740a720142 In msdosfs_inactive(), reclaim the vnodes both for SLOT_DELETED and
SLOT_EMPTY deName[0] values. Besides conforming to FAT specification, it
also clears the issue where vfs_hash_insert found the vnode in hash, and
newly allocated vnode is vput()ed. There, deName[0] == 0, and vnode is
not reclaimed, indefinitely kept on mountlist.

ef6a2be307 Assert that the msdosfs vnode is (e)locked in several places.
The plan is to use vnode lock to protect denode and fat cache,
and having separate lock for block use map.

Change the check and return on impossible condition into KASSERT().

22e62e7e6e In both if_tun and if_tap:
Do not do additional dev_ref() on the newly created interface in the
if_clone create method [1]. This reference is not needed and never
removed, causing struct cdevpriv leakage. Remove the setting of
SI_CHEAPCLONE flag as well, since it is unused.

For dev_clone handlers, create cdevs with the call make_dev_credf(MAKEDEV_REF)
instead of calling make_dev() and then dev_ref(), to avoid a race.

Call drain_dev_clone_events() at the module unload time after dev_clone
handler is deinstalled.

d1d5b9c5a6 Interrupt related cleanups:
o  Assign vectors based on priority, because vectors have
   implied priority in hardware.
o  Use unordered memory accesses to the I/O sapic and use
   the acceptance form of the mf instruction.
o  Remove the sapicreg.h and sapicvar.h headers. All definitions
   in sapicreg.h are private to sapic.c and all definitions in
   sapicvar.h are either private or interface functions. Move the
   interface functions to intr.h.
o  Hide the definition of struct sapic.

0b993ee5fd When running as a guest operating system, the FreeBSD kernel must assume
that the virtual machine monitor has enabled machine check exceptions.
Unfortunately, on AMD Family 10h processors the machine check hardware
has a bug (Erratum 383) that can result in a false machine check exception
when a superpage promotion occurs.  Thus, I am disabling superpage
promotion when the FreeBSD kernel is running as a guest operating system
on an AMD Family 10h processor.

00305546f3 For kinfo_proc in kp->ki_siglist, return the set of the signals pending
in the process queue when gathering information for the process, and set
of signals pending for the thread, when gathering information for the
thread. Previously, the sysctl returned a union of the process and some
arbitrary thread pending set for the process, and union of the process
and the thread pending set for the thread.

443e752d97 Revamp the pieces of some of the stuff I forgot to do when shifting to
32 bit handles. The RIO (reduced interrupt operation) and fast posting
for the parallel SCSI cards were all 16 bit handles. Furthermore,
target mode parallel SCSI only can have 16 bit handles.

Use part of a supplied patch to switch over to using 32 bit handles.
Be a bit more conservative here and only do this for parallel SCSI
for the 12160 (Ultra3) cards. There were a lot of marginal Ultra2
cards, and, frankly, few are findable now for testing.

Fix the target handle routine to only do 16 bit handles for parallel
SCSI cards. This is okay because the upper sixteen bits of the new
32 bit handles is a sequence number to help protect against duplicate
completions. This would be very unlikely to happen with parallel
SCSI target mode, and wasn't present before, so we're no worse off
than we used to be.

While we're at it, finally split the async mailbox completion handlers
into FC and parallel SCSI functions. This makes it much cleaner and
easier to figure out what is or isn't a legal async mailbox completion
code for different card classes.

be95548d86 Disable TSO on BCM5755M controller until I understand better for
the issue. I still have no idea why TSO does not work on this
controller. davidch@ also confirmed there is no known TSO related
issues for this controller.

c3b399103c Move TSO setup to new function bce_tso_setup(). Also remove VLAN
parsing code in TSO path as the controller requires VLAN hardware
tagging to make TSO work over VLANs.
While parsing the mbuf in TSO patch, always perform check for
writable mbuf as bce(4) have to reset IP length and IP checksum
field of IP header and make sure to ensure contiguous buffer before
accessing IP/TCP headers. While I'm here replace magic number 40 to
more readable sizeof(struct ip) + sizeof(struct tcphdr).

b2ec4a73f5 Make toggling TSO, VLAN hardware checksum offloading work. Also fix
TX/RX checksum handler to set/clear relavant assist bits which was
used to cause unexpected results.
With this change, bce(4) can be bridged with other interfaces that
lack TSO, VLAN checksum offloading.

