This speeds up Windows guests tremendously.
The patch does:
Add a new tuneable 'hw.vmm.vmx.use_tpr_shadowing' to disable TLP shadowing.
Also add 'hw.vmm.vmx.cap.tpr_shadowing' to be able to query if TPR shadowing is used.
Detach the initialization of TPR shadowing from the initialization of APIC virtualization.
APIC virtualization still needs TPR shadowing, but not vice versa.
Any CPU that supports APIC virtualization should also support TPR shadowing.
When TPR shadowing is used, the APIC page of each vCPU is written to the VMCS_VIRTUAL_APIC field of the VMCS
so that the CPU can write directly to the page without intercept.
On vm exit, vlapic_update_ppr() is called to update the PPR.

r357614 added CTLFLAG_NEEDGIANT to make it easier to find nodes that are
still not MPSAFE (or already are but aren’t properly marked). Use it in
preparation for a general review of all nodes.
This is non-functional change that adds annotations to SYSCTL_NODE and
SYSCTL_PROC nodes using one of the soon-to-be-required flags.

The bhyve virtual local APIC uses an instance-global flag to indicate
when an error LVT is being delivered to prevent infinite recursion.
Use a function argument instead to reduce the amount of instance-global
state.
This was inspired by reviewing the bhyve save/restore work, which
saves a copy of the instance-global state for each vlapic.

This is mostly a nop, because the vmm initializes all vCPUs up to
vm_maxcpus, so even if the target CPU is not active, lapic/vlapic code
still has the valid data to use. As John notes, dropping such
interrupts more closely matches the real harware, which ignores all
interrupts for not started APs.

Use 'struct bintime' instead of 'sbintime_t' to manage times in vPIT
to postpone rounding to final results rather than intermediate
results. In tests performed by Joyent, this reduced the error measured
by Linux guests by 59 ppm.

pci_alloc_msix() requires both the table and PBA BARs to be allocated
by the driver. ppt was only allocating the table BAR so would fail
for devices with the PBA in a separate BAR. Fix this by allocating
the PBA BAR before pci_alloc_msix() if it is stored in a separate BAR.
While here, release BARs after calling pci_release_msi() instead of
before. Also, don't call bus_teardown_intr() in error handling code
if bus_setup_intr() has just failed.

This allows replacing "sys/eventfilter.h" includes with "sys/_eventfilter.h"
in other header files (e.g., sys/{bus,conf,cpu}.h) and reduces header
pollution substantially.
EVENTHANDLER_DECLARE and EVENTHANDLER_LIST_DECLAREs were moved out of .c
files into appropriate headers (e.g., sys/proc.h, powernv/opal.h).
As a side effect of reduced header pollution, many .c files and headers no
longer contain needed definitions. The remainder of the patch addresses
adding appropriate includes to fix those files.
LOCK_DEBUG and LOCK_FILE_LINE_ARG are moved to sys/_lock.h, as required by
sys/mutex.h since r326106 (but silently protected by header pollution prior
to this change).
No functional change (intended). Of course, any out of tree modules that
relied on header pollution for sys/eventhandler.h, sys/lock.h, or
sys/mutex.h inclusion need to be fixed. __FreeBSD_version has been bumped.
Replace most VM_MAXCPU constant useses with an accessor function to
vm->maxcpus which for now is initialized and kept at the value of
VM_MAXCPUS.

For PCI device (i.e. child of a PCI bus), reset tries FLR if
implemented and worked, and falls to power reset otherwise.
For PCIe bus (child of a PCIe bridge or root port), reset
disables PCIe link and then re-trains it, performing what is known as
link-level reset.

A large number of devices don't support PCIe FLR, in particular
graphics adapters. Use PCI power management to perform the
reset if FLR fails or isn't available, by cycling the device
through the D3 state.
This has been tested by a number of users with Nvidia and AMD GPUs.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.
The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.
timer frequency a power of two. This changes the frequency from 10 to
16.7 MHz (2 ^ 24 HZ). Using a power of two avoids roundoff errors when
doing arithmetic in sbintime_t units.
Testing shows this can fix erratic ntpd behavior in guests using the
hpet timer (which is the default for multicore guests).

Specifically, devices that do not support PCI-e FLR and were not
gracefully shutdown by the guest OS could continue to issue DMA
requests after the VM was terminated. The changes in r305485 meant
that those DMA requests were completed against the host's memory which
could result in random memory corruption. Instead, leave ppt devices
that are not attached to a VM disabled in the IOMMU and only restore
the devices to the host domain if the ppt(4) driver is detached from a
device.
As an added safety belt, disable busmastering for a pass-through device
when before adding it to the host domain during ppt(4) detach.

