c9296b3227 loader: F_READ/F_WRITE should be checked against masked flag
The work to make it possible to avoid bcache via using F_NORA modifier did
miss the fact that not all loader platforms are using the bcache, and so
it is possible the modifier is not cleared, as bcache strategy function is
not used.

For fix, we make sure the checks are dont with masked flag.

This patch does fix boot for platforms which do not use bcache.

7b02037ef8 loader: disk io should not use alloca()
The alloca() does give us pointer and we have no practical way to check if the
area is actually available, resulting in corruption in corner cases.

Unfortunately we do not have too many options right now, but to use one page.

1ecc859193 dosfs support in libstand is broken since r298230
Apparently the libstand dosfs optimization is a bit too optimistic
and did introduce possible memory corruption.

This patch is backing out the bad part and since this results in
dosfs reading full blocks now, we can also remove extra offset argument
from dv_strategy callback.

The analysis of the issue and the backout patch is provided by Mikhail Kupchik.

77d81a4dab lsdev device name section headers should be printed by dv_print callback.
lsdev command does walk over devsw list, prints list element name and
will use dv_print() callback to print the device list.
Unfortunately this approach will add unneeded noise when there are no
particular devices detected.

To remove "empty" device section headers, the dv_print() callback
should print the header instead.

In addition, fixed dv_print callback for md module.

cbd6713146 Loader paged/pageable data is not always paged.
This change does modify devsw dv_print() to return the int value,
enabling walkers to interrupt the walk on non zero value from dv_print().

This will allow the pager_print actually to stop displaying data on
user input, and additionally pager is used in various *dev_print callbacks,
where it was missing.

For test, lsdev [-v] command should display data by screenfuls and should
stop when the key 'q' is pressed on pager prompt.

9a0b26ec6f Fix several instances where the boot loader ignored pager_output
return value when it could return 1 (indicating we should stop).
Fix a few instances of pager_open() / pager_close() not being called.
Actually use these routines for the environment variable printing code
I just committed.

ffd50bca7e bcache read ahead may attempt to read past end of disk
The new bcache code does not know the size of the disk, and therefore may attempt to read past the end of the disk while trying to fill its read-ahead cache.

This is usually not an issue, it fails gracefully on all of my machines, but some BIOSes seem to retry the reads for up to 30 seconds each, resulting in a long stall during boot

87ed2b7f5a A new implementation of the loader block cache
The block cache implementation in loader has proven to be almost useless, and in worst case even slowing down the disk reads due to insufficient cache size and extra memory copy.
Also the current cache implementation does not cache reads from CDs, or work with zfs built on top of multiple disks.
Instead of an LRU, this code uses a simple hash (O(1) read from cache), and instead of a single global cache, a separate cache per block device.
The cache also implements limited read-ahead to increase performance.
To simplify read ahead management, the read ahead will not wrap over bcache end, so in worst case, single block physical read will be performed to fill the last block in bcache.

Booting from a virtual CD over IPMI:
0ms latency, before: 27 second, after: 7 seconds
60ms latency, before: over 12 minutes, after: under 5 minutes.

5b9e248af5 - Add a new header for the x86 boot code that defines various structures
and constants related to the BIOS Enhanced Disk Drive Specification.
- Use this header instead of magic numbers and various duplicate structure
  definitions for doing I/O.
- Use an actual structure for the request to fetch drive parameters in
  drvsize() rather than a gross hack of a char array with some magic
  size.  While here, change drvsize() to only pass the 1.1 version of
  the structure and not request device path information.  If we want
  device path information you have to set the length of the device
  path information as an input (along with probably checking the actual
  EDD version to see which size one should use as the device path
  information is variable-length).  This fixes data smashing problems
  from passing an EDD 3 structure to BIOSes supporting EDD 4.

9777aa3f12 Add a version of the FreeBSD bootloader which can run in userland, packaged
as a shared library. This is intended to be used by BHyVe to load FreeBSD
kernels into new virtual machines.

47193e464b The recent change to use memory > 1MB for the heap by default broke CD
booting because the CD driver did not use bounce buffers to ensure
request buffers sent to the BIOS were always in the first 1MB.  Copy over
the bounce buffer logic from the BIOS disk driver (minus the 64k boundary
code for floppies) to fix this.

5c5b5d4607 Slightly cleanup the 'bootdev' concept on x86 by changing the various
macros to treat the 'slice' field as a real part of the bootdev instead
of as hack that spans two other fields (adaptor (sic) and controller)
that are not used in any modern FreeBSD boot code.

932d8c46a2 Extend struct devdesc with a unit field, called d_unit. Promote the
device (kind) specific unit field to the common field. This change
allows a future version of libefi to work without requiring anything
more than what is defined in struct devdesc and as such makes it
possible to compile said version of libefi for different platforms
without requiring that those platforms have identical derivatives
of struct devdesc.

197e5e73ef Libdisk does not need to include <sys/diskslice.h> any more.
Move the remaining bits of <sys/diskslice.h> to <i386/include/bootinfo.h>

Move i386/pc98 specific bits from <sys/reboot.h> to
<i386/include/bootinfo.h> as well.

Adjust includes in sys/boot accordingly.

0322b80600 Add a device driver for the BIOS device for CD-ROM's booted via El Torito
no emulation mode.  Unlike other BIOS devices, this device uses 2048 byte
sectors.  Also, the bioscd driver does not have to worry about slices
or partitions.