e89abed364 Allow disabling VLAN hardware tag stripping with software work
around. Management firmware(ASF/IPMI/UMP) requires the VLAN
hardware tag stripping so don't actually disable VLAN hardware tag
stripping. If VLAN hardware tag stripping was disabled, bce(4)
manually reconstruct VLAN frame by appending stripped VLAN tag.
Also remove unnecessary IFCAP_VLAN_MTU message.

c72f075a24 Allocate single MSI message. msk(4) used to allocate 2 MSI messages
for controllers like 88E8053 which reports two MSI messages.
Because we don't get anything useful things with 2 MSI messages,
allocating 1 MSI message would be more sane approach.
While I'm here, enable MSI for dual-port controllers too. Because
status block is shared for dual-port controllers, I don't think
msk(4) will encounter problem for using MSI on dual-port
controllers.

7420e9dc9d Don't hardcod register offset to set PCIe max read request size.
The register offset is not valid on 88E8072 controller. Also don't
blindly increase max read request size to 4096, instead, use 2048
which seems to be more sane value and only change the value if the
hardware default size(512) was used on that register.
For PCIX controllers, use system defined constant rather than using
magic value.
While I'm here stop showing negotiated link width.

1b7757c024 Optimize inserting LE for TX checksum computation. Controller does
not require checksum LE configuration if checksum start and write
position is the same as before. So keep track last checksum start
and write position and insert new LE whenever the position is
changed. This reduces number of LEs used in TX path as well as
slightly enhance TX performance.

f4a943617a Fix another bug involving /dev/mem and the OEA64 scratchpage. When
the scratchpage is updated, the PVO's physical address is updated as well.
This makes pmap_extract() begin returning non-zero values again, causing
the panic partially fixed in r204297. Fix this by excluding addresses
beyond virtual_end from consideration as KVA addresses, instead of allowing
addresses up to VM_MAX_KERNEL_ADDRESS.

3258030144 Introduce the new kernel sub-tree x86 which should contain all the code
shared and generalized between our current amd64, i386 and pc98.

This is just an initial step that should lead to a more complete effort.
For the moment, a very simple porting of cpufreq modules, BIOS calls and
the whole MD specific ISA bus part is added to the sub-tree but ideally
a lot of code might be added and more shared support should grow.

7f450feb07 Fix edge cases in several KASSERTs: use <= rather than < when testing that
counters have not gone about MAXCPU or NETISR_MAXPROT.  These problems
caused panics on UP kernels with INVARIANTS when using sysctl -a, but
would also have caused problems for 32-core boxes or if the netisr
protocol vector was fully populated.

44f06ae57d Move the OEA64 scratchpage to the end of KVA from the beginning, and set
its PVO to map physical address 0 instead of kernelstart. This fixes a
situation in which a user process could attempt to return this address
via KVM, have it fault while being modified, and then panic the kernel
because (a) it is supposed to map a valid address and (b) it lies in the
no-fault region between VM_MIN_KERNEL_ADDRESS and virtual_avail.

While here, move msgbuf and dpcpu make into regular KVA space for
consistency with other implementations.

2e5514defa Don't inforce an upper bound on kern.ngroups. The INT_MAX-1 limit was
too high due to several overflows.  The actual limit is somewhere in the
neighborhood of INT_MAX/4 on 64-bit machines, but most systems could not
support such a limit due to a lack of memory and the cost of duplicate
credentials.

e83ec3e5c3 There is no need to test __FreeBSD_version for features that have
been around for a long time now (7.1-ish or even earlier); assume
they are present.  These includes MSI, TSO, LRO, VLAN, INTR_FILTERS,
FIRMWARE, etc.

Also, eliminate some dead code and clean up in other places as part
of this quick once-over.

be688bde90 Accessing an mbuf after it has been handed off to the hardware is a bad
race as it could already have been tx'd and freed by that time.  Place
the bpf tap just _before_ writing the gen bit.

This fixes a panic when running tcpdump on a cxgb interface.

83c01b8cf6 Close a race involving the OEA64 scratchpage. When the scratch page's
physical address is changed, there is a brief window during which its PTE
is invalid. Since moea64_set_scratchpage_pa() does not and cannot hold
the page table lock, it was possible for another CPU to insert a new PTE
into the scratch page's PTEG slot during this interval, corrupting both
mappings.

Solve this by creating a new flag, LPTE_LOCKED, such that
moea64_pte_insert will avoid claiming locked PTEG slots even if they
are invalid. This change also incorporates some additional paranoia
added to solve things I thought might be this bug.

