This patch adds support for LED management of the devices behind VMD.
It's possible to set four different states: off, on (fault), 1Hz
blinking (rebuild), and 4Hz blinking (identification).

When splitting bdev's, if we run out of child IOVs, we have to
make sure that we end block aligned. That may require us to
shorten or eliminate one or more child IOVs to be picked up
on the next split. If we eliminate enough such that there
are no IOVs for this split IO, just continue and the next
split completion will kick off another split to pick up
the remaining data to be transferred.

Failed is not a final state for either fabric or pcie controllers. We
have historically not allowed resets in the failed state, but we should.
Instead of checking for the failed state, we should check for the
removed state. If the controller is removed, then we cannot even attempt
a reset.

The way these two functions were separated previously represented a
pretty sserious bug when doing a controller reset.
If there were any outstanding requests in the rqpair, they would get
overwritten during the call to nvme_rdma_qpair_register_reqs and the
application would never get a completion for the higher level requests.
The only thing that we need to do in this function is assign the proper
lkeys.

check_enabled had a couple bugs in it that made it unfriendly for enabling
I/O qpairs after a reset.
1. It was calling nvme_qpair_abort_queued_requests before setting the
enabled flag to true. For applications that submit new I/O in the
completion callback for old I/O, this means you enter an infinite loop
of submitting requests, and then immediately completing them. SO
instead, wait for the qpair to reset, then just submit those requests to
the lower layer.
2. It didn't check whether we were already in the middle of calling it,
so we could reenter function calls like
nvme_qpair_abort_queued_requests.
Also, now that we have a coherent state machine for qpairs, we can limit
the enabling to a specific state in that state machine.

As the todo states later on in the function, the discovery controller
should really be initialized through traditional methods, but it was
hacked in. For now, enable the admin qpair to get past the non-standard
nature of this controller.

The driver has historically waited until we have to do a listen
before enabling the admin qpair. That is a very PCIe-centric mindset.
For fabric controllers, a lot of the early initialization operations such
as get_cc and set_cc are handled through the admin qpair so it should be
enabled before we begin the initialization process.
As a side effect of this cahnge, the internal API
nvme_ctrlr_enable_admin_qpair has been removed. It would have turned
into a one-liner.

This wasn't being done in the previous case which meant that I/O qpairs
were not being moved to the connecting state when connecting for the
first time. However, to prepare the way for a coherent state machine for
nvme qpairs, we need to ensure that all qpairs go through the same
states.

This path updates ocf library to version 19.06 and also introduces
all necessary changes required to integrate ocf bdev adapter with
new version of ocf.
Summary of changes introduces with new OCF in ocf bdev:
- ocf_env.h increased limit for memory operations, changed behaviour
of strncpy to less restrictive, both changes are required to run new OCF
- ctx.c changed functions to new from new OCF
- added new cache mode "write only"
- added missed cache modes wa and wi to RPC scripts
- rewritten spdk_rpc_bdev_ocf_get_stats function to use asynhronus
ocf_mngt_cache_read_lock
- used new asynhronus ocf_mngt_cache_flush instead of waiting for request
- removed no longer valid filed - cfg->device.min_free_ram
- changed expected result in metadata_probe_cb

To use zero copy bdev I/O APIs for write I/O, we have to allocate
iSCSI task before allocating data buffer because allocating data
buffer will be changed to spdk_bdev_zcopy_start() call and the
allocated iSCSI task will be passed to the call.
One critical change is that we have to read all data for the
current PDU even if handling the PDU is rejected. For that purpose,
use is_rejected flag and do not call iscsi_pdu_payload_handle()
is is_rejected is true. If iscsi_pdu_hdr_handle() returns negative,
close the connection, so return the state machine immediately.

To use zero copy bdev I/O APIs, we have to allocate iSCSI task
before allocating data buffer.
Hence we separate PDU header and payload handling for SCSI Data-Out,
and include iSCSI task allocation into PDU header handling.
Factor out PDU header handling in iscsi_op_data() into iscsi_pdu_hdr_op_data().
spdk_nvmf_tcp_h2c_data_hdr_handle() and spdk_nvmf_tcp_h2c_data_payload_handle()
in lib/nvmf/tcp.c are used as a reference implementation.
Use pdu->task to pass the task allocated by iscsi_pdu_hdr_op_data() into
iscsi_op_data().
In iscsi_op_data(), if it sees pdu->is_rejected is true or pdu->task
is NULL, do nothing. Besides, check LUN hot plug again to separate
PDU header handling and PDU payload handling.

