Change default revive block size to 60 kB (was 64 kB), and set a

vinum_conf, struct drive: add fields for the current number of active
requests and the maximum ever active.

struct sd: Add fields for initialize progress.

struct mc: set the size of saved file names to MCFILENAMELEN instead
of the previous explicit constant.
