c17f8bfdd5 Add the atomic_thread_fence() family of functions with intent to
provide a semantic defined by the C11 fences with corresponding
memory_order.

atomic_thread_fence_acq() gives r | r, w, where r and w are read and
write accesses, and | denotes the fence itself.

atomic_thread_fence_rel() is r, w | w.

atomic_thread_fence_acq_rel() is the combination of the acquire and
release in single operation.  Note that reads after the acq+rel fence
could be made visible before writes preceeding the fence.

atomic_thread_fence_seq_cst() orders all accesses before/after the
fence, and the fence itself is globally ordered against other
sequentially consistent atomic operations.

67f2eebb44 Generalised support for copy-on-write structures shared by threads.
Thread credentials are maintained as follows: each thread has a pointer to
creds and a reference on them. The pointer is compared with proc's creds on
userspace<->kernel boundary and updated if needed.

This patch introduces a counter which can be compared instead, so that more
structures can use this scheme without adding more comparisons on the boundary.

0969667a9e The kernel sends signals to the processes via ABI specific sv_sendsig method.
Native ABI do not need signal conversion, only emulators may want this. Usually
emulators implements its own sv_sendsig method. For now only ibcs2 emulator does
not have own sv_sendsig implementation and depends on native sendsig() method.
So, remove any extra attempts to convert signal numbers from native sendsig()
methods except from i386 where ibsc2 is living.

b0d837707d ddb: finish converting boolean values.
The replacement started at r283088 was necessarily incomplete without
replacing boolean_t with bool.  This also involved cleaning some type
mismatches and ansifying old C function declarations.

34a1e2f18c Update this driver to not save copies of registers that are no longer used
after r281874.  While here, also update it to always write the parent's

e2a65d5cfa Add support for the uart classes to set their default register shift value.
This is needed with the pl011 driver. Before this change it would default
to a shift of 0, however the hardware places the registers at 4-byte
addresses meaning the value should be 2.

This patch fixes this for the pl011 when configured using the fdt. The
other drivers have a default value of 0 to keep this a no-op.

148355cbb6 Move the 32-bit compatible procfs types from freebsd32.h to <sys/procfs.h>
and export them to userland.
- Define __HAVE_REG32 on platforms that define a reg32 structure and check
  for this in <sys/procfs.h> to control when to export prstatus32, etc.
- Add prstatus32_t and prpsinfo32_t typedefs for the 32-bit structures.
  libbfd looks for these types, and having them fixes 'gcore' in gdb of a
  32-bit process on a 64-bit platform.
- Use the structure definitions from <sys/procfs.h> in gcore's elf32 core
  dump code instead of duplicating the definitions.

57feb6fb43 Fix integer truncation bug in malloc(9)
A couple of internal functions used by malloc(9) and uma truncated
a size_t down to an int.  This could cause any number of issues
(e.g. indefinite sleeps, memory corruption) if any kernel
subsystem tried to allocate 2GB or more through malloc.  zfs would
attempt such an allocation when run on a system with 2TB or more
of RAM.

Note to self: When this is MFCed, sparc64 needs the same fix.

3bbc91d138 Do not qualify the mcontext_t *mcp argument for set_mcontext(9) as
const.  On x86, even after the machine context is supposedly read into
the struct ucontext, lazy FPU state save code might only mark the FPU
data as hardware-owned.  Later, set_fpcontext() needs to fetch the
state from hardware, modifying the *mcp.

The set_mcontext(9) is called from sigreturn(2) and setcontext(2)
implementations and old create_thread(2) interface, which throw the
*mcp out after the set_mcontext() call.

7e7e145818 Factor out duplicated code from dumpsys() on each architecture into generic
code in sys/kern/kern_dump.c. Most dumpsys() implementations are nearly
identical and simply redefine a number of constants and helper subroutines;
a generic implementation will make it easier to implement features around
kernel core dumps. This change does not alter any minidump code and should
have no functional impact.

d5b2a401e7 This configuration file removes several debugging options, including
WITNESS and INVARIANTS checking, which are known to have significant
performance impact on running systems.  When benchmarking new features
this kernel should be used instead of the standard GENERIC.
This kernel configuration should never appear outside of the HEAD
of the FreeBSD tree.

b5d711d3a6 Renove faith(4) and faithd(8) from base. It looks like industry
have chosen different (and more traditional) stateless/statuful
NAT64 as translation mechanism. Last non-trivial commits to both
faith(4) and faithd(8) happened more than 12 years ago, so I assume
it is time to drop RFC3142 in FreeBSD.

