the "disappearing subdisks" problem when new subdisks can't be created
due to some errors.
This is in fact an ugly hack, but a more elegant solution would probably
require a redesign of vinum in several places.

make sure we return any allocated space to the drive. This should get
rid of a number of inconsistencies (hopefully all) that have been seen
after configuration errors.

Correct some inaccurate and badly formatted comments.
config_subdisk: If our drive is down, ensure that the subdisk is
crashed. Previously it was possible for the subdisk
to be up when the drive was down.
Change the way the plex lock mutexes work. Previously they were part
of the struct plex, which tore apart the mutex linked lists when the
plex table was expanded. Now we maintain a pool of mutexes (currently
32) to be shared by all plexes. This is still a lot better than the
splhigh() method used in other architectures.
update_volume_config: Remove redundant diskconfig parameter.

Maintain sector sizes for all objects, not just for drives. Some of
this could do with improvement: in particular, we get an error if the
components of an object have different sector sizes.
Clean up some comments.

referred to. Instead, deprecate the "prefer" keyword for volumes
(though it's still there for the moment) and add a keyword "preferred"
to the plex definition. The relationship is like this:
Old:
vol foo ... prefer foo.p3
New:
plex foo.p3 volume foo preferred
give_plex_to_volume: set preferred plex if specified on plex
definition entry. This involves adding a parameter to the function to
specify the preferred plex.
config_plex: Implement preferred keyword.

Change name of find_drive_by_dev to the more descriptive
find_drive_by_name.
Rewrite minor number decoding. Now we have only three types of
object: subdisks, plexes and volumes. The encoding for plexes and
subdisks no longer reflects the object to which they belong. The
super devices are high-order volume numbers. This gives vastly more
potential volumes (4 million instead of 256).
could already exist, and this triggers a booby trap panic in make_dev.
remove_plex_entry: Don't remove the stripe mutex here, it gets done in
free_plex.

before freeing so that WITNESS doesn't dereference mutex data pointers
and page fault. It's now possible to unload vinum.ko with a GENERIC
kernel on 5.0-CURRENT without panic.
Debugged/fixed with the aid of: jake, grog
most cases NULL is passed, but in some cases such as network driver locks
(which use the MTX_NETWORK_LOCK macro) and UMA zone locks, a name is used.
Tested on: i386, alpha, sparc64

throw_rude_remark: Make sure we're holding the config lock before
proceeding. There's no reason to assume that this
has ever happened, but the alternative might be a
double fault.
remove_sd_entry() to:
Simplify (hopefully) it by moving all error returns closer to
the beginning of the function.
Return an error when "Error removing subdisk %s: not found in
plex %s\n" would have been reported, as I doubt that we are "OK"
after printing that error message.
Adding make_dev() and destroy_dev() calls in (hopefully) the right
places.
This is done by calling make_dev() in each object constructor and
caching the dev_t's returned from make_dev() in each struct
'subdisk'(sd), 'plex' and 'volume' such that the 'object'_free()
functioncs can call destroy dev.
This change makes a subset of the old /dev/vinum appear under devfs.
Enough nodes appear such that I'm able to mount my striped volume.
There may be more work needed to get vinum configuration working
properly.

if so. This fixes problems which arose when a drive is replaced in a
running system.

loop. This caused some removals to only remove half the subdisks.

Use set_sd_state to do the work.
lockrange: correctly expand rangelock struct, including expanding a
null struct. Previously lockrange would attempt to lock a
NULL pointer under these circumstances.

checkdiskconfig(): remove. It didn't make any sense to complain about
kernel keywords in user config files; it just made it more difficult
to convert. Now we ignore kernel keywords if we're not in kernel
mode.
get_empty_sd: initialize sectors.
free_drive: don't close if we don't have a vp. Maybe this will help
fix the problem that peter had, but I wouldn't count on it.
config_plex: If the plex is RAID-5, give it a rangelock structure.
start_config: Reset current drive, plex and volume so that a new
'create' command doesn't get long-dead defaults.
is probably gone, but the debug checks remain for a while.
update_plex_config: Catch yet another divide-by-zero problem when
detaching the last subdisk from a striped plex.
