44b779b337 Identify ICH9 USB controllers.
I've taken a slightly different approach than is used with the ICH8 controllers
in that each controller is not identified individually (eg USB A, USB B, etc).
Instead I've given then same description to each one even though the device ID
differs.  This can easily be changed if desired, or ICH8 (and any others using
that approach) can be made to work as this does.

e249f70262 Relax the BIOS/OS sempahore handoff code to workaround different hard
hangs (one at boot, one at shutdown) in recent machines.  First, only try
to take ownership of the EHCI controller if the BIOS currently owns the
controller.  On a HP DL160 G5, the machine hangs when we try to take
ownership.  Second, don't bother trying to give up ownership of the
controller during shutdown.  It's not strictly required and a Dell DCS S29
hangs on shutdown after the config write.

Both of these changes match the behavior of the Linux EHCI driver.  I also
think both of these hangs are caused by bugs in the BIOS' SMM handler
causing it to get stuck in an infinite loop in SMM.

will not install without the usb changes in the install kernel, so I

75d4506b87 - Only assume that a PCI device on the same slot is a "neighbor" USB
controller if it's sole child device has the "usb" device class.
  Previously ehci(4) would think that PCI-ISA bridges on the same slot
  (such as in some Intel ICHs) were "neighbors" resulting in spurious
  warnings about neighbor count mismatches.
- Fix a memory leak when looking for neighbors.

bbc84740c1 Latest round of usb cleanups:
o Consistently use device_foo_t and bus_foo_t for functions implementing
  device_foo and bus_foo respectively.  Adjust those routines that were wrong
  (we should do this throughout the tree).
o make all the modules depend on usb.  Otherwise these modules won't
  load.
o ucycom doesn't need usb_port.h
o Minor unifdefing
o uhub, umass, ums, urio, uscanner conversion complete.
o ukbd: Remove the NO_SET_PROTO quirk (fixes a PR 77940).  NetBSD removed
  their check and setting the proto a long time ago.
o umodem panic fixed.  UQ_ASSUME_CM_OVER_DATA quirk removed because I've never
  seen a umodem that needed this rejection for proection (this gets rid of
  ~20% of the quirks).

c2175ff5ca Change the remainder of the drivers for DMA'ing devices enabled in the
sparc64 GENERIC and the sound device drivers known working on sparc64
to use bus_get_dma_tag() to obtain the parent DMA tag so we can get rid
of the sparc64_root_dma_tag kludge eventually. Except for ath(4), sk(4),
stge(4) and ti(4) these changes are runtime tested (unless I booted up
the wrong kernels again...).

368030a87a Use the limited scatter-gather capabilities of ehci, ohci and uhci
host controllers to avoid the need to allocate any multi-page
physically contiguous memory blocks. This makes it possible to use
USB devices reliably on low-memory systems or when memory is too
fragmented for contiguous allocations to succeed.

The USB subsystem now uses bus_dmamap_load() directly on the buffers
supplied by USB peripheral drivers, so this also avoids having to
copy data back and forth before and after transfers. The ehci and
ohci controllers support scatter/gather as long as the buffer is
contiguous in the virtual address space. For uhci the hardware
cannot handle a physical address discontinuity within a USB packet,
so it is necessary to copy small memory fragments at times.

ATI EHCI controllers exhibit simmilar stall issues and require
	this dropped interrupts workaround. Be verbose about it.

afcb6f8261 Work around a problem seen on VIA EHCI controllers where occasionally
an interrupt appears to occur before the transfer has been marked
as completed. This caused umass transfers to get stuck, especially
when writing large files. The workaround sets up a timer that
rechecks for missed completed transfers if some operations are still
pending. Other suggested workarounds, such as performing a PCI read
immediately after acknowledging the interrupts, do not appear to
help.

2ece8174c1 Use BUS_PROBE_DEFAULT in preference to 0. Also for vx, return
BUS_PROBE_LOW_PRIORITY in stead of ifdef for devices that xl and vx
both support so that xl will snarf them on up.

4f2a7ca189 Pass an explicit pointer to the bus structure in the 'usb' device ivar
rather than a softc pointer (with the bus structure at the start).

This is a non-functional change. It just helps when reading the code to
know that the ehci, ohci and uhci drivers share the bus structure, not the
entire softc.

da37d6ad41 Attempt to follow the correct procedure for synchronising with the
system BIOS to disable legacy device emulation as per the "EHCI
Extended Capability: Pre-OS to OS Handoff Synchronisation" section
of the EHCI spec. BIOSes that implement legacy emulation using SMIs
are supposed to disable the emulation when this procedure is performed.

347934fa63 Sometimes cardbus attachments don't attach, so while we track down
this problem put these lines back in.  While they should be
unnecessary, they appear to be sometimes necessary.

0be389f3ca Remove explicit cardbus attachments from drivers where this is identical
to the pci attachment. Cardbus is a derived class of pci so all pci
drivers are automatically available for matching against cardbus devices.

This code reduces the number of trailing white space to be more in line

changes.