29a659ef8e Add fueword(9) and casueword(9) functions. They are like fuword(9)
and casuword(9), but do not mix value read and indication of fault.

I know (or remember) enough assembly to handle x86 and powerpc.  For
arm, mips and sparc64, implement fueword() and casueword() as wrappers
around fuword() and casuword(), which means that the functions cannot
distinguish between -1 and fault.

On architectures where fueword() and casueword() are native, implement
fuword() and casuword() using fueword() and casuword(), to reduce
assembly code duplication.

494dc32ba6 ddb: allow specifying the exact address of the symtab and strtab
When the FreeBSD kernel is loaded from Xen the symtab and strtab are
not loaded the same way as the native boot loader. This patch adds
three new global variables to ddb that can be used to specify the
exact position and size of those tables, so they can be directly used
as parameters to db_add_symbol_table. A new helper is introduced, so callers
that used to set ksym_start and ksym_end can use this helper to set the new
variables.

It also adds support for loading them from the Xen PVH port, that was
previously missing those tables.

e4c630d701 Migrate ie->ie_assign_cpu and associated code to use an int for CPU rather
than u_char.

Migrate post_filter to use an int for a CPU rather than u_char.

Change intr_event_bind() to use an int for CPU rather than u_char.

It touches the ppc, sparc64, arm and mips machdep code but it should
(hah!) be a no-op.

3a8cf1a38b Create a separate structure for per-CPU state saved across suspend and
resume that is a superset of a pcb.  Move the FPU state out of the pcb and
into this new structure.  As part of this, move the FPU resume code on
amd64 into a C function.  This allows resumectx() to still operate only on
a pcb and more closely mirrors the i386 code.

094158b3f2 Change pmap_enter(9) interface to take flags parameter and superpage
mapping size (currently unused).  The flags includes the fault access
bits, wired flag as PMAP_ENTER_WIRED, and a new flag
PMAP_ENTER_NOSLEEP to indicate that pmap should not sleep.

For powerpc aim both 32 and 64 bit, fix implementation to ensure that
the requested mapping is created when PMAP_ENTER_NOSLEEP is not
specified, in particular, wait for the available memory required to
proceed.

991a1f338d Merge all MD sf_buf allocators into one MI, residing in kern/subr_sfbuf.c
The MD allocators were very common, however there were some minor
differencies. These differencies were all consolidated in the MI allocator,
under ifdefs. The defines from machine/vmparam.h turn on features required
for a particular machine. For details look in the comment in sys/sf_buf.h.

As result no MD code left in sys/*/*/vm_machdep.c. Some arches still have
machine/sf_buf.h, which is usually quite small.

38b6c535da Retire pmap_change_wiring(). We have never used it to wire virtual pages.
We continue to use pmap_enter() for that.  For unwiring virtual pages, we
now use pmap_unwire(), which unwires a range of virtual addresses instead
of a single virtual page.

3d6d8cd540 Add vt(4) support to sparc64. The only driver currently present (ofwfb)
provides support for a variety of low-end graphics hardware (SBus adapters,
Mach64, QEMU's framebuffer, XVR-100). A driver for at least the Creator3D
cards will have to be present before this can become the default console
driver.

To test vt(4) on sparc64, set kern.vty=vt at the loader prompt.

d74e85dbb9 Introduce pmap_unwire(). It will replace pmap_change_wiring(). There are
several reasons for this change:

pmap_change_wiring() has never (in my memory) been used to set the wired
attribute on a virtual page.  We have always used pmap_enter() to do that.
Moreover, it is not really safe to use pmap_change_wiring() to set the wired
attribute on a virtual page.  The description of pmap_change_wiring() says
that it assumes the existence of a mapping in the pmap.  However, non-wired
mappings may be reclaimed by the pmap at any time.  (See pmap_collect().)
Many implementations of pmap_change_wiring() will crash if the mapping does
not exist.

pmap_unwire() accepts a range of virtual addresses, whereas
pmap_change_wiring() acts upon a single virtual page.  Since we are
typically unwiring a range of virtual addresses, pmap_unwire() will be more
efficient.  Moreover, pmap_unwire() allows us to unwire superpage mappings.
Previously, we were forced to demote the superpage mapping, because
pmap_change_wiring() only allowed us to express the unwiring of a single
base page mapping at a time.  This added to the overhead of unwiring for
large ranges of addresses, including the implicit unwiring that occurs at
process termination.

