mem: rename segment address from physical to IOVA

Renaming rte_memseg {.phys_addr} to {.iova}
Keep the deprecated name in an anonymous union to avoid breaking
the API.

Use rte_iova_t and RTE_BAD_IOVA where appropriate in
memory segment handling.
