TRACK event.
When re-adding an event, do not reset the event state.  If the event was
pending, it will remain pending.  This allows the user to change the udata
field after the event was registered, while not losing any events which
have already occurred.

445572c1ed Add 'kern.disks', a sysctl which returns the list of disks from
disk_enumerate(), space delimited.  This allows non-root users to get a
list of disks and will simplify libdisk's Disk_Names().

a7769538cc Took TDFX_VERBOSE out of Makefile, added $FreeBSD$ and option for DEBUG.
Added a notice for linux users that the kld isn't working for them yet, so
no linux emu from the kld. It compiles statically alright though.

9147e09e64 Converted all TDFX_VERBOSE ifdefs to DEBUG, now it gets debugged with the
rest of the kernel. Added new option TDFX_LINUX for optional
LINUX_EMULATION, so that perhaps some people don't have to use linux emu to
run the driver if they don't want to.

f9c078dffe First import of my 3dfx voodoo driver. Currently it supports the Voodoo Graphics and Voodoo2 perfectly. It works just like the 3dfx driver does for linux, by using a character device at /dev/3dfx of Major 107 to provide a window into the 3dfx card's memory space. This interface is used by glide and mesa as far as i know, and probably some other libraries too.

b5f05e294c Fixed to support RSA98-III non-pnp mode. rman_get_start() had returned
iobase + 8 because the I/O address table for RSA98-III starts with +8.
Now, bus_alloc_resource() is used instead of isa_alloc_resourcev() if
device type is RSA98III.

55a39fc5a2 Added true support for PPTP aliasing. Some nice features include:
- Multiple PPTP clients behind NAT to the same or different servers.

- Single PPTP server behind NAT -- you just need to redirect TCP
  port 1723 to a local machine.  Multiple servers behind NAT is
  possible but would require a simple API change.

- No API changes!

For more information on how this works see comments at the start of
the alias_pptp.c.

PacketAliasPptp() is no longer necessary and will be removed soon.

a79b71281c return of the accept filter part II
accept filters are now loadable as well as able to be compiled into
the kernel.

two accept filters are provided, one that returns sockets when data
arrives the other when an http request is completed (doesn't work
with 0.9 requests)

49c0f52e11 Support bounce buffers for ISA DMA on the alpha. This is required for the
irongate chipset (used in the UP1000) which does not support scatter/gather
DMA.  We'll still use scatter gather if the core logic chipset supports it.

335dd56674 - Add some more details to the 'lsdev' output for FAT file systems.
- Add in support for the EDD (Enhanced Disk Drive) BIOS extensions to
  use LBA mode for accessing drives past cylinder 1024.  This should allow
  us to load a kernel from anywhere on a newer drive up to 2 TB.  Part
  of this came from the PR below.

6019e6208f When running with quotas enabled on a filesystem using soft updates,
the system would panic when a user's inode quota was exceeded (see
PR 18959 for details). This fixes that problem.

d3abb52714 Some additional performance improvements. When freeing an inode
check to see if it has been committed to disk. If it has never
been written, it can be freed immediately. For short lived files
this change allows the same inode to be reused repeatedly.
Similarly, when upgrading a fragment to a larger size, if it
has never been claimed by an inode on disk, it too can be freed
immediately making it available for reuse often in the next slowly
growing block of the same file.

0218cb013b General tidy up and plough through TODO list.
Rewrote intro at top of file to reflect my better understanding of how it
the memory mapping works.

Clear the DONE list and move some thoughts into the TODO list.

Remove RECERR from RAY_DEBUG

Start to use a desired network parameter structure, only used in download
code as I've realised that there are some problems with the idea.

Break up ray_rx, and move the data packet handler into a seperate function. This meant some knock on changes in ray_rx_mgt/ray_rx_ctl to do with
mbuf freeing.

Remove some debug code/XXX comments that are out of date.

56aef50302 Clean up firmware load issues and remove darn near all config options.
Force alphas to prefer mem mapping as the default.

