Add a "ccd.ctl" device for config operations.
Implement ioctls on ccd.ctl which rely on the explicityly passed
unit numbers.
Update ccdconfig to use the new ccd.ctl interface.
Add code to the kernel to detect old ccdconfig binaries, and whine
about it.
Add code to ccdconfig to detect old kernels, and whine about it.
These two compatibility measures will be retained only for a limited
period since they are in the way of GEOM'ification of ccd.

Removal of sys/device.h
...
- Move intrhook stuff into kernel.h
- Remove all occurrences of #device <device.h>
- Add kernel.h were necessary (nowhere)
- delete device.h
This file contained the structures for cfdata (old style config) and is no
longer used. It was included by most drivers.
It confuses the remote debugger as the definition of 'struct device' in
device.h is found before the one in bus_private.h.

Make the long-awaited change from $Id$ to $FreeBSD$
...
This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.
Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.
