binmiscctl(8): miscellaneous cleanup

- Bad whitespace in Makefile.
- Reordered headers, sys/ first.
- Annotated fatal/usage __dead2 to help `make analyze` out a little bit.
- Spell a couple of sizeof constructs as "nitems" and "howmany" instead.