Previously, for iSCSI target, freeing bdev_io of SCSI task was deferred
until the reference count of the SCSI task becomes zero.
But this will cause the use-after-free issue when doing LUN hotplug during
large write I/O workload.
The scenario is the following:
- Large iSCSI write I/O is split into multiple I/Os, the first I/O is from immediate,
and subsetquent I/Os are from R2T.
1. The first I/O allocates iSCSI task as primary, and is submitted to the bdev layer.
The first I/O is pending in the bdev layer.
2. The second I/O allocates iSCSI task as secondary (secondary is associated with
primary by incrementing reference count).
3. Before submitting the second I/O to the bdev layer, LUN hotplug is started.
LUN hotplug waits for getting completion of the first write I/O from the bdev layer.
4. The bdev layer completes the first I/O. The primary iSCSI task is tried to free,
but reference count is still one, and is not done yet.
5. LUN hotplug detects completion of the first write I/O, and returns
LUN I/O channel to the bdev layer.
6. The second I/O is tried to submit to the bdev layer, but LUN is already removed,
and so free the secondary iSCSI task.
7. Then the reference count of the primary iSCSI task becomes zero,
and its bdev_io is freed. However, LUN I/O channel is already freed and freeing
bdev_io fails.
This issue is caused by separating iSCSI task allocation and submission.
For write I/O, we don't have to keep bdev_io after getting completion
of it from the bdev layer.
This applies to other non-read I/O types.
So for non-read I/O, free bdev_io after getting SCSI status in
bdev_scsi_task_complete_cmd(), and for read I/O, set bdev_io to
task as same as before.
The next patch will do the same for management task.

Separate PDU header and payload handling for Login Request PDU.
This will clarify the header handling and payload handling.
Especially store the allocated and initialized response PDU to
the current connection and refer it later. This idea is as same as
iSCSI task and will work because login processing is synchronous
and only once per connection.

A local variable alloc_len had been used to hold the data buffer
length of the response PDU. But it's more intuitive to use
rsp_pdu->data_buf_len instead.
The subsequent two patches will replace related functions accordingly.

Including getting incoming login parameters into iscsi_op_login_rsp_init()
was not so intuitive.
Initializing login response done in iscsi_op_login_rsp_init() can be
done before reading data segment but getting incoming login parameters
can be done only after reading data segment.
As a preparation to separate PDU header and payload handling for
Login Request PDU, extract getting incoming login parameters from
iscsi_op_login_rsp_init() into an new function
iscsi_op_login_store_incoming_params().
Besides, refine a few pointer and return code handling to make the
code a little clearer.

Previously, pdu->data_segment_len had been set after reading all
data segment to the allocate data buffer pdu->data_bur together with
pdu->data and pdu->data_from_mempool.
But we don't have any ordering constraint to do that and the value of
pdu->data_segment_len has been fixed when reading all PDU BHS.
Hence move setting pdu->data_segment_len up to immediately after
reading PDU Basic Header Segment to pdu->bhs in iscsi_read_pdu().

Fix static analyzer error by ensuring 4 * total_ahs_len is
not larger than ISCSI_AHS_LEN.
When ./configure -enable-werror --enable-ubsan --enable-asan,
iscsi target reports heap-buffer-overflow error
when I feeding unexpeted pdu opcode.

Refine the public helper function spdk_scsi_dev_has_pending_tasks to
be able to check tasks only from the specific initiator.
Then use the function in iSCSI target to fix the issue.
Besides add UT code to test the updated spdk_scsi_dev_has_pending_tasks().
Automated multi hosts test is much better but some UT code will be of any
help to mitigate the risk of degradation.

Refine helper functions spdk_scsi_lun_has_pending_mgmt_tasks() and
spdk_scsi_lun_has_pending_tasks() to be able to check tasks only from the
specific initiator.
SCSI port is used by passing the pointer and so simple pointer
comparison is appropriate in the functions.
Add UT code to test the updated functions.
The next patch will change spdk_scsi_dev_has_pending_tasks() to
get not only SCSI device but also initiator port and make iSCSI
target use the function to fix the issue.

When closing iSCSI connection, if we check pending tasks, we must
check tasks only from the corresponding initiator.
We have to add new API to do that.
On the other hand, during IO submission or LUN hotplug, we must check
all tasks.
The latter case is done only in SCSI LUN, and the function is not
necessary to be public.
As a preparation, change scsi_lun_has_pending_tasks/_mgmt_tasks
from public to private.

1. Change signature of nvmf_rdma_fill_wr_sge - pass ibv_send_wr ** in
order to update caller's variable, add a pointer to the number of extra WRs

RDMA WR has a predefined number of SGL entries (16) and with new logic added to
support DIF the number of entries required to process the request may exceed
this limit since IO unit buffers might be divided into several parts to remove
metadata chunks from the transmition. This change calculates and allocates
required number of WRs.
Error handling section in spdk_nvmf_rdma_request_fill_iovs has been updated
to free allocated WRs

It can be useful for passing additional information about nvmf
target to a handler for new nvmf connections. Context can be
stored in globals as it is currently done in nvmf code. However
in case of multiple targets or languages where accessing global
state is challenging (i.e. Rust), this becomes inconvenient.