8d521790d0 Yet another attempt to make palette loading more safer:
- Add a separate palette data for 8-bit DAC mode when SC_PIXEL_MODE is set
and fill it up with default gray-scale palette data for text.  Now we don't
have to set `hint.sc.0.vesa_mode' to get the default palette data.
- Add a new adapter flag, V_ADP_DAC8 to track whether the controller is
using 8-bit palette format and load correct palette when switching modes.
- Set 8-bit DAC mode only for non-VGA compatible graphics mode.

940907907b xpt_rescan only honors a wildcard in the target field. Revert the previous
change and have isp_make_here scan the whole bus which will then scan all
luns.

I think xpt_rescan needs to be fixed, but that's a separable issue.

9e48f1e7a5 According to the Linux sungem driver, in case of Apple (K2) GMACs
GEM_MIF_CONFIG_MDI0 cannot be trusted when the firmware has powered
down the chip so the internal transceiver has to be hardcoded. This
is also in line with the AppleGMACEthernet driver, which just doesn't
distinguish between internal/external transceiver and MDIO/MDI1

ecafbbb5b9 Add TSO on VLANs. Because re(4) has a TSO limitation for jumbo
frame, make sure to update VLAN capabilities whenever jumbo frame
is configured.
While I'm here rearrange interface capabilities configuration. The

so explicitly check this requirement.

daf6545e61 Enforce stronger semantics for bus-dma alignment (currently only on amd64).
Now all contiguous regions returned from bus-dma will be aligned to the
alignment constraint and all but the last region are guaranteed to be
a multiple of the alignment in length.  This also means that the relative
alignment of two adjacent bytes in the I/O stream have a difference of 1
even if they are not physically contiguous.

The old code, when needing to perform a copy in order to align data, only
copied the amount of data needed to reach the next page boundary.  This
often left an unaligned end to the segment.  Drivers such as Xen's blkfront
can't deal with such segments.

The downside to this approach is that, once an unaligned region is encountered,
the remainder of the I/O will be bounced.  However, bouncing should be rare.
It is typically caused by non-performance critical userland programs that
don't bother to align their I/O buffers (e.g. bsdlabel).  In-kernel I/O
buffers are always aligned to at least a page boundary.

fd776d18a0 Align the start of the clean submap to a superpage boundary. Although

5702371bd2 ifconfig(8) expects interface fooX to be supported by the module if_foo,
and will try to load it if it's not present.  To better meet these
expectations, change the module name for the loopback interface from
'loop' to 'if_lo'.  The loopback interface is always compiled into the
base kernel, so there are no resulting changes in kld files, etc.

62399df2c9 Add proper const keywords to sysctl(3) parameters.
The `name' and `newp' arguments can be marked const, because the buffers
they refer to are never changed. While there, perform some other
cleanups:

- Remove K&R from sysctl.c.
- Implement sysctlbyname() using sysctlnametomib() to prevent
  duplication of an undocumented kernel interface.
- Fix some whitespace nits.

It seems the prototypes are now in sync with NetBSD as well.

CPU implementation number from a global variable to the per-CPU data.
Functions which are called before the latter is available are passed the
implementation number as a parameter now.

02d77e455f Increase PCIe maximuim read request size to 2048. Because re(4) uses
Tx DMA burst size 2048, I beleive PCIe maximum read request size
also should match to the value of Tx DMA burst size. With this
change I can get more than 800Mbps for TCP bulk transfers.
Previously I was not able to get more than 700Mbps. If I enable TSO
it now shows 927Mbps.

c9fdacdac8 Start to implement ifnet DDB support:
- 'show ifnets' prints a list of ifnet *s per virtual network stack,
- 'show ifnet <struct ifnet *>' prints fields matching the given ifp.

We do not yet print the complete set of fields and might want to
factor this out to an extra if_debug.c file in case this grows
a lot[1]. We may also want to grow 'show ifnet <if_xname>' support[1].

0364c7f075 Some fixes to the current RMI interrupt handling, changes in this patch are:
- (cleanup) remove rmi specific 'struct mips_intrhand' - this is no
longer needed since 'struct intr_event' have all the required hooks
- add xlr_cpu_establish_hardintr, which has args for pre/post ithread
and filter hooks, so that the PCI code can add the PCI controller
interrupt ack code here
- make 'cpu_establish_hardintr' use the above function.
- (fix) change type of eirr/eimr from register_t to uint64_t. These
have to be 64bit otherwise we cannot handle interrupts from 32.
- (fix) use eimr to mask eirr before checking interrupts, so that we
will not handle masked interrupts.

ab73970649 Reduce KVA pressure on OEA64 systems running in bridge mode by mapping
UMA segments at their physical addresses instead of into KVA. This emulates
the direct mapping behavior of OEA32 in an ad-hoc way. To make this work
properly required sharing the entire kernel PMAP with Open Firmware, so
ofw_pmap is transformed into a stub on 64-bit CPUs.