Basically, we have a pointer to a function which we can call which will
return us a pointer to firmware for the card we have. We call this function
(if it's non-NULL) with the address of our mdvec f/w pointer.

The way this works is that if ispfw (as a module or a static) is loaded,
it initializes the pointer in isp_pci, so we can call into to it to fetch
a pointer to a f/w set.

If ispfw is MOD_UNLOADed, it's retained a pointer to our mdvec f/w pointers,
which then get zeroed out so we don't have any references to data that's
now gone from kernel memory. Removing the f/w saves ~360KBytes.

Alas, there is no autounload mechanism that works for is here.

This should allow one to load oldcard or newcard for testing.  Please
let me know if this doesn't work.  Don't load this and either of pcic
or pccard.  I've not tried it, but I suspect bad things will happen.

6cd40f893b o Implement some compatibility functions for NEWCARD compat. This is
incomplete, but will eventually allow the same drivers to function
  with both oldcard and newcard.
o Remove include of opt_bus.h.  It isn't needed and gets in the way of
  module building.

fb1d37adcd Once we have firmware running (if isp_reset) and this is the first time
through, establish what our LUN width is. Unfortunately, we can't ask
the f/w. If we loaded the f/w, we'll now assume we have expanded LUNs
(SCCLUN for fibre channel, just plain 32 LUN for SCSI). If we didn't
load firmware, assume 8 LUNs for SCSI and 1 LUN for Fibre Channel. We
have to assume only one LUN for Fibre Channel because the LUN setting
in Request Queue entries is in different places whether we have SCCLUN
firmware or not, so the only LUN guaranteed to work for both is LUN 0.

Clean up the rest of isp.c so that ISP2100_SCCLUN defines aren't used-
instead use run time determinants based upon isp->isp_maxluns.

After starting firmware, delay 500us to give it a chance to get rolling.

Fix the interrupt service routine to check for both isr && sema being zero
before thinking this was a spurious interrupt.  Following the manuals,
allow for both Mailbox as well as Queue Reponse type interrupts for regular
SCSI.

c46d792d58 Deal with quoted arguments. This hack parser uses whitespace to delimit
fields, not lex/yacc grammar so it is not an exact match but should be
close enough for most cases.
Deal with 'port?', 'irq?' style specifications.  These are parsed as
seperate values in lex/yacc in config(8) but tripped up this helper tool.

7ebe1c3acf Use while (<>) instead of while(<STDIN>) so that perl will automagically
deal with filename arguments.  It is amazing how much you forget over time.

Thanks to the people that reminded me this.  I knew there was an easy way
that didn't involve messing with $argv, filehandles, etc, but just could
not remember - all of my books are on the opposite side of the planet..

1ddb0ab555 Add support for emulating the RDMSR and WRMSR instructions into BTX. In
theory, this should allow the K7V Athlon motherboard to boot ok with boot
virus protection enabled.  However, I have no hardware to test this.  It
shouldn't break anything though. :)

b106252c19 Implement SIOCSIFLLADDR, which allows you to change the link-level
address on an interface. This basically allows you to do what my
little setmac module/utility does via ifconfig. This involves the
following changes:

socket.h: define SIOCSIFLLADDR
if.c: add support for SIOCSIFLLADDR, which resets the values in
      the arpcom struct and sockaddr_dl for the specified interface.
      Note that if the interface is already up, we need to down/up
      it in order to program the underlying hardware's receive filter.
ifconfig.c: add lladdr command
ifconfig.8: document lladdr command

You can now force the MAC address on any ethernet interface to be
whatever you want. (The change is not sticky across reboots of course:
we don't actually reprogram the EEPROM or anything.) Actually, you
can reprogram the MAC address on other kinds of interfaces too; this
shouldn't be ethernet-specific (though at the moment it's limited to
6 bytes of address data).

Nobody ran up to me and said "this is the politically correct way to
do this!" so I don't want to hear any complaints from people who think
I could have done it more elegantly. Consider yourselves lucky I didn't
do it by having ifconfig tread all over /dev/kmem.

