Use the pmap_kenter/pmap_kremove inline functions
	instead of duplicating them.

pmap_remove_all:
	Eliminate an unused (but initialized) variable.

pmap_ts_reference:
	Change the implementation.  The new implementation is much smaller
	and simpler, but functionally identical.  (Reviewed by
	"John S. Dyson" <dyson@iquest.net>.)