Also implement some more tweaks to get more mileage out of our limited
amount of KVA, principally by extending KVA into segment 16 until the
beginning of the first OFW mapping.

19317dfd85 Merge r198724 to Book-E. casuword() non-atomically read the current value
of its argument before atomically replacing it, which could occasionally

c03dee5d5b Fix DDB backtrace that includes a kernel exception frame.
The backtrace code tries to look for an instruction of the form "sw ra, x(sp)"
to figure out the program counter of the calling function. When we generate
the kernel exception frame we store the 'ra' at the time of the exception
using an instruction of the same form. The problem is that the 'ra' at the
time of the exception is not the same as the 'program counter' at the time
of the exception.

The fix is to save the 'exception program counter' register by staging
it through the 'ra' register.

32115b105a Please welcome HAST - Highly Avalable Storage.
HAST allows to transparently store data on two physically separated machines
connected over the TCP/IP network. HAST works in Primary-Secondary
(Master-Backup, Master-Slave) configuration, which means that only one of the
cluster nodes can be active at any given time. Only Primary node is able to
handle I/O requests to HAST-managed devices. Currently HAST is limited to two
cluster nodes in total.

HAST operates on block level - it provides disk-like devices in /dev/hast/
directory for use by file systems and/or applications. Working on block level
makes it transparent for file systems and applications. There in no difference
between using HAST-provided device and raw disk, partition, etc. All of them
are just regular GEOM providers in FreeBSD.

062c8f4c86 Fix a bug where pages being removed from memory entirely no longer have
PVOs, and so the modified state of the page can no longer be communicated
to the VM layer, causing pages not to be flushed to swap when needed, in
turn causing memory corruption. Also make several correctness adjustments
to I-Cache synchronization and TLB invalidation for 64-bit Book-S CPUs.

ddc534916d Allow the pmap code to be built with GCC from FreeBSD 7 again.
This patch basically gives us the best of both worlds. Instead of
forcing the compiler to emulate GNU-style inline semantics even though
we're using ISO C99, it will only use GNU-style inlining when the
compiler is configured that way (__GNUC_GNU_INLINE__).

f7bb996d92 Various fixes to get the SWARM config working on a big-endian Sibyte CPU.
Getting the little-endian PCI bus working on the big-endian CPU proved to be
quite challenging. We let the PCI devices be mapped in the "match byte lanes"
address window. This is where they are mapped by the CFE and DMA transfers
generated to or from addresses within this window are not subject to automatic
byte-swapping.

However any access by the driver to memory-mapped pci space is redirected
via the "match bit lanes" address window. We get the benefit of automatic
byte swapping through this address window and drivers don't need to change
to deal with CPU big-endianness.

b06b8fe3a7 Factor out the code shared between NFS client and server into its own

45d9abdbc3 adds bwn(4) driver for supporting Broadcom BCM43xx chipsets.
o uses v4 firmware instead of v3.  A port will be committed to create
    the bwn firmware module.
  o supports B/G and LP(low power) PHYs.
  o supports 32 / 64 bits DMA operations.
  o tested on big / little endian machines so should work on all
    architectures.

It'd not connected to the build until the firmware port is committed.

26279767e4 Some code churn:
o   Eliminate IA64_PHYS_TO_RR6 and change all places where the macro is used
    by calling either bus_space_map() or pmap_mapdev().
o   Implement bus_space_map() in terms of pmap_mapdev() and implement
    bus_space_unmap() in terms of pmap_unmapdev().
o   Have ia64_pib hold the uncached virtual address of the processor interrupt
    block throughout the kernel's life and access the elements of the PIB
    through this structure pointer.

This is a non-functional change with the exception of using ia64_ld1() and
ia64_st8() to write to the PIB. We were still using assignments, for which
the compiler generates semaphore reads -- which cause undefined behaviour
for uncacheable memory. Note also that the memory barriers in ipi_send() are
critical for proper functioning.