8f4e4aa5f1 add socketoptions DELAYACCEPT and HTTPACCEPT which will not allow an accept()
until the incoming connection has either data waiting or what looks like a
HTTP request header already in the socketbuffer.  This ought to reduce
the context switch time and overhead for processing requests.

The initial idea and code for HTTPACCEPT came from Yahoo engineers and has
been cleaned up and a more lightweight DELAYACCEPT for non-http servers
has been added

81e3fd99fc This is a temporary bandaid to get vidconsole working again without
options USERCONFIG being present.  Due to the lack of early boot hints
neither sio or sc would succeed the console probe.  If USERCONFIG was
active, there was a second cninit() after userconfig had run and that
happened to make the console selection work.  If you left out USERCONFIG,
you would end up with no console at all. :-(

This needs a proper fix, especially when sc looses the "at isa" hint.
But for now, this works.

6f6b2cd019 Linux allows to mmap annonymous with a file descriptor passed, FreeBSD
doesn't.  In the Linux emulation layer, ignore the fd passed when
MAP_ANON is specified.

Known application to be fixed: Xanalys/Harlequin Lispworks

Also improve debug output for mmap, now showing what the emulation
layer mapped to what (-DDEBUG).

03c16f958a This file lies by saying 200-255 are reserved for local use, thus
implying that they aren't used for the rest of the system.

Fix the lies:
	253 is used by mfs (bad MFS for not registering it).
	254 is a magic cookie inside of the dev code in at least one place.
	255 is -1 which is magic in a different way in the dev code.
So, that means that 200-252 are reserved for local users.  A grep for
252 didn't turn anything up, so I'm assuming it and lower are safe.

And I thought I was being smart by allocating our local major numbers
from 254 on down.  This caused very very odd problems that were hard
to track down: close not being called, sync failing at reboot, etc.

18ee60a576 Remove the setting of sourceid from bf_vm(), as bf_vm() really has
no clue.

Set sourceid to 0 when booting, which is the correct setting for stdin.

Set sourceid to an arbitrary fd when include'ing, preserving and restoring
the previous sourceid. This is possibly broken(), as 0 is a valid fd. Maybe
we should +1 to this value.

This fixes the version problem widely reported.

8e8cac5555 sys/malloc.h:
Order the SYSINIT() for MALLOC_DEFINE() correctly so that malloc()
doesn't have to waste time initializing itself.  The
(SI_SUB_KMEM, SI_ORDER_ANY) order was shared with syscons' SYSINIT()
for scmeminit(), and scmeminit() calls malloc(), so malloc()
initialization was not always complete on the first call to malloc().

kern/kern_malloc.c:
- Removed self-initialization in malloc().
- Removed half-baked sanity check in free().  Trust MALLOC_DEFINE().

979ab75162 Removed support for generating inline code for MALLOC() and FREE()
in the dysfunctional !KMEMSTATS case.  This hasn't compiled since
rev.1.31 of kern_malloc.c quietly removed the core of the support
for the !KMEMSTATS case.  I fixed it to see if it was worth saving
and found that (as usual) inlining just wasted space and increased
complexity without significantly affecting time, at least for the
lmbench2 micro-benchmark on a Celeron.  The space bloat was
surprisingly large - the text size increased from 1700K to 1840K
for a version with the entire malloc() family inlined.

Removed even older garbage (kmemxtob() and btokmemx() macros).

Attempt to deprecate MALLOC() and FREE().  Given current compilers
(gcc-2.x or C99), they don't do anything that (safe) function-like
macros or inline functions named malloc() and free() couldn't do.

Fixed missing casts of macro args in MALLOC() and FREE().

