ab808d2b68 lib/scsi: Pass bdev_name instead of bdev to scsi_lun_construct()
Pass not bdev but bdev_name to scsi_lun_construct() to fix the
race condition due to the time gap between spdk_bdev_get_by_name()
and spdk_bdev_open(). A pointer to a bdev is valid only while the
bdev is opened.

spdk_bdev_open() has been replaced recently by spdk_bdev_open_ext(),
but the issue still existed.

5029fe14b6 scsi: add bdev resize callback support
Currently, the scsi bdev only supports the hotremove event,
and the scsi library uses the deprecated `spdk_bdev_open` function.

In this patch, add the resize event support, so the upper layer
could do more actions, like vhost-scsi could notify the guest os.

For the scsi compatibility, add _ext suffix for some public api.

799e0a84af lib/scsi: Merge append and execute SCSI task into a single function
Both for SCSI IO task and management task, append and execute
operations bad bbeen separated into different functions.

Append operation was for LUN reset, and separating into two different
functions was for clarification and readability.

LUN reset is sufficiently stable now.

Merging append and execute SCSI task into a single function is good
as API and enables us to do optimization.

For SCSI management task, merge spdk_scsi_lun_append_mgmt_task into
spdk_scsi_lun_execute_mgmt_task() simply.

For SCSI IO task, merge spdk_scsi_lun_append_task into
spdk_scsi_lun_execute_task() and do a small optimization.
The refined spdk_scsi_lun_execute_task() adds the IO task to the
pending list if there is any pending management task, executes all
existing penging IO tasks first and then the IO task if there is any
pending IO task, or executes the IO task directly otherwise.

Update unit test accordingly.

6383602952 lib/scsi: Remove workaround for astyle format check about break-after-return-type
To avoid the starnge formatting, typedef has been used. But this comment
is hard to get the meaning. So stop breaking after return type for this case.

The strange formating is

db91cfe045 lib/scsi: Stop submitting new task and abort pending tasks for LUN hotplug
Previously iSCSI LUN hotplug had critical bugs and we had no choice but
accept submitting new tasks as late as possible. We fixed the bug now and
we can stop submitting new task immediately after starting LUN removal
process.

By this change, no task is submitted to the LUN and previously queued
tasks have no chance to be kicked. Hence we execute them instead after
stopping new task submission.

This change simplifies LUN hotplug process and reproduce LUN hotplug
issues solidly if we don't have the fix.

1dc9a7627f lib/scsi: Check pending tasks for the SCSI device only from the specific initiator
Refine the public helper function spdk_scsi_dev_has_pending_tasks to
be able to check tasks only from the specific initiator.

Then use the function in iSCSI target to fix the issue.

Besides add UT code to test the updated spdk_scsi_dev_has_pending_tasks().
Automated multi hosts test is much better but some UT code will be of any
help to mitigate the risk of degradation.

f9583f2c0d lib/scsi: Check pending tasks for the LUN only from the specific initiator
Refine helper functions spdk_scsi_lun_has_pending_mgmt_tasks() and
spdk_scsi_lun_has_pending_tasks() to be able to check tasks only from the
specific initiator.

SCSI port is used by passing the pointer and so simple pointer
comparison is appropriate in the functions.

Add UT code to test the updated functions.

The next patch will change spdk_scsi_dev_has_pending_tasks() to
get not only SCSI device but also initiator port and make iSCSI
target use the function to fix the issue.

8f33ac020d scsi: Fix seg.fault due to the gap between IO channel put and LUN removal
The reported github issue #938 has been reported intermettently.
The issue is that the bdev descriptor passed to spdk_bdev_reset()
is not valid and causes seg. fault.

Current implementation of LUN hot plug is that putting IO channel
and removing LUN are done by different poller. Hence if any task
management command is issued between the gap, the reported issue
is likely to occur.

The flag removing is set at the start of LUN hot plug and so
spdk_scsi_dev_get_lun() can return NULL even before completing
removal by referring the flag removing.

  Fixes #938.

56d8b78576 lib/scsi: Make spdk_scsi_dev_destruct asynchronous
This is the end of the patch series. After this patch,
delete_target_node RPC will wait for the completion of
removal of the SCSI device and then free the iSCSI target.

SCSI device holds passed callback and calls it in free_dev().
free_dev() is ensured to be called after all iSCSI sessions
are closed. So iSCSI target resource can be freed safely
after that.

728972cdf3 scsi: Set TMF code prior to calling spdk_scsi_dev_queue_mgmt_task
By subsequent patches for iSCSI, spdk_scsi_dev_queue_mgmt_task()
will not be called directly from the function that knows TMF code,
and currently setting TMF code to SCSI task is done in
spdk_scsi_dev_queue_mgmt_task().

Hence after subsequent patches for iSCSI, to hand off TMF code to
SCSI task, any dynamic context will be required.

To avoid the dynamic context, extract setting TMF code from
spdk_scsi_dev_queue_mgmt_task() and put appropriate place for
each call of spdk_scsi_dev_queue_mgmt_task().

Additionally, in spdk_abort_transfer_task_in_task_mgmt_resp(),
ref_task_tag is got from PDU but getting it from SCSI task is
much easier. Hence get ref_task_tag from SCSI task in the callback.

Submission queue deleted by https://review.gerrithub.io/393911
is necessary to support proper LUN reset.

Hence revert the removal in this patch.

And using the submission queue, suspend IO task submission during
LUN reset to limit completion time of LUN reset.

6dd09113e5 scsi: Serialize LUN reset execution
Task management functions don't require performance. Serializing
execution of task managment functions will sipmlify and stabilize
the logic and will be helpful for upcoming patches to support
other task management functions.

