NO_GEOM cleanup:

Move <sys/conf.h> before <sys/disk.h>.
No need for raidread()/raidwrite(), we have generic code for that.
Remove non-functional dump code.
Make raidinit() return the softc, not the dev_t.
Move to "struct disk*" centric API.
Fix printfs' to get name from struct disk instead of dev_t.