- SECURITY: FTP aliasing engine now ensures that:
  o the segment preceding a PORT/227 segment terminates with a \r\n;
  o the IP address in the PORT/227 matches the source IP address of
    the packet;
  o the port number in the PORT command or 277 reply is greater than
    or equal to 1024.

f71c01cc52 Borrow phk's axe and apply the next stage of config(8)'s evolution.
Use Warner Losh's "hint" driver to decode ascii strings to fill the
resource table at boot time.

config(8) no longer generates an ioconf.c table - ie: the configuration
no longer has to be compiled into the kernel.  You can reconfigure your
isa devices with the likes of this at loader(8) time:
  set hint.ed.0.port=0x320

userconfig will be rewritten to use this style interface one day and will
move to /boot/userconfig.4th or something like that.

It is still possible to statically compile in a set of hints into a kernel
if you do not wish to use loader(8).  See the "hints" directive in GENERIC
as an example.

All device wiring has been moved out of config(8).  There is a set of
helper scripts (see i386/conf/gethints.pl, and the same for alpha and pc98)
that extract the 'at isa? port foo irq bar' from the old files and produces
a hints file.  If you install this file as /boot/device.hints (and update
/boot/defaults/loader.conf - You can do a build/install in sys/boot) then
loader will load it automatically for you.  You can also compile in the
hints directly with:  hints "device.hints"  as well.

There are a few things that I'm not too happy with yet.  Under this scheme,
things like LINT would no longer be useful as "documentation" of settings.
I have renamed this file to 'NOTES' and stored the example hints strings
in it.  However... this is not something that config(8) understands, so
there is a script that extracts the build-specific data from the
documentation file (NOTES) to produce a LINT that can be config'ed and
built.  A stack of man4 pages will need updating. :-/

Also, since there is no longer a difference between 'device' and
'pseudo-device' I collapsed the two together, and the resulting 'device'
takes a 'number of units' for devices that still have it statically
allocated.  eg:  'device fe 4' will compile the fe driver with NFE set
to 4.  You can then set hints for 4 units (0 - 3).  Also note that
'device fe0' will be interpreted as "zero units of 'fe'" which would be
bad, so there is a config warning for this.  This is only needed for
old drivers that still have static limits on numbers of units.
All the statically limited drivers that I could find were marked.

Please exercise EXTREME CAUTION when transitioning!

e19c49c532 Fix a problem of user settings from TEKRAM NVRAM
layout introduced in driver 1.5.3. The driver was
confused by the bogus TEKRAM table used to translate
user sync. setting to SCSI sync. factor.
Btw, the new TEKRAM DC-390 U3D and U3W Ultra-160
controllers seem to be using BIOS from SYMBIOS/LSI
and thus SYMBIOS NVRAM layout.
If that means that TEKRAM will now offer real
SYMBIOS software compatible SCSI controllers, then
it is a *GREAT NEWS*.

65cbb03cfe Added new options CPU_PPRO2CELERON and CPU_L2_LATENCY to support
Socket 8 to 370 converters.  When (1) CPU_PPRO2CELERON option is
defined, (2) Intel CPU is found and (3) CPU ID is 0x66?, L2 cache is
enabled through MSR 0x11e.  The L2 cache latency value can be
specified by CPU_L2_LATENCY option.  Default value of L2 cache latency
is 5.

These options are useful if you use Socket 8 to Socket 370 converter
(e.g. Power Leap's PL-Pro/II.)  Most PentiumPro BIOSs don't enable L2
cache of Mendocino Celeron CPUs because they don't know Celeron CPUs.
These options are needles if you use a Coppermine (FCPGA) Celeron or
PentiumIII, becuase the L2 cache enable bit is hard wired and L2 cache
is always enabled.

What we'd like to know is whether or not we have a listener
upstream that really hasn't configured yet. If we do, then
we can give a more sensible reply here. If not, then we can
reject this out of hand.

Choices for what to send were
	Not Ready, Unit Not Self-Configured Yet
	(0x2,0x3e,0x00)
for the former and
	Illegal Request, Logical Unit Not Supported
	(0x5,0x25,0x00)
for the latter.

We used to decide whether there was at least one listener
based upon whether the black hole driver was configured.

However, recent config(8) changes have made this hard to do
at this time.

Actually, we didn't use the above quite yet, but were sure considering it.

