ea13b9803b Consistently mark std(in|out|err) with .Dv, because that's how they
are marked up in stdio(3), and because they are defined expressions
of type "FILE *".

867a23440c Cross-reference between byte and wide-character stdio functions. Remove
references to fputwc() and fgetwc() being macros while I'm at it.

f7a8bc4c04 Add $Id$, to make it simpler for members of the translation teams to
track.

The $Id$ line is normally at the bottom of the main comment block in the
man page, separated from the rest of the manpage by an empty comment,
like so;

     .\"    $Id$
     .\"

If the immediately preceding comment is a @(#) format ID marker than the
the $Id$ will line up underneath it with no intervening blank lines.
Otherwise, an additional blank line is inserted.
