Robert Watson 542a638396 General style, white space, and comment cleanup; move to ANSI C
prototypes, don't use register, etc.  Synchronize structure and
layout to the IPv4 versions of these functions to a greater extent,
making visual comparison easier.

Remove now stale or incorrect comments.

Enable full lock assertions, and correct one exception handling
case where the wrong label was jumped to.
