ab581af779 Eliminate unused argument in PCMCIA_CARD macro. 
Provide a backwards compatible way to have the extra macro by defining
PCCARD_API_LEVEL 5 before including pccarddevs for driver writers that
want/need to have the same driver on 5 and 6 with pccard attachments.

d4fcf3cba5 Remove bus_{mem,p}io.h and related code for a micro-optimization on i386 
and amd64.  The optimization is a trivial on recent machines.

dba6dd177b Move to generating pccarddevs.h on the fly, both for the kernel and 
the modules.

Also generate usbdevs.h automatically now, but a non-kernel file is
stopping that at the moment.

ed34d0ade2 Turn on interrupt-entropy harvesting for all/any mass storage devices 
I could find. I have no doubt missed a couple.

Interrupt entropy harvesting is still conditional on the
kern.random.sys.harvest_interrupt sysctl.

bdbd54e46d Add aic to the list of drivers that might work with NEWCARD. I've added 
the same config lines that NetBSD has.  This builds with both NEWCARD
and GENERIC config files.

2a457ef118 Add support to aic for pccard attachments. Reports from testers have 
been so positive that I'm going to go ahead and commit this now rather
than do another round of patches.

My Adaptec 1460D works great with these changes.
