af034b6de4 bdev: unregister bdevs top-down during shutdown.
There are some use cases such as multipath and RAID expansion where a
vbdev could have been registered before one of its base bdevs.

Currently we unregister bdevs at shutdown in reverse order of their
registration. Continue to do that in general, but skip any bdev that
is still claimed. Any bdevs skipped in this way will eventually be
unregistered once any bdevs that have claimed it have completed
unregistration.

3126157a21 bdev_nvme: set to deactive state when deleting NS bdev
Since the number of NVMe controller's Namespace is a const
variable, so the Namespace block device is a static array,
and is assigned with NSID after initialization, so when
users want to delete a specified block device, we can
just change it into inactive state.

Fix issue #495.

have caused that failure as well (I don't think so though, there

0d8783cabc bdev/passthru: check early for duplicate passthru bdev names
The bdev layer will sort of check this when we try to
register the bdev, but it's better to catch this much
earlier. That way we catch it when the name gets inserted
into the list, rather than when the base bdev appears.

49e9bfda9c bdev/crypto: remove unnecessary driver init call on crypto dev hotplug
The crypto drivers are now always initialized at
bdev module init, so any subsequent attempts to
init them would immediately return with rc = 0.

Besides, from the code perspective, this call does
not have the proper error handling. To clean up
all of this, we remove the call entirely.

13e69db5c0 bdev/iscsi: attach poller to lun instead of channel
There is no guarantee that the first io_channel created for
a LUN will be the last one destroyed. So we need to attach
the poller to the LUN, not the io_channel.

While here, and related to this patch, remove the assert()
that the last channel destroyed must be on the LUN's
master thread. This assertion simply isn't true.

7f6738e12d crypto: add proper reset handling
Previously there was no consideration for IO that were outstanding
to the crypto device when handling a reset. This patch makes sure
that those IO are completed with FAIL status prior to completing
the reset that we pass down the stack.

It does so by sending down the reset first and in the completion
using spdk_for_each_channel and the poller to quiesce each channel
allowing the crypto side to complete all IOs before we finally
complete the reset IO after the last channel is quiesced.

Resets are tracked on a per bdev basis.

Addresses github issue #449.

f3096dace8 bdev/virtio: deprecate old bdev construct RPCs
The 4 different RPC commands to create Virtio bdevs
have been unified to a single, yet another RPC command
in hopes the previous ones will be eventually removed.
That happened in 18.07 and now in order to remove the
old ones we mark them deprecated.

The unified RPC command was introduced in 18.07 and
has already replaced the old, separate RPCs in our
test scripts some time ago.

This patch also removes the old RPCs from doc/bdev.md
and while here, rephrases the Virtio section a bit to
make it more readable after some alphabetization
changes from the past.

jsonrpc.md already described the old RPCs as about to
be deprecated, so there are no changes required in
there.