Add routines to trigger a function level reset (FLR) of a PCI-express
device via the PCI-express device control register. This also includes
support routines to wait for pending transactions to complete as well
as calculating the maximum completion timeout permitted by a device.
Change the ppt(4) driver to reset pass through devices before attaching
to a VM during startup and before detaching from a VM during shutdown.

When the I/O MMU is active in bhyve, all PCI devices need valid entries
in the DMAR context tables. The I/O MMU code does a single enumeration
of the available PCI devices during initialization to add all existing
devices to a domain representing the host. The ppt(4) driver then moves
pass through devices in and out of domains for virtual machines as needed.
However, when new PCI devices were added at runtime either via SR-IOV or
HotPlug, the I/O MMU tables were not updated.
This change adds a new set of EVENTHANDLERS that are invoked when PCI
devices are added and deleted. The I/O MMU driver in bhyve installs
handlers for these events which it uses to add and remove devices to
the "host" domain.

This allows a pass through device to be reset to a normal device driver
on the host and reused on the host. ppt devices are now always active in
some I/O MMU domain when the I/O MMU is active, either the host domain
or the domain of a VM they are attached to.

Rather than enabling the I/O MMU when the vmm module is loaded,
defer initialization until the first attempt to pass a PCI device
through to a guest. If the I/O MMU fails to initialize or is not
present, than fail the attempt to pass a PCI device through to a
guest.
The hw.vmm.force_iommu tunable has been removed since the I/O MMU is
no longer enabled during boot. However, the I/O MMU support can be
disabled by setting the hw.vmm.iommu.enable tunable to 0 to prevent
use of the I/O MMU on any systems where it is buggy.

devmem is used to represent MMIO devices like the boot ROM or a VESA framebuffer
where doing a trap-and-emulate for every access is impractical. devmem is a
hybrid of system memory (sysmem) and emulated device models.
devmem is mapped in the guest address space via nested page tables similar
to sysmem. However the address range where devmem is mapped may be changed
by the guest at runtime (e.g. by reprogramming a PCI BAR). Also devmem is
usually mapped RO or RW as compared to RWX mappings for sysmem.
Each devmem segment is named (e.g. "bootrom") and this name is used to
create a device node for the devmem segment (e.g. /dev/vmm/testvm.bootrom).
The device node supports mmap(2) and this decouples the host mapping of
devmem from its mapping in the guest address space (which can change).

Allow the ppt driver to attach to devices that were hinted to be
passthrough devices by the PCI code creating them with a driver
name of "ppt".
Add a tunable that allows the IOMMU to be forced to be used. With
SR-IOV passthrough devices the VFs may be created after vmm.ko is
loaded. The current code will not initialize the IOMMU in that
case, meaning that the passthrough devices can't actually be used.

The new RTC emulation supports all interrupt modes: periodic, update ended
and alarm. It is also capable of maintaining the date/time and NVRAM contents
across virtual machine reset. Also, the date/time fields can now be modified
by the guest.
Since bhyve now emulates both the PIT and the RTC there is no need for
"Legacy Replacement Routing" in the HPET so get rid of it.
The RTC device state can be inspected via bhyvectl as follows:

This reduces variability during timer calibration by keeping the emulation
"close" to the guest. Additionally having all timer emulations in the kernel

As of git submit e179f6914152eca9, the Linux kernel does a simple
probe of the PIC by writing a pattern to the IMR and then reading it
back, prior to the init sequence of ICW words.
The bhyve PIC emulation wasn't allowing the IMR to be read until
the ICW sequence was complete. This limitation isn't required so
relax the test.
With this change, Linux kernels 3.15-rc2 and later won't hang
on boot when calibrating the local APIC.

processor. Briefly, the hypervisor sets V_INTR_VECTOR to the APIC vector
and sets V_IRQ to 1 to indicate a pending interrupt. The hardware then takes
care of injecting this vector when the guest is able to receive it.
Legacy PIC interrupts are still delivered via the event injection mechanism.
This is because the vector injected by the PIC must reflect the state of its
pins at the time the CPU is ready to accept the interrupt.
Accesses to the TPR via %CR8 are handled entirely in hardware. This requires
that the emulated TPR must be synced to V_TPR after a #VMEXIT.
The guest can also modify the TPR via the memory mapped APIC. This requires
that the V_TPR must be synced with the emulated TPR before a VMRUN.
