590adc1bc2 Remove "HMAC" from <HASH>_HMAC_BLOCK_LEN macro names
The block size is a property of the underlying hash algorithm, and has
nothing to do with the HMAC construction.

No functional change.

6573d7580b epoch(9): allow preemptible epochs to compose
- Add tracker argument to preemptible epochs
- Inline epoch read path in kernel and tied modules
- Change in_epoch to take an epoch as argument
- Simplify tfb_tcp_do_segment to not take a ti_locked argument,
there's no longer any benefit to dropping the pcbinfo lock
and trying to do so just adds an error prone branchfest to
  these functions
- Remove cases of same function recursion on the epoch as
  recursing is no longer free.
- Remove the the TAILQ_ENTRY and epoch_section from struct
thread as the tracker field is now stack or heap allocated
  as appropriate.

1fcf4de055 Incorporate bus and chip select numbers into spigen(4) cdev names. Rather
than assigning spigen device names in order of creation, this uses a device
name that corresponds to the owning spibus and chip-select index.

Example: /dev/spigen0.1 would be a child of spibus0, and use cs = 1

The intent is for systems like Raspberry Pi to have a consistent way of
using an SPI interface with a specific cs value from a user application.
Otherwise, there is no consistent way of knowing which cs pin will be
assigned to a particular spigen device. The alternative is to specify
everything in "the right order" in an overlay file, which is less than
ideal. Additionally, this duplicates (to some extent) the way Linux handles
a similar situation with their 'spidev' device, so it would be somewhat
familiar to those who also use Linux.

A new kernel config option, SPIGEN_LEGACY_CDEVNAME, causes the driver to
also create /dev/spigenN device name aliases, with N incrementing in the
order of device instantiation. This is provided to ease the transition
for existing systems using the original naming convention (particularly
when these changes are MFC'd to stable branches).

across multiple regulators (e.g. single GPIO pin which works as enable
for multiple regulates). In this case, the implementation of regulator
should perform his own enable counting therefore it is necessary to
distinguish between the regulator enable/disable method (which
increments/decrements enable counter for shared resource) and regulator
stop method (which don't affect it).

So:
- add regnode_stop() method to regulator framework and default it to
  regnode_enable(..., false, ...)
- implement it in regulator_fixed with proper enable counting.

While I'm in, also fix handling of always_on property. If any of regulators
sharing same GPIO pin have it enabled, then none of them can disable regulator.

cba506f2c1 extres/regulator: Properly refcount gpio regulators
regnode::enable_cnt is generally used to refcount regulator nodes. For
GPIOs, the refcount was done on the gpio_entry since more than one regulator
can share a GPIO.

GPIO regulators were not taking part in the node refcount, since they had
their own mechanism. This caused some fallout after manu started disabling
everybody's unused regulators in r331989.

Refcount it.

* The Linux driver enables automatic duplex and speed detection when

Because base gcc does not have a -W flag to suppress this particular

7b2c7b92be md: use prestaged mfs_root
On PowerNV systems, the rootfs is passed through kexec, which loads the rootfs
into memory and set two fdt entries to describe where the file is located in
the memory;

I need to pass this memory region to the md device as a mfs_root, but, current
md driver does not support two things:

 * Just getting a pointer from an external (bootloader) memory. If I need to
workaround it, I would need to declare a static array and memcopy from this
external memory to this static variable.

 * The size of the image. The usage of mfs_root_end, which is not a pointer,
seems to be not possible for this prestaged scenario.

This patch simply adds a new way to load mfs_root from memory.

5de96e33d6 hwpmc: support sampling both kernel and user stacks when interrupted in kernel
This adds the -U options to pmcstat which will attribute in-kernel samples
back to the user stack that invoked the system call. It is not the default,
because when looking at kernel profiles it is generally more desirable to
merge all instances of a given system call together.

Although heavily revised, this change is directly derived from D7350 by
Jonathan T. Looney.

ba96f37758 Use __builtin for various mem* and b* (e.g. bzero) routines.
Some of the routines were using artificially limited builtin already,
drop the explicit limit.

The use of builtins allows quite often allows the compiler to elide the call
or most zeroing to begin with. For instance, if the target object is 32 bytes
in size and gets zeroed + has 16 bytes initialized, the compiler can just
add code to zero out the rest.