With all the mapping of uncached memory done by pmap_mapdev(), we can keep
track of the translations and wire them in the CPU. This then eliminates
the need to reserve a whole region for uncached I/O and it eliminates
translation traps for device I/O accesses.

root nexus device for the CPUs as starting with UltraSPARC IV the 'cpu'
  nodes hang off of from 'cmp' (chip multi-threading processor) or 'core'
  or combinations thereof. Also in large UltraSPARC III based machines
  the 'cpu' nodes hang off of 'ssm' (scalable shared memory) nodes which
  group snooping-coherency domains together instead of directly from the
  nexus.
  It would be great if we could use newbus to deal with the different ways
  the 'cpu' devices can hang off of pseudo ones but unfortunately both
  cpu_mp_setmaxid() and sparc64_init() have to work prior to regular device
  probing.
- Add support for UltraSPARC IV and IV+ CPUs. Due to the fact that these
  are multi-core each CPU has two Fireplane config registers and thus the
  module/target ID has to be determined differently so the one specific
  to a certain core is used. Similarly, starting with UltraSPARC IV the
  individual cores use a different property in the OFW device tree to
  indicate the CPU/core ID as it no longer is in coincidence with the
  shared slot/socket ID.
  This involves changing the MD KTR code to not directly read the UPA
  module ID either. We use the MID stored in the per-CPU data instead of
  calling cpu_get_mid() as a replacement in order prevent clobbering any
  registers as side-effect in the assembler version. This requires CATR()
  invocations from mp_startup() prior to mapping the per-CPU pages to be
  removed though.
  While at it additionally distinguish between CPUs with Fireplane and
  JBus interconnects as these also use slightly different sizes for the
  JBus/agent/module/target IDs.
- Make sparc64_shutdown_final() static as it's not used outside of
  machdep.c.

dfade4c0a4 - At least the trap table of the Sun Fire V1280 firmware apparently has
no cleanwindows handler so just remove trying to trigger it from _start
  and the AP trampoline code as that leads to a crash there. This should
  be okay as leaking data from the OFW via the CPU registers on start of
  the kernel should be no real concern.
- Make the comments of _start and the AP trampoline code regarding the
  initializations they perform match each other and reality.
- Make the comments of the AP trampoline code regarding iTLB accesses
  refer to the right macro.

606e071c41 Use the SUNW,{d,i}tlb-load methods for entering locked TLB entries like
OpenBSD and OpenSolaris do instead of fiddling with the MMUs ourselves.
Unlike direct access the firmware methods don't automatically use the
next free (?) TLB slot, instead the slot to be used has to be specified.
We allocate the TLB slots for the kernel top-down as OpenSolaris suggests
that the firmware will always allocate the ones for its own use bottom-up.
Besides being simpler, according to OpenBSD using the firmware methods is
required to allow booting on Sun Fire E10K with multi-systemboard domains.

2950ff259c When ffs_realloccg() failed to allocate bigger fragment and, because
pending blocks are scheduled for removal, goes to retry the (re)allocation,
clear the bp pointer. It might happen that meantime free space is really
exhausted and we are entering nospace: label without bread()ing buffer,
causing stale bp value to be brelse()d again.

9dcae110dc Remove the PCI_IOSPACE_SIZE and PCI_IOSPACE_ADDR hack from nexus.c. Implement
this in the Sibyte PCI hostbridge driver instead.

The nexus driver sees resource allocation requests for memory and irq
resources only. These are legitimate resources on all MIPS platforms.

aa83516001 acpi cpu: probe+attach before all other enumerated children on acpi bus
Some current systems dynamically load SSDT(s) when _PDC/_OSC method
of Processor is evaluated.  Other devices in ACPI namespace may access
objects defined in the dynamic SSDT.  Drivers for such devices might
have to have a rather high priority, because of other dependencies.
Good example is acpi_ec driver for EC.
Thus we attach to Processors as early as possible to load the SSDTs
before any other drivers may try to evaluate control methods.
It also seems to be a natural order for a processor in a device
hierarchy.

On the other hand, some child devices on acpi cpu bus need to access
other system resources like PCI configuration space of chipset devices,
so they need to be probed and attached rather late.
For this reason we probe and attach the cpu bus at
SI_SUB_CONFIGURE:SI_ORDER_MIDDLE SYSINIT level.
In the future this could be done more elegantly via multipass.

Please note that acpi drivers that might access ACPI namespace from
device_identify will do that before _PDC/_OSC of Processors are evaluated.

Legacy cpu driver is not affected by this change.

e870d1e6f9 This fix corrects a problem in the file system that treats large
inode numbers as negative rather than unsigned. For a default
(16K block) file system, this bug began to show up at a file system
size above about 16Tb.

