Method to identify the CS4223/CS4227 is incorrect and unreliable.
Provide a new register to differentiate between these PHY SKUs.

Fully initialize X550em_a 1G PHYs; move the PHY definitions from
ixgbe_x550.h to ixgbe_phy.h, where they really belong; define
register numbers in decimal because that is how they are in the spec.

This patch updates ixgbe_setup_mac_link_sfp_x550a for X550 SFP+.
ixgbe_set_lan_id_multi_port_pcie has been updated to set the MAC
instance(0/1) which is needed when configuring the external PHY,
since X550a has two instances of MGPK. The MAC instance is read
from the EEPROM.

It's more valuable to abstract the link read/write interface. As such,
change the following method names, and add them to a new link info
structure:
read_i2c_combined => read_link
read_i2c_combined_unlocked => read_link_unlocked
write_i2c_combined => write_link
write_i2c_combined_unlocked => write_link_unlocked
This will allow X550EM_a to override these methods for MDIO access
while X550EM_x provides methods to use I2C combined access.
Initially the structure is just method pointers and a bus
address.
Two functions involved in combined I2C accesses were moved from
ixgbe_phy.c to ixgbe_x550.c. The underlying functions that carry
out the combined I2C accesses were left in ixgbe_phy.c because
they share some functions with other I2C methods.

KR auto-neg mode is what we will be using going forward. The SW
interface for this mode is different than what was used for iXFI.
While debugging, it was determined that the ucode diagnostic was
no longer needed. This code has been removed to simplify the init
flow.
A subtle semaphore error in the CS4227 reset flow was fixed.

According to the hardware, the LINE side of the cs4227 needs to be
set to 10 Gbps SR mode regardless of the configuration of the link.

Configure the CS4227 correctly for both 1G and 10G operation,
by moving the code to ixgbe_setup_mac_link_sfp_x550em(). It
needs to be in this function because we need both the module
type and the speed, and this is the only function in the init
flow that knows the speed. In contrast,
ixgbe_setup_sfp_modules_X550em() does not know the speed, so we
can't do anything useful here. This is a fundamental difference
from the 82599 flow.

This patch adds SFP+ dual-speed support.
82599 fiber link code was moved from ixgbe_82599.c to ixgbe_commom.c
for use by X550em, and the API was updated to support the common code
usage. SFP MAC link code is added to x550em.

Most I2C accesses take and release semaphores for each access. It's
also necessary to perform multiple I2C operations under the same
holding of the semaphore, so provide unlocked I2C methods for that
purpose.
