grog 3f5c94f026 Major tidy up. Add explicit header files, thus enabling the .c files

to avoid including the kernel headers.

Move a number of definitions of userland functions from
dev/vinum/vinumext.h.

Desired by:	   bde

This commit is the first of a general cleanup of the header files..
It won't be enough to make bde happy.

Remove vinum_perror and associated DEVBUG definition.