This patch introduces two queues, pending and submitted queue,
and serializes LUN reset exection and makes LUN hot removal
wait for LUN reset.

Besides, checking if LUN is NULL is moved to the upper layer as
same as IO task submission.

fec7c88e45 scsi: Enforce to open LUN and then alloc IO channel by using descriptor
Use a descriptor got by opening the LUN to allocate IO channel of the LUN.
This requires opening the LUN is done before allocating IO channel of the
LUN.

Use the descriptor to free IO channel of the LUN too.

Additionally, assert is added to the close LUN function to check if
IO channel is freed before the last close LUN function is called.

af9c9a415a scsi: validate dev name length
Instead of silently truncating overly-long SCSI dev names, add an
explicit check and return an error if the name is too long.

Since we now calculate the length of name up front, this also allows
simplification of the copy into dev->name.

This also ensures that dev->name will always be zero-terminated, so we
can simplify the strnlen() in spdk_bdev_scsi_pad_scsi_name() to a
strlen() and remove the invalid unit test case for padding names longer
than SPDK_SCSI_DEV_MAX_NAME.

3b3c6002c9 iscsi: Add LUN to an existing target (not runtime)
Removing an LUN from an existing iSCSI target is possible by
removing the corresponding BDEV. However adding an LUN to an
existing iSCSI target is not possible yet.

Add a new function spdk_iscsi_tgt_node_add_lun() and related
functions first toward supporting this function.

JSON-RPC for this operation will be submitted an another patch.

Informing the newly added LUN to the initiator is not included
in this patch. Hence this operation is possible only for any
inactive target.

85a61ea6f0 scsi: fix LUN removal/hotremoval path
bdev hotremove event is send directly to hotremove callback given
during spdk_scsi_dev_construct() call. So any bdev hotremove might be
promoted to whole SCSI device removal (like in vhost) wich will trigger
LUN removal. But after returning from hotremove callback LUN and device
might be still referenced (eg to register poller or by outstanding IO).

Even worse: spdk_scsi_dev object is not dynamicaly allocated but
returned from static array which mean there is no way to detect
use-after-free error on spdk_scsi_dev by any tool. This might lead to
using SCSI device that is freed or assigned to different device.

To fix this:
- always delete LUN using hotremove path
- defer spdk_scsi_dev delete/removal after all LUNS are really
deleted.

583e969922 scsi: remove lun name
There is no need to keep a lun name anymore - we always
use the bdev name as the lun name so it is not providing any
additional value. This also keeps us from associating
the same bdev with different LUNs on different iSCSI target
nodes or vhost-scsi controllers.

Side effect of this change is:

1) Use "bdev_name" across the APIs to make it more clear
   what these names refer to.

9014e913d9 scsi/dev: print an error when cannot find bdev with specified name
When trying to create a scsi dev with inexistent
lun (bdev name), the scsi_dev_construct would
just return a NULL without printing any error.
We should probably return an error code of some
kind, but for now let's just print an extra
message.

59970a89be astyle: enforce braces around single-line statements
Require braces around all conditional statements, e.g.:

    if (cond)
        statement();

becomes:

    if (cond) {
        statement();
    }

This is the style used through most of the SPDK code, but several
exceptions crept in over time.  Add the astyle option to make sure we
are consistent.

e44731d659 scsi: Remove the claimed flag from LUN
Struct spdk_scsi_lun has the claimed flag as a member.

However the claimed flag is no longer required.

Moreover the claimed flag itself is not used but
spdk_scsi_lun_claim() and _unclaim() functions are used in the
UT code by hard to understand manner. Moving the logic into
spdk_scsi_lun_construct() and _destruct() in the UT code will
be more natural.

Hence remove the claimed flag from LUN and do a little refactoring
in the UT code.

36a8f75006 scsi: fix LUN 0 check in scsi_dev_construct
When constructing a SCSI device, LUN 0 must be assigned to some LUN.
However, the current code was assuming that LUN 0 had to be in the first
array element of lun_id_list in spdk_scsi_dev_construct().  Combined
with the scripts/rpc.py implementation that uses a Python (unordered)
dictionary object to generate the lun_ids array, there could be cases
where LUN 0 exists in the list but isn't first.

Fix the check by allowing LUN 0 in any position of the lun_id_list
array.

f25f84b796 scsi: fix off-by-one error in spdk_scsi_dev_get_lun()
lun_id could potentially overflow the dev->lun array bounds if it was
equal to dev->maxlun, which is actually the maximum lun array index plus
one, not the maximum valid LUN index.

4185205540 scsi: Remove cb_event. It is no longer necessary.
This is an artifact from a past design. There is no longer
any reason to create an event here - the bdev layer will
correctly queue events and call completions on the correct
thread.

5e132b6b7a scsi: Set proper transport protocol id
This patch adds property of transport protocol id to spdk_scsi_dev.
This allows to change it depending on which appliation is using
the device. Previously only iSCSI was used for all.

Setting protocol id is done at the time when device is added to
vhost controller (vhost) or target node (iSCSI).

Please note that for SPDK vhost SAS protocol id is used,
as that is what kernel vhost reports as well.

90b0873665 scsi: handle return status of spdk_scsi_lun_claim(lun)
This is necessary to prevent claiming the same LUN twice
and properly cleanup in case of an error during spdk_scsi_dev_construct.

This patch addresses three issues:
- spdk_scsi_lun_claim error is correctly handled in spdk_scsi_dev_add_lun
- on error when constructing scsi dev, it is now correctly removed along with attached luns
- spdk_scsi_dev_destruct not only unclaims, but calls spdk_scsi_lun_destruct on each lun in dev

Unit tests relevant to this behaviour are changed to show this functionality is now working.
