346273b3b2 Remove MAXBSIZE use from drivers where it has nothing to do.
In some cases limits are just not needed, in others -- DFLTPHYS is the
right constant to use instead.

eebc91c3f0 Remove instances of variables that were set, but never used. gcc 4.9
warns about these by default.

33fdc14c0c Update kernel inclusions of capability.h to use capsicum.h instead; some
further refinement is required as some device drivers intended to be
portable over FreeBSD versions rely on __FreeBSD_version to decide whether
to include capability.h.

28bd1409da Update PCI drivers to no longer look at the MEMIO-enabled bit in the PCI
command register.  The lazy BAR allocation code in FreeBSD sometimes
disables this bit when it detects a range conflict, and will re-enable
it on demand when a driver allocates the BAR.  Thus, the bit is no longer
a reliable indication of capability, and should not be checked.  This
results in the elimination of a lot of code from drivers, and also gives
the opportunity to simplify a lot of drivers to use a helper API to set
the busmaster enable bit.

This changes fixes some recent reports of disk controllers and their
associated drives/enclosures disappearing during boot.

9d137ef1b1 Unbreak ia64 tinderbox: 64-bit support was made conditional upon
__amd64__, and thus limited. Eliminate 2 trivial conditionals by
casting the 64-bit integral, holding an address, via (uintptr_t)
to (void *) and replace the last remaining check for __amd64__
with a check for __LP64__ instead.

10ab667100 Driver 'aacraid' added. Supports Adaptec by PMC RAID controller families Series 6, 7, 8 and upcoming products. Older Adaptec RAID controller families are supported by the 'aac' driver.