Implementations for arm and powerpc will follow.

bd1ed65f0f Extend the meaning of the CTLFLAG_TUN flag to automatically check if
there is an environment variable which shall initialize the SYSCTL
during early boot. This works for all SYSCTL types both statically and
dynamically created ones, except for the SYSCTL NODE type and SYSCTLs
which belong to VNETs. A new flag, CTLFLAG_NOFETCH, has been added to
be used in the case a tunable sysctl has a custom initialisation
function allowing the sysctl to still be marked as a tunable. The
kernel SYSCTL API is mostly the same, with a few exceptions for some
special operations like iterating childrens of a static/extern SYSCTL
node. This operation should probably be made into a factored out
common macro, hence some device drivers use this. The reason for
changing the SYSCTL API was the need for a SYSCTL parent OID pointer
and not only the SYSCTL parent OID list pointer in order to quickly
generate the sysctl path. The motivation behind this patch is to avoid
parameter loading cludges inside the OFED driver subsystem. Instead of
adding special code to the OFED driver subsystem to post-load tunables
into dynamically created sysctls, we generalize this in the kernel.

Other changes:
- Corrected a possibly incorrect sysctl name from "hw.cbb.intr_mask"
to "hw.pcic.intr_mask".
- Removed redundant TUNABLE statements throughout the kernel.
- Some minor code rewrites in connection to removing not needed
TUNABLE statements.
- Added a missing SYSCTL_DECL().
- Wrapped two very long lines.
- Avoid malloc()/free() inside sysctl string handling, in case it is
called to initialize a sysctl from a tunable, hence malloc()/free() is
not ready when sysctls from the sysctl dataset are registered.
- Bumped FreeBSD version to indicate SYSCTL API change.

8f3f80c382 Bring in the mpr(4) driver for LSI's MPT3 12Gb SAS controllers.
This is derived from the mps(4) driver, but it supports only the 12Gb
IT and IR hardware including the SAS 3004, SAS 3008 and SAS 3108.

Some notes about this driver:
 o The 12Gb hardware can do "FastPath" I/O, and that capability is included in
   this driver.

 o WarpDrive functionality has been removed, since it isn't supported in
   the 12Gb driver interface.

 o The Scatter/Gather list handling code is significantly different between
   the 6Gb and 12Gb hardware.  The 12Gb boards support IEEE Scatter/Gather
   lists.

Thanks to LSI for developing and testing this driver for FreeBSD.

62a64f0d2b Retire smp_active. It was racey and caused demonstrated problems with
the cpufreq code.  Replace its use with smp_started.  There's at least
one userland tool that still looks at the kern.smp.active sysctl, so
preserve it but point it to smp_started as well.

bd031ca10c Rather than require a makeoptions DEBUG to get debug correct,
add it in kern.mk, but only if we're using clang. While this
option is supported by both clang and gcc, in the future there
may be changes to clang which change the defaults that require
a tweak to build our kernel such that other tools in our tree
will work. Set a good example by forcing -gdwarf-2 only for
clang builds, and only if the user hasn't specified another
dwarf level already. Update UPDATING to reflect the changed
state of affairs. This also keeps us from having to update
all the ARM kernels to add this, and also keeps us from
in the future having to update all the MIPS kernels and is
one less place the user will have to know to do something
special for clang and one less thing developers will need
to do when moving an architecture to clang.

6fcf6199a4 Rename global cnt to vm_cnt to avoid shadowing.
To reduce the diff struct pcu.cnt field was not renamed, so
PCPU_OP(cnt.field) is still used. pc_cnt and pcpu are also used in
kvm(3) and vmstat(8). The goal was to not affect externally used KPI.

Bump __FreeBSD_version_ in case some out-of-tree module/code relies on the
the global cnt variable.

Exp-run revealed no ports using it directly.

6e6e271c34 Add support for managing PCI bus numbers. As with BARs and PCI-PCI bridge
I/O windows, the default is to preserve the firmware-assigned resources.
PCI bus numbers are only managed if NEW_PCIB is enabled and the architecture
defines a PCI_RES_BUS resource type.
- Add a helper API to create top-level PCI bus resource managers for each
  PCI domain/segment.  Host-PCI bridge drivers use this API to allocate
  bus numbers from their associated domain.
- Change the PCI bus and CardBus drivers to allocate a bus resource for
  their bus number from the parent PCI bridge device.
- Change the PCI-PCI and PCI-CardBus bridge drivers to allocate the
  full range of bus numbers from secbus to subbus from their parent bridge.
  The drivers also always program their primary bus register.  The bridge
  drivers also support growing their bus range by extending the bus resource
  and updating subbus to match the larger range.
- Add support for managing PCI bus resources to the Host-PCI bridge drivers
  used for amd64 and i386 (acpi_pcib, mptable_pcib, legacy_pcib, and qpi_pcib).
- Define a PCI_RES_BUS resource type for amd64 and i386.