Enables access control to allow subsystem access on one or more FC ports.
If a subsystem definition does not have a single valid "Listen" directive,
then the subsystem will allow dynamic listen address binding. For such
subsystems,
* When a FC port comes online, FC transport will add port's address
to subsystem's listen list.
* When a FC port goes offline, FC transport will remove port's address
from subsystem's listen list.
If subsystem definition has 1 or more valid "Listen" directives then FC ports
coming online or going offline will not affect subsystem's listen address
whitelist.

SCSI command uses iSCSI task and data buffer, and will use zero copy
bdev I/O APIs.
To use zero copy bdev I/O APIs, we have to allocate iSCSI task
before allocating data buffer.
Hence we separate PDU header and payload handling for SCSI command,
include iSCSI task allocation into PDU header handling first.
Use pdu->task to pass the task allocated by the former to the latter.
In iscsi_pdu_hdr_op_scsi(), we can not expect pdu->data_segment_len
is set. So getting data segment length from PDU BHS instead.
In the updated iscsi_op_scsi(), if it sees pdu->task is NULL or
pdu->is_rejected is true, do nothing. Besides, check LUN hot plug
again to separate PDU header handling and PDU payload handling.

to use zero copy APIs in iSCSI target. As a preparation, we have to

The subsequent patches will separate PDU header handling and PDU
payload handling. But for the PDUs which have data segment,
if PDU header is correct, we have to read data segment even if any other
error is found and the PDU is rejected.
To do that, add a flag is_rejected to pass if the PDU is rejected or
not from PDU header handling to PDU payload handling.
If PDU payload handling sees the PDU is already relected, do nothing.

Subsequent patches will separate PDU handler into PDU header handler
and PDU payload handler.
Some PDU types will have both PDU header handler and PDU payload handler,
and command is executed in PDU payload handler.
Some PDU types will have only PDU header handler, and command is
executed in PDU header handler.
CmdSN must be updated appropriately depending on the case.
As a preparation, factor out updating CmdSN into a helper function
iscsi_update_cmdsn().
Besides, storing the passed CmdSN into PDU is duplicated and remove
the duplication in this patch.

Add PDU receive state to each connection, and each connection
transits among states during receiving incoming PDUs.
Four states, AWAIT_PDU_READY, AWAIT_PDU_HDR, AWAIT_PDU_PAYLOAD,
and ERROR are same as SPDK NVMe-TCP target.
Move clearing conn->pdu_in_progress just before putting pdu
when PDU processing completes normally to match with error case.

Leaving these on the stack outstanding list can cause unnecessary
buildup. If we fail to post the request to ibv, then the upper layer
request will be freed immediately for reuse, but we will keep that
request in the outstanding queue at the RDMA layer.

The patch of this purpose is to exclude the CPU cores
occupied by the DPDK thread. To mitigate the corner
case, we only do it when the number of online CPU cores
is larger than then DPDK thread occupied cpu cores.
The purpose is uset to improve the performance and avoid the
contention between DPDK thread and user's own thread.

If PDU header digest error is detected, it indicates that the length
field of the header may have been corrupted. Hence it's may not be
possible to identify the location of the beginning of a later PDU.
So move checking header digest before handling payload and then
close the connection if header digest error is detected.

Move spdk_iscsi_execute() from iscsi_conn_handle_incoming_pdus()
to spdk_iscsi_read_pdu() and then strip the prefix spdk_ from
spdk_iscsi_execute() and make it private.
This is to introduce state machine into receive incoming PDU processing.

IO trace ISCSI_TASK_EXECUTED was taken after PDU was freed.
spdk_trace_record() doesn't dereference the passed pointers and it's fine,
but get the trace before PDU is freed to make the coder clearer.

We can move data segment length check from iscsi_check_data_segment_length()
to iscsi_op_text() and iscsi_op_scsi().
Task Management Function request, SNACK request, and Logout request
don't have data segment, and so any related check is not added.
Of course we can add check if data segment length is zero though.
This patch also changes the return type of spdk_get_max_immediate_data_size()
and a related variable spdk_iscsi_pdu::data_segment_len to uint32_t to
remove unnecessary casts. They are little to stand as an independent
patch.
The next patch will remove iscsi_check_data_segment_length().

For SCSI Data-Out and NOP-Out, we can move data segment length check from
iscsi_check_data_segment_length() to iscsi_op_data() and iscsi_op_nopout(),
respectively.
In iscsi_op_nopout(), data_len had been got from reqh->data_segment_len
but reqh->data_segment_len is already copied to pdu->data_segment_len at
spdk_iscsi_read_pdu(). So add a change to use pdu->data_segment_len to
this patch. This is little change to create a single independent patch.

