John Dyson 4722175765 Tie up some loose ends in vnode/object management. Remove an unneeded
config option in pmap.  Fix a problem with faulting in pages.  Clean-up
some loose ends in swap pager memory management.

The system should be much more stable, but all subtile bugs aren't fixed yet.