4ac2e7d8d9 Make process descriptors standard part of the kernel. rwhod(8) already
requires process descriptors to work and having PROCDESC in GENERIC
seems not enough, especially that we hope to have more and more consumers
in the base.

6c60c88452 As of r257209, all architectures have defined VM_KMEM_SIZE_SCALE. In other
words, every architecture is now auto-sizing the kmem arena.  This revision
changes kmeminit() so that the definition of VM_KMEM_SIZE_SCALE becomes
mandatory and the definition of VM_KMEM_SIZE becomes optional.

Replace or eliminate all existing definitions of VM_KMEM_SIZE.  With
auto-sizing enabled, VM_KMEM_SIZE effectively became an alternate spelling
for VM_KMEM_SIZE_MIN on most architectures.  Use VM_KMEM_SIZE_MIN for
clarity.

Change kmeminit() so that the effect of defining VM_KMEM_SIZE is similar to
that of setting the tunable vm.kmem_size.  Whereas the macros
VM_KMEM_SIZE_{MAX,MIN,SCALE} have had the same effect as the tunables
vm.kmem_size_{max,min,scale}, the effects of VM_KMEM_SIZE and vm.kmem_size
have been distinct.  In particular, whereas VM_KMEM_SIZE was overridden by
VM_KMEM_SIZE_{MAX,MIN,SCALE} and vm.kmem_size_{max,min,scale}, vm.kmem_size
was not.  Remedy this inconsistency.  Now, VM_KMEM_SIZE can be used to set
the size of the kmem arena at compile-time without that value being
overridden by auto-sizing.

Update the nearby comments to reflect the kmem submap being replaced by the
kmem arena.  Stop duplicating the auto-sizing formula in every machine-
dependent vmparam.h and place it in kmeminit() where auto-sizing takes
place.

04bb6e10cd Add a mmap flag (MAP_32BIT) on 64-bit platforms to request that a mapping use
an address in the first 2GB of the process's address space.  This flag should
have the same semantics as the same flag on Linux.

To facilitate this, add a new parameter to vm_map_find() that specifies an
optional maximum virtual address.  While here, fix several callers of
vm_map_find() to use a VMFS_* constant for the findspace argument instead of
TRUE and FALSE.

aa9a7bb9e6 Significantly reduce the cost, i.e., run time, of calls to madvise(...,
MADV_DONTNEED) and madvise(..., MADV_FREE).  Specifically, introduce a new
pmap function, pmap_advise(), that operates on a range of virtual addresses
within the specified pmap, allowing for a more efficient implementation of
MADV_DONTNEED and MADV_FREE.  Previously, the implementation of
MADV_DONTNEED and MADV_FREE relied on per-page pmap operations, such as
pmap_clear_reference().  Intuitively, the problem with this implementation
is that the pmap-level locks are acquired and released and the page table
traversed repeatedly, once for each resident page in the range
that was specified to madvise(2).  A more subtle flaw with the previous
implementation is that pmap_clear_reference() would clear the reference bit
on all mappings to the specified page, not just the mapping in the range
specified to madvise(2).

Since our malloc(3) makes heavy use of madvise(2), this change can have a
measureable impact.  For example, the system time for completing a parallel
"buildworld" on a 6-core amd64 machine was reduced by about 1.5% to 2.0%.

Note: This change only contains pmap_advise() implementations for a subset
of our supported architectures.  I will commit implementations for the
remaining architectures after further testing.  For now, a stub function is
sufficient because of the advisory nature of pmap_advise().

05a9dff802 Revert r254501. Instead, reuse the type stability of the struct pmap
which is the part of struct vmspace, allocated from UMA_ZONE_NOFREE
zone.  Initialize the pmap lock in the vmspace zone init function, and
remove pmap lock initialization and destruction from pmap_pinit() and
pmap_release().

16c7563cf4 The soft and hard busy mechanism rely on the vm object lock to work.
Unify the 2 concept into a real, minimal, sxlock where the shared
acquisition represent the soft busy and the exclusive acquisition
represent the hard busy.
The old VPO_WANTED mechanism becames the hard-path for this new lock
and it becomes per-page rather than per-object.
The vm_object lock becames an interlock for this functionality:
it can be held in both read or write mode.
However, if the vm_object lock is held in read mode while acquiring
or releasing the busy state, the thread owner cannot make any
assumption on the busy state unless it is also busying it.

