jasone 4179e6a903 Add a separate simple internal base allocator and remove base_arena, so that 
there is never any need to recursively call the main allocation functions.

Remove recursive spinlock support, since it is no longer needed.

Allow chunks to be as small as the page size.

Correctly propagate OOM errors from arena_new().
