71ddd31390 Make all file-system (MFS, FFS, NFS, LFS, DEVFS) related option new-style.
This introduce an xxxFS_BOOT for each of the rootable filesystems.
(Presently not required, but encouraged to allow a smooth move of option *FS
to opt_dontuse.h later.)

LFS is temporarily disabled, and will be re-enabled tomorrow.

e868842f11 Use ENOIOCTL instead of -1 (= ERESTART) for tty ioctls that are
not handled at a particular level.  This fixes mainly restarting
of interrupted TIOCDRAINs and TIOCSETA{W,F}s.

f44f4b4ca1 Make all the documented (in pcvt(4)) options supported options. While
i was at it, do no longer insist on `PCVT_FREEBSD' being declared in
the config file, but default it to a reasonable value.

More cleanup to follow, but this part is safe for RELENG_2_2, too.

e7c3d728c1 Fix the broken VT_WAITACTIVE ioctl for the case where the third arg
was 0.

While i was at it, also fixed a broken return value for the VT_RELDISP
ioctl, iff the third arg was legally VT_TRUE, but the destination
screen was in process mode so the actual switch had to be deferred.
This was breaking the ability to directly toggle between two X servers
running on two VTs, since the server getting the bogus error return
was running wild, and competing with the other one for the hardware.
(Sigh, this was a very long-standing bug.)

2d072e5f9d Fixed the type of timeout functions and removed casts that hid the
type mismatches.  There was no problem in practice (at least on 386's).

Don't cast NULL in calls to timeout functions.  pcvt is fully prototyped
and doesn't support K&R.

Timeout functions take args of type `void *', so use magic numbers of
type `void *' for UPDATE_* to reduce the danger of wrong conversions.

Removed FreeBSD-pre-1.1-related TIMEOUT_FUNC_T macro.  It was especially
bogus for the pre-1.1 case.

553a7d9d25 Incorporate kludge/fix from `syscons' ver 1.207 and 1.209.
1) Dell Latitude XPi
This laptop has a strange, IMHO broken :-), keyboard controller which
wouldn't disable the keyboard interrupt. The kludge is to disable tty
intr. during set_keyboard(), used for changing LED and setting
typematic.

The patch also changes the function name:
    set_keyboard() -> set_keyboard_param()
Although it is a static function, the name corrides with a routine in
`syscons' and is confusing when debugging the kernel which has both
`syscons' and `pcvt' with DDB. (Suggested by Bruce)

2) doreset() bug
doreset() failed to preserve some bits in the keyboard controller's
command byte during keyboard reset. This bug may put some keyboard
controllers in old motherboards (386 and 486) in a strange state,
resulting in complete keyboard lockup or random key input.

117209856b Don't include <sys/ioctl.h> in the kernel. Stage 1: don't include
it when it is not used.  In most cases, the reasons for including it
went away when the special ioctl headers became self-sufficient.

0bc1781701 Fixed some invalid (non-atomic) accesses to `time', mostly ones of the
form `tv = time'.  Use a new function gettime().  The current version
just forces atomicicity without fixing precision or efficiency bugs.
Simplified some related valid accesses by using the central function.

dafbb728f2 Fix a bogon in pcvt that caused a characterset designation to not take
effect immediately, but required a following (normally redundant) G0
into GL mapping.  This adds one layer of indirection (thus might make it
slower), but fixes the broken box character drawing in pcvt.

Hellmuth and Bruce are unfortunately too busy too review this right now,
but i wanna have it in 2.2 since it has often been asked in the past.

70864abe49 Move pcvt towards using kbdio (finally!).
Warning: this won't work yet with PCVT_SCANSET=2 along in early
console mode (boot -c, or boot -d).

A big thanks to Kazutaka, and a word of apologies for delaying the
review for that long time...

2a670d60f7 Fixed struct access bug and style bugs in previous revision. The same
fix as for syscons did not work because pcvt uses a struct where syscons
uses a struct pointer.

0a4f73a54d add the same fix to pcvt that I did to syscons. Make sure that the signals
provided by the user are valid else return EINVAL.

9434a8b4e5 Avoid touching the LEDs too early in the game. After the recent
changes to the keyboard code in pcvt, this update_led() very often
caused pcvt to hang early at boot time.

(Eventually, a better solution should be found, but the simple
omission serves well as a workaround for something that is actually a
show-stopper class problem.)

Candidate for 2.2.

5a3b6bd7f9 Fixed setting of the keyboard LEDs. For some keyboards and/or keyboard
controllers, it is an error to issue a command before the keyboard
has had time to reply to the previous command.  Setting the LEDs
involves issueing 2 commands, so it never worked on these keyboards.

