a2d1957bbc Updates to UFS/FFS superblock integrity checks when reading a superblock.
Check for an uninitialed (zero valued) fs_maxbsize and set it
to its minimum valid size (fs_bsize). Uninitialed fs_maxbsize
were left by older versions of makefs(8) and the superblock
integrity checks fail when they are found.

No legitimate superblocks should fail as a result of these changes.

fe5e6e2cc5 Improvement in UFS/FFS directory placement when doing mkdir(2).
The algorithm for laying out new directories was devised in the 1980s
and markedly improved the performance of the filesystem. In those days
large disks had at most 100 cylinder groups and often as few as 10-20.
Modern multi-terrabyte disks have thousands of cylinder groups. The
original algorithm does not handle these large sizes well. This change
attempts to expand the scope of the original algorithm to work well
with these much larger disks while still retaining the properties
of the original algorithm for small disks.

The filesystem implementation is divided into policy routines and
implementation routines. The policy routines can be changed in any
way desired without risk of corrupting the filesystem. The policy
requests are handled by the implementation layer. If the policy
asks for an available resource, it is granted. But if it asks for
an already in-use resource, then the implementation will provide
an available one nearby the request. Thus it is impossible for a
policy to double allocate. This change is limited to the policy
implementation.

This change updates the ffs_dirpref() routine which is responsible
for selecting the cylinder group into which a new directory should
be placed. If we are near the root of the filesystem we aim to
spread them out as much as possible. As we descend deeper from the
root we cluster them closer together around their parent as we
expect them to be more closely interactive. Higher-level directories
like usr/src/sys and usr/src/bin should be separated while the
directories in these areas are more likely to be accessed together
so should be closer. And directories within commands or kernel
subsystems should be closer still.

We pick a range of cylinder groups around the cylinder group of the
directory in which we are being created. The size of the range for
our search is based on our depth from the root of our filesystem.
We then probe that range based on how many directories are already
present. The first new directory is at 1/2 (middle) of the range;
the second is in the first 1/4 of the range, then at 3/4, 1/8, 3/8,
5/8, 7/8, 1/16, 3/16, 5/16, etc.

It is desirable to store the depth of a directory in its on-disk
inode so that it is available when we need it. We add a new field
di_dirdepth to track the depth of each directory. Because there are
few spare fields left in the inode, we choose to share an existing
field in the inode rather than having one of our own. Specifically
we create a union with the di_freelink field. The di_freelink field
is used to track inodes that have been unlinked but remain referenced.
It is not needed until a rmdir(2) operation has been done on a
directory. At that point, the directory has no contents and even
if it is kept active as a current directory is no longer able to
have any new directories or files created in it. Thus the use of
di_dirdepth and di_freelink will never coincide.

previously failed to fail to compile because the type remained int.

42c82aad32 Improve chance of finding an alternate superblock in sbsearch(3).
When requesting a superblock read for the sole purpose of getting
the parameters needed to find if backup parameters have been stored,
specify UFS_NOCSUM as only the base superblock is needed. This
change reduces the number of checks that the superblock must pass.

069767091e Do not panic in case of corrupted UFS/FFS directory.
Historically the system panic'ed when it encountered a corrupt
directory. This change recovers well enough to continue operations.
This change is made in response to a similar change made in the ext2
filesystem as described in the cited Differential Revision.

0eabe33087 Updates to UFS/FFS superblock integrity checks when reading a superblock.
Have to add a check that the computed cylinder group size does not
exceed the block size of the filesystem.

It is also necessary to validate additional parameters when a
superblock is going to be used in read-only mode if its supplementary
information is going to be read in to ensure that the size and
location of the supplementary information is valid. Also when a
warning is raised let it be accepted, but bound the flagged field
to the value checked by the warning.

No legitimate superblocks should fail as a result of these changes.

3b6056204d FIOSEEKHOLE/FIOSEEKDATA: correct consistency for bmap-based implementation
Writes on UFS through a mapped region do not allocate disk blocks in
holes immediately. The blocks are allocated when the pages are paged out
first time.

This breaks the algorithm in vn_bmap_seekhole() and ufs_bmap_seekdata(),
because VOP_BMAP() reports hole for the place which already contains a
valid data.

Clean the pages before doing VOP_BMAP() in the affected functions.  In
principle, we could clean less by only requesting clean starting from
the offset, but it is probably not very important.

6e1eabadcb ffs_syncvnode(): avoid a LoR for SU
There is another case where SU code does ffs_syncvnode(dvp) for the
parent directory dvp while the child vnode vp is locked.  Avoid the
issue by relocking and returning ERELOOKUP to indicate the need of
resync.

As is there is no mechanism which allows filesystems to denote that a
vnode is fully initialized, consequently problems like the above are
only found the hard way(tm).

Add rudimentary support for state transitions, which in particular allow
to assert the vnode is not legally unlocked until its fate is decided
(either construction finishes or vgone is called to abort it).

The new field lands in a 1-byte hole, thus it does not grow the struct.

b13110e9f3 ufs/ffs: detect endian mismatch between machine and filesystem
Mount on a LE machine a filesystem formatted for BE is not supported
currently. This adds a check for the superblock magic number using
swapped bytes to guess and warn the user that it may be a valid
superblock but endian is incompatible.

78f4129876 Enable taking snapshots on UFS/FFS filesystems using journaled soft updates.
All the needed infrastructure updates have been made to allow
snapshots to be taken on UFS/FFS filesystems that are using journaled
soft updates. The most immediate benefit is the ability to use a
snapshot to take a consistent filesystem dump on a live filesystem
using the -L option to dump(8).

