kbdmap: allow INDEX.keymaps to provide the dialog title
Previously kbdmap had a localized menu heading ("Choose your keyboard layout") but not the dialog title ("Keyboard Menu"). MFC after: 2 weeks Sponsored by: The FreeBSD Foundation