Catch detach on ray_ccs_alloc a little better.

Move sc_promisc into desired and current n/w parameters.

Remove IFQ_PEEK, we know the driver runs okay without it.

Drain the output queue in ray_stop.

Only use ray_mcast for ADD/DEL multi ioctls. ray_init_multi resets the
multicast list on startup. Simplifies ray_init a little.

Tidy some old comments.

ray_download_done now copies the whole desired n/w parameter set into the
current set. This is because I was missing soem parameters - like the
net type!

0477138dad Fixed allocation of unit numbers. Allocate the amount of space actually
required (rounded up a little) instead of twice the previous amount (or
a fixed amount for the first allocation).

The bug caused memory corruption when a new unit number for a devclass
was more than about twice the previous maximum one (or more than 3 for
the first one), so it corrupted memory (which happened to be the atkbdc
port resource list) in the reporter's configuration with sio unit
numbers { 0, 25, 1, 2, ... }.

3f664fc689 A checkpoint of a part of a work-in-progress. Some more cleanups for
config(8).  This commit allows control of the creation of the
#include "foo.h" files.  We now only create them explicitly when needed.
BTW; these are mostly bad because they usually imply static limits on
numbers of units for devices.  eg: struct mysoftc sc[NFOO];
These static limits have Got To Go.

ff69e08ad6 The AMI MegaRAID's internal memory map conflicts with scatter/gather
map physical addresses below 0x2000 (accoding to AMI).  If we
allocate our s/g tables and get an address below this point, leak the
memory and try again.

This should fix booting from these controllers.

8204ff6102 Remove promisc_user, ray_reset and ray_reset_timo as they are not used. Incidental remove of a timer too. Remove the runq_abort code.
Get ray_detach working correctly. This is a very simple routine as it
just wakes up sleeping processes. Note that anything woken has NO softc
structure available! runq_add is suitably modified to detect a detach and
return straight away.

Due to ray_detach and its implications use a macro for adding things
to the runq in user land.

597472167a A driver for programming the AGP hardware. This is only very lightly
tested on Intel BX chipsets only. The other agp minidrivers are totally
untested.

The programming api is a subset of the Linux api and is only intended to
be enough for the X server to use. There is also an in-kernel api for the
use of other kernel modules such as the 3D DRI.

129e169651 Always include the full symbol table (as specified by its start and
end values in bootinfo) in kernel space if it is loaded (i.e., if its
specified end address is nonzero), not just if it is loaded and DDB
is configured.  This may be used to fix kldsym(2) for booting without
/dev/loader; currently, in this case, it just fixes unused pointers
and wastes space consistently.  For booting in the normal way with
/boot/loader, the table is included and pointed to in a different way
and kldsym(2) works.

c1312289dd Modify boot-conf so it can take a kernel or directory name as
a parameter and dtrt.

Also, make boot-conf always unload first. There wasn't really any

042c8f6ee1 When I tweaked if_dc.c to alter the polling interval for non-MII
21143 chips, I accidentally removed the DC_MII_REDUCED_POLL flag
for all 21143 cards. This caused problems with timer-instigated
TCP retransmits, which happened to occur at the same time as an
MII poll tick on MII-based cards (e.g. D-Link DFE-570TX). Fixed this,
plus made some other cleanups. The autoneg fixes for the non-MII
cards still work. Also tested the PNIC II now that I have one again.

50ed7dfec0 INTR_TYPE_FAST / FAST_INTR interrupts (currently just serial interrupts)
have their own lock and do not need the MP lock.  The SMP cleanup was

As part of the IFF_RUNNING stuff, we've added an extra flag so callers
can request that runq routines should check IFF_RUNNING before executing.

Remove BPF taps as this is now done by ether_input.

Resurrect multicast code, moving the multicast list stuff to the runq
routine.

Dump ray_promisc_user as all flag changes are now handled by ray_init, and
add a couple of checks to ray_promisc.

In uppparams_user, allow changes before the card is running (need to
fix some breakage with _download here later). In addition, don't
assume that the current n/w parameters are valid - they are only valid
in the runq.