243a0eda9a Increase the maximum size of the journaled soft-updates journal.
The size of the journaled soft-updates journal should be big enough
to hold two minutes of filesystem metadata-update activity. The
maximum size of the soft updates journal was set in the 1990s. At
the time it was assummed that disk arrays would top out at 16 drives
and disk writes per drive would top out at 500 per second. Today's
I/O subsystems are considerably bigger and faster than those limits.
Thus this delta removes the hard upper limit and lets tunefs(8) and
newfs(8) set the upper bound based on the size of the filesystem and
its cylinder groups.

6b9d4fbb7f Explicitly initialize rather than reading newly allocated UFS inodes.
The function ffs_vgetf() is used to find or load UFS inodes into a
vnode. It first looks up the inode and if found in the cache its
vnode is returned. If it is not already in the cache, a new vnode
is allocated and its associated inode read in from the disk. The
read is done even for inodes that are being initially created.
The contents for the inode on the disk are assumed to be empty. If
the on-disk contents had been corrupted either due to a hardware
glitch or an agent deliberately trying to exploit the system, the
UFS code could panic from the unexpected partially-allocated inode.

Rather then having fsck_ffs(8) verify that all unallocated inodes
are properly empty, it is easier and quicker to add a flag to
ffs_vgetf() to indicate that the request is for a newly allocated
inode. When set, the disk read is skipped and the inode is set to
its expected empty (zero'ed out) initial state. As a side benefit,
an unneeded disk I/O is avoided.

e688661642 Move the ability to search for alternate UFS superblocks from fsck_ffs(8)
into ffs_sbsearch() to allow use by other parts of the system.

Historically only fsck_ffs(8), the UFS filesystem checker, had code
to track down and use alternate UFS superblocks. Since fsdb(8) used
much of the fsck_ffs(8) implementation it had some ability to track
down alternate superblocks.

This change extracts the code to track down alternate superblocks
from fsck_ffs(8) and puts it into a new function ffs_sbsearch() in
sys/ufs/ffs/ffs_subr.c. Like ffs_sbget() and ffs_sbput() also found
in ffs_subr.c, these functions can be used directly by the kernel
subsystems. Additionally they are exported to the UFS library,
libufs(8) so that they can be used by user-level programs. The new
functions added to libufs(8) are sbfind(3) that is an alternative
to sbread(3) and sbsearch(3) that is an alternative to sbget(3).
See their manual pages for further details.

The utilities that have been changed to search for superblocks are
dumpfs(8), fsdb(8), ffsinfo(8), and fsck_ffs(8). Also, the prtblknos(8)
tool found in tools/diag/prtblknos searches for superblocks.

The UFS specific mount code uses the superblock search interface
when mounting the root filesystem and when the administrator doing
a mount(8) command specifies the force flag (-f). The standalone UFS
boot code (found in stand/libsa/ufs.c) uses the superblock search
code in the hope of being able to get the system up and running so
that fsck_ffs(8) can be used to get the filesystem cleaned up.

The following utilities have not been changed to search for
superblocks: clri(8), tunefs(8), snapinfo(8), fstyp(8), quot(8),
dump(8), fsirand(8), growfs(8), quotacheck(8), gjournal(8), and
glabel(8). When these utilities fail, they do report the cause of
the failure. The one exception is the tasting code used to try and
figure what a given disk contains. The tasting code will remain
silent so as not to put out a slew of messages as it trying to taste
every new mass storage device that shows up.

1482113008 stand: Compile out the extensive superblock diagnostic messages for BIOS loader
The BIOS loader operates in a very constrained environment. The messages
for the super block integrity tests take up about 12k of space. Compile
them out for the BIOS loader, while leaving it intact for all other
loaders that aren't space constrained. These aren't used in the 'super
tiny' *boot* programs, so no adjustment is needed there.

We reply on the fact that (a) i386 doesn't support 32-bit UEFI booting
and (b) LIBSA_CPUARCH is "i386" when building on both i386 and when
we're building the 32-bit libsa32 library.

This saves about 12k of space for this constrained envrionment and will
take a bit of the pressure off some machines where the loader has grown
too big for their BIOS (see comments in i386/loader/Makefile for
details).

c6d31b8306 AST: rework
Make most AST handlers dynamically registered.  This allows to have
subsystem-specific handler source located in the subsystem files,
instead of making subr_trap.c aware of it.  For instance, signal
delivery code on return to userspace is now moved to kern_sig.c.

Also, it allows to have some handlers designated as the cleanup (kclear)
type, which are called both at AST and on thread/process exit.  For
instance, ast(), exit1(), and NFS server no longer need to be aware
about UFS softdep processing.

The dynamic registration also allows third-party modules to register AST
handlers if needed.  There is one caveat with loadable modules: the
code does not make any effort to ensure that the module is not unloaded
before all threads processed through AST handler in it.  In fact, this
is already present behavior for hwpmc.ko and ufs.ko.  I do not think it
is worth the efforts and the runtime overhead to try to fix it.

d22531d572 Identify each UFS/FFS superblock integrity check as a warning or fatal error.
Identify each of the superblock validation checks as either a
warning or a fatal error. Any integrity check that can cause a
system hang or crash is marked as fatal. Those that may simply
lead to poor file layoutor other less good operating conditions
are marked as warning.

Normally both fatal and warning are treated as errors and prevent
the superblock from being loaded. A new flag, UFS_NOWARNFAIL, is
added. When passed to ffs_sbget() it will note warnings that it
finds, but will still proceed with loading the superblock. Note
that when UFS_NOWARNFAIL is used, it also includes UFS_NOHASHFAIL.

No legitimate superblocks should fail as a result of these changes.

Eliminate several checks for non-negative values where the fields
are checked for specific values. Since these specific values are
non-negative, if the value is a verified positive value then it
cannot be negative and such a check is redundant and unnecessary.

No legitimate superblocks should fail as a result of these changes.