During login processing, only login request is accepted. So we
can move data segment length check from iscsi_check_data_segment_length()
to iscsi_op_login().
A few patches from this will inline data segment length check into
each opcode handler and then remove iscsi_check_data_segment_length().

When doing a reset on an NVMe-oF target with active I/O qpairs, we need
to be able to submit fabrics commands on them in order to perform a reset.
Currently, resetting a fabric controller with any I/O qpairs active will
cause the reset to hang indefinitely.

The remove callback is a built in way of alerting the user application
that we have removed a controller. Once we fail a controller, we never
move it back out of that state so it is in essence removed.

hwqp->fgroup is valid only for IO queues and this particular function
deals with pending requests for IO queues. Check hwqp->fgroup and
bailout if called in LS queue context.

My recent changes that introduced batching to queued request
resubmission also introduced a regression that can lead to reordering
requests before submitting them to the drive. This change prevents that.
We wait until inside the internal _nvme_qpair_submit_request function to
check for queued entries to avoid queueing a request that has children.
If a request that has children gets queued, when we process completions
and resubmit the parent, it will result in the children being submitted.
Since we only account for the number of requests we completed in the
last iteration, some of the child requests may be requeued out of order,
or worse, none of the child requests will end up being submitted to the
transport and they will all be queued behind previously queued requests.

When we ran out of child_iov space, ensure the iovs to be aligned
with block size. However the calculation was wrong.
(to_next_boundary_bytes % blocklen) meant not to_last_block but to_next_block.
So calculate to_last_block_size by reducing to_last_block_size from blocklen.
The data was collected when the issue occured. So add unit test
by using the data.

va_list is not passed in logfunc, so the user-provided log call can't
get the arguments corresponding to the format string.
This patch fixes it and replaces log func pointer in spdk_app_opts
with logfunc.

By moving the contents of spdk_nvme_ctrlr_reset to a new internal
function, I am paving the way for providing two reset paths. One, which
can be used by the user as an external API function and which provides
the same legacy behavior. Specifically, that it will always fail the
ctrlr after an attempted reset, and a second, internal path, which will
be used by the qpair reconnect code which will defer failing the qpair
to the qpair code.

spdk_reactors_fini() is unconditionally called in spdk_app_fini()
at the end of every application and it currently throws a ton
of warning messages if the reactors weren't initialized yet [1].
Let's silence those warnings.

Set g_spdk_app.config early in the app init function,
so that the subsequently called spdk_app_fini() will

In iscsi_conn_flush_pdus(), call iscsi_conn_flush_pdus_internal()
only when the connection state is RUNNING or INVALID.
Besides, we can remove the iscsi_conn_flush_pdus() call from
iscsi_conn_sock_cb() because the connection state is already EXITING.

Request logout to the initiator asynchronously when exiting the
running connection from the target.
Based on this patch, the next patch will make flushing PDUs possible
only when the connection is RUNNING (or INVALID) and remove the
tight loop in iscsi_conn_flush_pdus().
Set timeout of logout request as 30 seconds. 30 seconds is as same as
NOP timeout and will be reasonable for now.
Add and use logout_request_timer to check the timeout.
When the connection gets internal a logout request, move the connection
to EXITING state if the connection is in INVALID state, request logout
to initiator and start 30 seconds timer if the connection is in RUNNING
state and logout is not requested yet, or do nothing otherwise.

Change logout_timeout() not to call spdk_iscsi_conn_construct() but
to move conn->state to EXITING. Then spdk_iscsi_conn_destruct() will
be called by the poller of iSCSI poll group, iscsi_poll_group_poll()
soon.
This will improve the maitainability slightly.

The connection state is mainly for flushing PDUs. ISCSI_CONN_STATE_LOGGED_OUT
was as same as ISCSI_CONN_STATE_RUNNING for this point.
Replace ISCSI_CONN_STATE_LOGGED_OUT state by a is_logged_out flag
and use the latter to reject any incoming PDU after sending logout
response.

Previously When the connection is in login processing, flushing PDUs
had been done by tight loop. Login processing will not fill out
send buffers but we can use poller to flush PDUs as same as when the
connection is running iscsi_conn_flush_pdus().
Hence change the check condition to include INVALID state to use
poller in iscsi_conn_flush_pdus().
This change is necessary because the subsequent patches will change
iscsi_conn_flush_pdus() not to flush PDUs when the connection is not
invalid or running.

We can remove the declaration of iscsi_conn_stop() by moving
the related functions before the caller to iscsi_conn_stop(),
_iscsi_conn_check_shutdown() and _iscsi_conn_destruct().
