Add support the set_mc_addr_list device operation in the bnx2xvf PMD.
The configured addresses are stored in the device private area, so
they can be flushed before adding new ones.
Without this v6 multicast packets were properly forwarded to the
Guest VF.

The logic to read vf_id used by ACQUIRE/TEARDOWN_Q/RELEASE TLVs,
multiplexed return value to convey vf_id value and status of read vf_id
API. This lets to segfault at dev_start() as resources are not properly
cleaned and re-allocated.
Fix read vf_id API to differentiate between vf_id value and return
status. Adjust the status checking accordingly.
Added bnx2x_vf_teardown_queue() API and moved relevant code from
bnx2x_vf_unload() to new API.

Add 'rte_' prefix to structures:
- rename struct ether_addr as struct rte_ether_addr.
- rename struct ether_hdr as struct rte_ether_hdr.
- rename struct vlan_hdr as struct rte_vlan_hdr.
- rename struct vxlan_hdr as struct rte_vxlan_hdr.
- rename struct vxlan_gpe_hdr as struct rte_vxlan_gpe_hdr.
Do not update the command line library to avoid adding a dependency to
librte_net.

Replace BNX2X_TLV_APPEND() with the clearer and safer bnx2x_add_tlv().
bnx2x_add_tlv() was previously prototyped at some point but can be static.

Don't use bnx2x_fill_accept_flags() to fill the RX mask in the VF
since the PF only handles a subset of the existing flags. now,
bnx2x_fill_accept_flags() can be static.

SR-IOV is supported using bnx2x poll mode driver running as VF driver and
native linux driver running as PF (in host/hypervisor). There is no issue
while running with the PF driver which is at the base version as that of
PMD. However, there is a compatibility issue between newer out-of-box PF
drivers with older VF driver. So the newer VFs would also need to send
BNX2X_VF_TLV_PHYS_PORT_ID (among other TLVs) to differentiate between
newer and older VFs.