Note not all the primites have asm variants and some of the existing ones
are not optimized. Maintaines are strongly encourage to take a look
(regardless of this change).

9729130321 Improve defaults for per-CPU kernel console colors, especially with 2
or 4 CPUs. Add a compile-time option SC_KERNEL_CONS_ATTRS to control the
defaults.

Default to color numbers in reverse order to CPU numbers (instead of
in the same order with white first and wrapping to dark grey), so that
the brightest bright colors are used first.  Don't use dark grey at all;
replace it by dark green.

Syscons has too many compile-time options, but this one is needed in
in case the defaults give something like white on white, or the user
really hates this feature and can't wait to turn it off in rc.

fa49511709 Use per-CPU attributes earlier.
The per-CPU ts is not initialized early, so the global kernel ts is used
early, but it ony has 1 (normal) attribute. Switch this to the per-CPU
attribute.

The difference is most visible with EARLY_AP_STARTUP.

Change to using the curcpu macro instead of PCPU_GET(cpuid) in 2 places for
the above and in 1 other place in my old code in syscons. The function-like
spelling is perhaps better for indicating that curcpu is volatile (unlike
curthread), but for CPU attributes volatility is a feature.

b9cedb46e2 Fix low-level locking during panics.
The SCHEDULER_STOPPED() hack breaks locking generally, and
mtx_trylock_*() especially. When mtx_trylock_*() returns nonzero,
naive code version here trusts it to have worked. But when
SCHEDULER_STOPPED() is true, mtx_trylock_*() returns 1 without doing
anything. Then mtx_unlock_*() crashes especially badly attempting to
unlock iff the error is detected, since mutex unlocking functions don't
check SCHEDULER_STOPPED().

syscons already didn't trust mtx_trylock_spin(), but it was missing the
logic to turn on sp->kdb_locked when turning off sp->mtx_locked during
panics. It also used panicstr instead of SCHEDULER_LOCKED because I
thought that panicstr was more fragile. They only differ for a window
of lines in panic(), and in broken cases where stop_cpus_hard() in panic()
didn't work.

0a15ff37d6 call AcpiLeaveSleepStatePrep after re-enabling interrupts
I want to do this change because this call (actually,
AcpiHwLegacyWakePrep) does a memory allocation and ACPI namespace
evaluation. Although it is not very likely to run into any trouble, it
is still not safe to make those calls with interrupts disabled.
witness(4) and malloc(9) do not currently check for a context with
interrupts disabled via intr_disable and we lack a facility for doing
that. So, those unsafe operations fly under the radar. But if
intr_disable in acpi_EnterSleepState was replaced with spinlock_enter
(which it probably should be), then witness and malloc would immediately
complain.

Also, AcpiLeaveSleepStatePrep is documented as called when interrupts
are enabled. It used to require disabled interrupts, but that
requirement was changed a long time ago when support for _BFS and _GTS
was removed from ACPICA.

The ACPI wakeup sequence is very sensitive to changes. I consider this
change to be correct, but there can be fallouts from it.

What AcpiHwLegacyWakePrep essentially does is writing a value
corresponding to S0 into SLP_TYPx bits of PM1 Control Register(s).
According to ACPI specifications that write should be a NOP as SLP_EN
bit is not set. But I see in some chipset specifications that they
allow to ignore SLP_EN altogether and to act on a change of SLP_TYPx
alone.

Also, there are a couple of accesses to ACPI hardware before the new
location of the call to AcpiLeaveSleepStatePrep. One is to clear the
power button status and the other is to enable SCI. So, the move may
affect the interaction between then OS and ACPI platform.

I have not seen any regressions on my test system, but it's a desktop.

e92a1350b5 hwpmc: remove unused pre-table driven bits for intel
Intel now provides comprehensive tables for all performance counters
and the various valid configuration permutations as text .json files.
Libpmc has been converted to use these and hwpmc_core has been greatly
simplified by moving to passthrough of the table values.

The one gotcha is that said tables don't support pentium pro and and pentium
IV. There's very few users of hwpmc on _amd64_ kernels on new hardware. It is
unlikely that anyone is doing low level optimization on 15 year old Intel
hardware. Nonetheless, if someone feels strongly enough to populate the
corresponding tables for p4 and ppro I will reinstate the files in to the
build.

