Jilles Tjoelker 384aedab58 sh: Various warning fixes (from WARNS=6 NO_WERROR=1):
- const
- initializations to silence -Wuninitialized (it was safe anyway)
- remove nested extern declarations
- rename "index" locals to "idx"