Fixed resetting of keyboard.  It is possible for unprocessed
scancodes to be present when the reset routine is called.  This
usually occurs after switching from one console driver to another
in userconfig.  pcvt and syscons attempt to flush any stale scancodes,
but sometimes fail to do so because keyboard and/or keyboard
controller takes a long time to send the scancodes after reset
(scancodes are apparently not flushed by reset!).  syscons handles
this later by not checking for errors at strategic places, but pcvt
was confused.

Use an impossible initial and failure mode setting for the LEDs
so that the LEDs always get set if they are possibly out of sync.

Added missing spltty() in update_led().

e8d192466f Don't claim the console when the driver is disabled. The getc/putc
part of the console driver usually works when the driver is disabled,
but the normal read/write part doesn't (it caused a panic).

68592e6b3e Changed cncheckc() interface so that it is 8-bit clean - return -1
instead of 0 if there is no input.

pcvt_drv.c:
Partially fixed pccncheckc().  It returned a boolean value instead of
the character that it fetches from the input fifo (if any).  I think
it still discards characters after the first for multi-char input.

dcde70f16b Fix two minor oddities introduced by my yesterday's patches:
. preserve a multi-char sequence in a small static buffer inside
  pccngetc(), so it won't be clobbered later (used to happen when
  breaking into DDB user Ctrl-Alt-ESC), and

. simplify the ``keystroke is present'' determination in sgetc(), thus
  making pccncheck() actually working without waiting for a keystroke.

b0b6f5d7b9 Fix many long-standing bugs and problems with pcvt, namely:
. make pccncheck() work even when interrupts are disabled, so the
  ``Press a key on the console...'' procedure will work,
. make kernel colors #ifndef, so they can be overridden from the
  config file,
. use shutdown_nice() instead of cpu_reset() if Ctrl-Alt-Del is
  enabled,
. allow pccngetc() to return more than a single character, so the
  arrow keys will work (and thus visual UserConfig!),
. fix a warning.

This closes all know PRs related to pcvt, in particular #845, #1236,
and #1265.  PR #991 is a duplicate for 845, and PR #1283 has already
been fixed earlier in rev 1.11 of pcvt_conf.h.

c3e6222aa5 Get rid of two and a half printf in the kernel.
Add more features to the one remaining to handle the job:
	+	signed quantity.
	#	alternate format
	-	left padding
	*	read width as next arg.
	n	numeric in (argument specified) default radix.

Fix the DDB debugger to use these.
Use vprintf in debug routine in pcvt.

The warnings from gcc may become more wrong and  intolerable because
of this.

Warning:  I have not checked the entire source for unsupported or
changed constructs, but generally belive that there are only a few.

956b9c95d2 Removed my devsw access functions [un]register_cdev() and
getmajorbyname() which were a better (sigh) temporary interface to
the going-away devswitches.

Note that SYSINIT()s to initialize the devswitches would be fatal
in syscons.c and pcvt_drv.c (and are bogus elsewhere) because they
get called independently of whether the device is attached; thus
devices that share a major clobber each other's devswitch entries
until the last one wins.

conf.c:
Removed stale #includes and comments.

1900eea896 Pass 3 of the great devsw changes
most devsw referenced functions are now static, as they are
in the same file as their devsw structure. I've also added DEVFS
support for nearly every device in the system, however
many of the devices have 'incorrect' names under DEVFS
because I couldn't quickly work out the correct naming conventions.
(but devfs won't be coming on line for a month or so anyhow so that doesn't
matter)

If you "OWN" a device which would normally have an entry in /dev
then search for the devfs_add_devsw() entries and munge to make them right..
check out similar devices to see what I might have done in them in you
can't see what's going on..
for a laugh compare conf.c conf.h defore and after... :)
I have not doen DEVFS entries for any DISKSLICE devices yet as that will be
a much more complicated job.. (pass 5 :)

pass 4 will be to make the devsw tables of type (cdevsw * )
rather than (cdevsw)
seems to work here..
complaints to the usual places.. :)

c0273b2f64 Replaced #includes of <sys/user.h> by less gross headers, usually
<sys/vm.h>.  Many device drivers need only the definition of vtophys()
from vm.

Added nearby #includes of <sys/conf.h> where appropriate.

a01aa94291 Add /dev/random hooks to the key input routines for pcvt. This allows
the scancodes on a PCVT machine to be used as stochastic input as well
as SysCons..

6073486300 Added `#include "ioconf.h"' to <machine/conf.h> and cleaned up the
misplaced extern declarations (mostly prototypes of interrupt handlers)
that this exposed.  The prototypes should be moved back to the driver
sources when the functions are staticalized.

Added idempotency guards to <machine/conf.h>.  "ioconf.h" can't be
included when building LKMs so define a wart in bsd.kmod.mk to help
guard against including it.
