o <machine/ansi.h> will continue to live so it can define MD clock
macros, which are only MD because of gratuitous differences between
architectures.
o Change all headers to make use of this. This mainly involves

since it is impossible to distinguish unpromoted types from small
(struct) types. Renamed __va_promote() to __va_size() since it is
related to sizes of args on the stack and not to promotion.

This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.
Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.
