Changpeng Liu f386632dd2 nvmf/vfio-user: add a pointer to struct spdk_nvmf_ctrlr 
The pointer to struct spdk_nvmf_ctrlr is used to save mandatory
controller registers to the migration region.

Also rename some ctrlr/qpiar to vu_ctrlr/vu_qpiar.