Code for the K8 counters and !x86 architectures remains unchanged.

ec6faf94c4 add support for console resuming, implement it for uart, use on x86
This change adds a new optional console method cn_resume and a kernel
console interface cnresume. Consoles that may need to re-initialize
their hardware after suspend (e.g., because firmware does not care to do
it) will implement cn_resume. Note that it is called in rather early
environment not unlike early boot, so the same restrictions apply.
Platform specific code, for platforms that support hardware suspend,
should call cnresume early after resume, before any console output is
expected.

This change fixes a problem with a system of mine failing to resume when
a serial console is used. I found that the serial port was in a strange
configuration and an attempt to write to it likely resulted in an
infinite loop.

To avoid adding cn_resume method to every console driver, CONSOLE_DRIVER
macro has been extended to support optional methods.

279be68bfd re-synchronize TSC-s on SMP systems after resume, if necessary
The TSC-s are checked and synchronized only if they were good
originally. That is, invariant, synchronized, etc.

This is necessary on an AMD-based system where after a wakeup from STR I
see that BSP clock differs from AP clocks by a count that roughly
corresponds to one second. The APs are in sync with each other. Not
sure if this is a hardware quirk or a firmware bug.

This is what I see after a resume with this change:
SMP: passed TSC synchronization test after adjustment
    acpi_timer0: restoring timecounter, ACPI-fast -> TSC-low

f439e3a4ff Refactor NVMe CAM integration.
- Remove layering violation, when NVMe SIM code accessed CAM internal
device structures to set pointers on controller and namespace data.
Instead make NVMe XPT probe fetch the data directly from hardware.
 - Cleanup NVMe SIM code, fixing support for multiple namespaces per
controller (reporting them as LUNs) and adding controller detach support
and run-time namespace change notifications.
 - Add initial support for namespace change async events. So far only
in CAM mode, but it allows run-time namespace arrival and departure.
 - Add missing nvme_notify_fail_consumers() call on controller detach.
Together with previous changes this allows NVMe device detach/unplug.

Non-CAM mode still requires a lot of love to stay on par, but at least
CAM mode code should not stay in the way so much, becoming much more
self-sufficient.

ffe4446b33 xen-blkback: do not use state 3 (XenbusStateInitialised)
Linux will not connect to a backend that's in state 3

The protocol seems to suggest that the backend should indeed wait in

Also make sure blkback will connect to the frontend if the frontend
reaches state 3 (XenbusStateInitialised) before blkback has processed

3621ba1ede Add Intel Spec Store Bypass Disable control.
Speculative Store Bypass (SSB) is a speculative execution side channel
vulnerability identified by Jann Horn of Google Project Zero (GPZ) and
Ken Johnson of the Microsoft Security Response Center (MSRC)
https://bugs.chromium.org/p/project-zero/issues/detail?id=1528.
Updated Intel microcode introduces a MSR bit to disable SSB as a
mitigation for the vulnerability.

Introduce a sysctl hw.spec_store_bypass_disable to provide global
control over the SSBD bit, akin to the existing sysctl that controls
IBRS. The sysctl can be set to one of three values:
0: off
1: on
2: auto

Future work will enable applications to control SSBD on a per-process
basis (when it is not enabled globally).

SSBD bit detection and control was verified with prerelease microcode.

I have a system that is very unstable after resuming from suspend-to-RAM

interrupts cause a trouble for it. Originally I wanted to solve the
problem in hpet_suspend() method, but that was insufficient as the event
timer could get reprogrammed again.

So, it's better, for my case and in general, to stop the event timer(s)

4f75b93007 Avoid writing to the frame buffer in early boot on PowerPC if the CPU's
MMU is disabled.

This expands some earlier logic and avoids a number of potential problems:
1. The CPU may not be able to access the framebuffer in real mode (real
mode does not necessarily encompass all available memory, especially
   under a hypervisor).
2. Real mode accesses generally assume cacheability, so it might not
   even have worked.
3. The difference in cacheability between real mode and later (and
potentially earlier) points in the boot with the MMU on may cause
ERAT parity problems, resulting in a machine check.

This fixes real-mode (usefdt=1) early boot on the G5 iMac, which was
previously broken as a result of issue #3. Late boot will require some
other fixups.

waiting forever for the rest of the record.