Fix a nasty flag bug - runq_add cleared all the flags on the last command!

Remove the hacks for setting the memory flags - problems were down
to buggy versions of pccardd. For some reason pccardd only dtrt with
the "right" debug_level.

b2b0497ab5 o If FFS_EXTATTR is defined, don't print out an error message on unmount
if an FFS partition returns EOPNOTSUPP, as it just means extended
  attributes weren't enabled on that partition.  Prevents spurious
  warning per-partition at shutdown.

7cadc2663e o Modify jail to limit creation of sockets to UNIX domain sockets,
TCP/IP (v4) sockets, and routing sockets.  Previously, interaction
  with IPv6 was not well-defined, and might be inappropriate for some
  environments.  Similarly, sysctl MIB entries providing interface
  information also give out only addresses from those protocol domains.

  For the time being, this functionality is enabled by default, and
  toggleable using the sysctl variable jail.socket_unixiproute_only.
  In the future, protocol domains will be able to determine whether or
  not they are ``jail aware''.

o Further limitations on process use of getpriority() and setpriority()
  by jailed processes.  Addresses problem described in kern/17878.

61b9b2965c The change to do a longword compare in the previous commit just broke an
apparently-intended micro-optimization ("testb" is equivalent and smaller)
and added a style bug (the size suffix for "testl" was missing).
linux-2.3.35 already had the correct fix.

59f577ad8c When attempting to transmit a packet, if the system fails to allocate
a mbuf, it may return without setting any timers.  If no more data is
scheduled to be transmitted (this was a FIN) the system will sit in
LAST_ACK state forever.

Thus, when mbuf allocation fails, set the retransmit timer if neither
the retransmit or persist timer is already pending.

f47f0edde4 Use "nm | awk ..." instead of genassym(1) to generate symbol value headers.
Symbol values are now represented using array sizes (4 arrays per symbol
so that 16-bit machines can represent 64-bit values) instead of being raw
binary values.

908a81d424 - Do not allow VOP_SETATTR to modify underlying vnodes at all. This caused
problems when fetch(1) was passed `-o -'.  The rationale of this change
  is that applications attempting to change underlying vnodes for /dev/fd
  nodes are improperly written and the use of this interface should not
  ever have been encouraged.  Proper alternatives are fchmod, fchown and
  others.

a2693d6d71 Handle watchdog timeouts better. We can't really call the foo_init()
routines from foo_watchdog() because foo_watchdog() is called at
interrupt context, and that's a no-no due to the way the USB stack
is currently set up.

What we do now is call the TX end of frame handler manually to clear
the completed transmission, then check the send queue and send off
any frames that are pending.

Also turned off the interrupt pipe stuff in if_aue, since it appears
to tickle a bug in the USB stack that I haven't found yet.

3e0f102e57 With this evil hack the DS1 driver works on our Intel desktop
motherboards that use YMF740's.  It has a strange subvendor and subdevice
ID and requires a disturbingly long delay after the ac97 codec init.
Cameron hasn't had this driver tested on another 740 yet, so we don't know
if this is a quirk of all 740's, or if its just something about the codec
that Intel used.

c3c50c4e3a Further fixes for multiple-IO-APIC systems from Tor Egge:
Further experimentation showed that some Dell 2450 machines with the
prevention kludge installed still got T_RESERVED traps.  CPU interrupt
vector 0x7A was observed to be triggered.  This might have been the
bitwise OR of two different vectors sent from each of the IOAPICs at
the same time.

	IOAPIC #0: 0x68 --> irq 8: RTC timer interrupt
	IOAPIC #1: 0x32 --> irq 18: scsi host adapter or network interface
		   ----
		   0x7a --> T_RESERVED

Both IOAPICs had ID 0.

Appendix B.3 in the MP spec indicates that the operating system is
responsible for assigning unique IDs to the IOAPICs.

The enclosed patch programs the IOAPIC IDs according to the IOAPIC
entries in the MP table.
