fa3e727156 Provide a nda man page. Add cross referneces. Bump dates.
Also, CAM is a storage subsystem, not a SCSI/ATA one these days.

bdf9a5ddfd kern.cam.da.X.delete_method: add -width to .Bl macro call
Now the section width is set appropriately per the BIO_DELETE
parameter being described.

0553238f16 Make delete method set via kern.cam.da.X.delete_method persistent.
This allows to set delete method via tunable, before device capabilities
are known.  Also allow ZERO method for devices not reporting LBP, if user
explicitly requests it -- it may be useful if storage supports compression
and WRITE SAME, but does not support UNMAP.

c05bbe4d23 It's 2015, and some people are still trying to use fdisk and then
go asking what debug flags to set for GEOM to make it work.  Advice
them to use gpart(8) instead.

Something similar should probably done with disklabel,
but I need to rewrite the disklabel examples first.

067557e546 - Axe the PARTITIONING and IOCTLS section as this has been made obsolete
now that all that stuff has been abstracted out of the disk drivers with
  GEOM.
- Reference bsdlabel(8) rather than disklabel(8).

03d0445c16 Move dynamic sysctl(8) variable creation for the cd(4) and da(4) drivers
out of cdregister() and daregister(), which are run from interrupt context.

The sysctl code does blocking mallocs (M_WAITOK), which causes problems
if malloc(9) actually needs to sleep.

The eventual fix for this issue will involve moving the CAM probe process
inside a kernel thread.  For now, though, I have fixed the issue by moving
dynamic sysctl variable creation for these two drivers to a task queue
running in a kernel thread.

The existing task queues (taskqueue_swi and taskqueue_swi_giant) run in
software interrupt handlers, which wouldn't fix the problem at hand.  So I
have created a new task queue, taskqueue_thread, that runs inside a kernel
thread.  (It also runs outside of Giant -- clients must explicitly acquire
and release Giant in their taskqueue functions.)

scsi_cd.c:	Remove sysctl variable creation code from cdregister(), and
		move it to a new function, cdsysctlinit().  Queue
		cdsysctlinit() to the taskqueue_thread taskqueue once we
		have fully registered the cd(4) driver instance.

scsi_da.c:	Remove sysctl variable creation code from daregister(), and
		move it to move it to a new function, dasysctlinit().
		Queue dasysctlinit() to the taskqueue_thread taskqueue once
		we have fully registered the da(4) instance.

taskqueue.h:	Declare the new taskqueue_thread taskqueue, update some
		comments.

subr_taskqueue.c:
		Create the new kernel thread taskqueue.  This taskqueue
		runs outside of Giant, so any functions queued to it would
		need to explicitly acquire/release Giant if they need it.

cd.4:		Update the cd(4) man page to talk about the minimum command
		size sysctl/loader tunable.  Also note that the changer
		variables are available as loader tunables as well.

da.4:		Update the da(4) man page to cover the retry_count,
		default_timeout and minimum_cmd_size sysctl variables/loader
		tunables.  Remove references to /dev/r???, they aren't used
		any longer.

cd.9:		Update the cd(9) man page to describe the CD_Q_10_BYTE_ONLY
		quirk.

taskqueue.9:	Update the taskqueue(9) man page to describe the new thread
		task queue, and the taskqueue_swi_giant queue.

0dd51f62a8 Update man pages in the section 4 to match the reality in -CURRENT.
- Change lines referring to kernel configuration file:
        device foo0 at isa port xxx irq yyy...
  to
        device foo
  Describe resource "hints" in /boot/device.hints.

- Try to describe resource allocation and probe/attach behavior in the
  newbus framework.

39c95a7322 Consistently use 'queueing' instead of 'queuing'. Both spellings are
correct, but the former seems to be preferred.  This makes all of the
section-4 manual pages consistent in this respect.

b45b9e3cde Remove single-space hard sentence breaks. These degrade the quality
of the typeset output, tend to make diffs harder to read and provide
bad examples for new-comers to mdoc.

4ee5733cc5 Devices are hardwired with "unit 0", not "lun 0" as was in these man pages.
Also, update the pass(4) man page to indicate that some CCBs must be
sent through the xpt(4) device and cannot be sent through a pass device.

808a36ef65 Make the long-awaited change from $Id$ to $FreeBSD$
This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.

Boy, I'm glad we're not using sup anymore.  This update would have been
insane otherwise.