Also:
- Add a new flag to directly shared busy pages while vm_page_alloc
  and vm_page_grab are being executed.  This will be very helpful
  once these functions happen under a read object lock.
- Move the swapping sleep into its own per-object flag

The KPI is heavilly changed this is why the version is bumped.
It is very likely that some VM ports users will need to change
their own code.

8de1718b60 Split the pagequeues per NUMA domains, and split pageademon process
into threads each processing queue in a single domain.  The structure
of the pagedaemons and queues is kept intact, most of the changes come
from the need for code to find an owning page queue for given page,
calculated from the segment containing the page.

The tie between NUMA domain and pagedaemon thread/pagequeue split is
rather arbitrary, the multithreaded daemon could be allowed for the
single-domain machines, or one domain might be split into several page
domains, to further increase concurrency.

Right now, each pagedaemon thread tries to reach the global target,
precalculated at the start of the pass.  This is not optimal, since it
could cause excessive page deactivation and freeing.  The code should
be changed to re-check the global page deficit state in the loop after
some number of iterations.

The pagedaemons reach the quorum before starting the OOM, since one
thread inability to meet the target is normal for split queues.  Only
when all pagedaemons fail to produce enough reusable pages, OOM is
started by single selected thread.

Launder is modified to take into account the segments layout with
regard to the region for which cleaning is performed.

95f847fd26 Add MD (for now) atomic_store_acq_<type>() and use it in pmap_activate()
to get the semantics when setting the PMAP right. Prior to r251782, the
latter already used implicit acquire semantics, which - currently - means
to not employ additional explicit memory barriers under the hood (see also
r225889).

721ce839c7 Decouple yarrow from random(4) device.
* Make Yarrow an optional kernel component -- enabled by "YARROW_RNG" option.
  The files sha2.c, hash.c, randomdev_soft.c and yarrow.c comprise yarrow.

* random(4) device doesn't really depend on rijndael-*.  Yarrow, however, does.

* Add random_adaptors.[ch] which is basically a store of random_adaptor's.
  random_adaptor is basically an adapter that plugs in to random(4).
  random_adaptor can only be plugged in to random(4) very early in bootup.
  Unplugging random_adaptor from random(4) is not supported, and is probably a
  bad idea anyway, due to potential loss of entropy pools.
  We currently have 3 random_adaptors:
  + yarrow
  + rdrand (ivy.c)
  + nehemeiah

* Remove platform dependent logic from probe.c, and move it into
  corresponding registration routines of each random_adaptor provider.
  probe.c doesn't do anything other than picking a specific random_adaptor
  from a list of registered ones.

* If the kernel doesn't have any random_adaptor adapters present then the
  creation of /dev/random is postponed until next random_adaptor is kldload'ed.

* Fix randomdev_soft.c to refer to its own random_adaptor, instead of a
  system wide one.

8a0279994d Fix issues with zeroing and fetching the counters, on x86 and ppc64.
Issues were noted by Bruce Evans and are present on all architectures.

On i386, a counter fetch should use atomic read of 64bit value,
otherwise carry from the increment on other CPU could be lost for the
given fetch, making error of 2^32.  If 64bit read (cmpxchg8b) is not
available on the machine, it cannot be SMP and it is enough to disable
preemption around read to avoid the split read.

On x86 the counter increment is not atomic on purpose, which makes it
possible for the store of the incremented result to override just
zeroed per-cpu slot.  The effect would be a counter going off by
arbitrary value after zeroing.  Perform the counter zeroing on the
same processor which does the increments, making the operations
mutually exclusive.  On i386, same as for the fetching, if the
cmpxchg8b is not available, machine is not SMP and we disable
preemption for zeroing.

PowerPC64 is treated the same as amd64.

For other architectures, the changes made to allow the compilation to
succeed, without fixing the issues with zeroing or fetching.  It
should be possible to handle them by using the 64bit loads and stores
atomic WRT preemption (assuming the architectures also converted from
using critical sections to proper asm).  If architecture does not
provide the facility, using global (spin) mutex would be non-optimal
but working solution.

1fdf0e5b3c Remove conflicting macros from SPARC64's atomic(9) header.
The atomic_load() and atomic_store() macros conflict with the equally
named macros from <stdatomic.h>. Remove them, as they are only used to
implement functions that are not present on any of the other
architectures.
