Added note to the nvmf documentation about an issue [1] with destroying
qpairs on E810 cards in RoCE mode. The note needs to be removed once
the issue is resolved.

The section "Using RPCs" uses the parameter --max-qpairs-per-ctrlr
but it has been deprecated.
Use max_io_qpairs_per_ctrlr instead.

- New files and updates to existing SPDK files to add the NVMf-FC transport.
- Depends on an existing low level driver library. This driver is not part of SPDK repository.
- Makefile updates to build FC transport (using CONFIG_FC)
- Update configure script for FC build.
- New FC unit test for FC-LS commands.
- Update unittest.sh to run FC unit test (when built).

Update doc to fix typo in the io unit size for TCP.
Using an io unit size of 16348 results in an invalid io size error
which is resolved once you change the value to 16384

- Remove outdated section about pre-release kernels; the Linux kernel
support has been in released kernels for a while now.
- Switch to ./configure rather than `make CONFIG_...`.
- Put the target configuration into subsections, and move it earlier in
the doc.
- Clean up some wording and style issues.

The nvmf library now supports the ability to assign arbitrary NSIDs,
rather than automatically assigning the next one in line. Expose this
functionality to the user via the configuration file and RPC interfaces.

There is now only virtual mode. Virtual mode has been
improved enough to reach feature parity with direct
mode and performance benchmarks show no degradation.
Simplify the code by always using virtual mode.

Doxygen interprets each Markdown input file as a separate section
(chapter). Concatenate all of the .md files in directories into a
single file per section to get a correctly-nested table of contents.
In particular, this matters for the navigation in the PDF output.