To fully handle this problem, newfs must be updated to ensure that
it will never create a filesystem with more than 2^32 inodes. That
patch will be forthcoming soon.

9518a2a361 If a mbuf is split across two pages, we
have code that detects this and makes two
transmit descriptors. However its possible
that the algorithm detects when the second
page is not used (when the data aligns perfectly
to the bottom of the page). This caused a 0
len descriptor to be added which locks up the
rge device. Skip such things with a continue.

ed6933b252 Enable interrupts before doing AST processing to avoid a deadlock.
Specifically on an SMP kernel it was observed that if both the
processors are doing an exit1() via ast()->postsig()->sigexit()
then we will deadlock.

This happens because exit1() calls vmspace_exit() that in turn
calls pmap_invalidate_all(). This function tries to do a
smp_rendezvous() which blocks because the other processor is not
responding to IPIs - because it too is doing AST processing with
interrupts disabled.

9ea01fedc0 - Move nfs_realign() from the NFS client to the shared NFS code and
remove the NFS server version in order to reduce code duplication.
  The shared version now uses a second parameter how, which is passed
  on to m_get(9) and m_getcl(9) as the server used M_WAIT while the
  client requires M_DONTWAIT, and replaces the the previously unused
  parameter hsiz.
- Change nfs_realign() to use nfsm_aligned() so as with other NFS code
  the alignment check isn't actually performed on platforms without
  strict alignment requirements for performance reasons because as the
  comment suggests unaligned data only occasionally occurs with TCP.

3e0490b3fe Add DDB support for printing vnet_sysinit and vnet_sysuninit
ordered call lists. Try to lookup function/symbol names and print
those in addition to the pointers, along with the constants for
subsystem and order.
This is useful for debugging vnet teardown ordering issues.

Make it possible to call the actual printing frunction from normal
code at runtime, ie. from vnet_sysuninit(), if DDB support is there.

61d033d436 Add an SDT provider for "vnet"s along with probes for vnet_alloc
and vnet_destroy.
Use the line number rather than NULL as dummy argument.

Note: the fbt provider does not reliably provide :return probes
(depending on optimization levels used at compile time) making
it unusable for scripts to generate complete call-traces with
well defined boundaries over allocations or destructions of
virtual network stacks.

90b4621a5f Add PT_VM_TIMESTAMP and PT_VM_ENTRY so that the tracing process can

530a5fb294 Improve checking whether an ARM VA has a valid mapping before performing cache
sync.

VIPT/PIPT caches need valid VA-PA mapping in PTE for a cache operation to
succeed (unlike VIVT). Prior to this fix pmap was using l2pte_valid() for that
check, but this is not sufficient as the function merely checks if a PTE
exists (there can be existing but _invalid_ entries in the table).

A new pmap_has_valid_mapping() routine is introduced to do this job right by
checking proper PTE flags.

Among other potential problems this cures coherency issues with L2 caches on
MV-78100.

58ce165dfd Fix single-stepping when the kernel was entered through the EPC syscall
path. When the taken branch leaves the kernel and enters the process,
we still need to execute the instruction at that address. Don't raise
SIGTRAP when we branch into the process, but enable single-stepping

f4ab0cccce acpi_cpu: prefer _OSC over _PDC, just in case
_PDC was deprecated in favor of _OSC long time ago, but it
seems that they still peacefully coexist and in some case
only _PDC is present.
Still _OSC provides a reacher interface and is capable to
report back its status.
If the status is non-zero, then report it, we may find
it useful to understand what firmware expects from OS.
Also clean up some comments that became less useful over time.

c3f7e882dc Reimplement all functions to access the system control unit in C.
The only reason we need to have the sb_load64() and sb_store64()
functions in assembly is to cheat the compiler and generate the
'ld' and 'sd' instructions which it otherwise will not do when
compiling for a 32-bit architecture. There are some 64-bit
registers in the SCD unit that must be accessed using 64-bit
load and store instructions.

0a705ab66f Instead of spamming the console on each curvnet recursion event, print
out each such call graph only once, along with a stack backtrace.  This
should make kernels built with VNET_DEBUG reasonably usable again in
busy / production environments.

Introduce a new DDB command "show vnetrcrs" which dumps the whole log
of distinctive curvnet recursion events.  This might be useful when
recursion reports get burried / lost too deep in the message buffer.
In the later case stack backtraces are not available.

c8b8a2c4e6 Redo how commands handles are created and managed and implement sequence
numbers and handle types in rational way. This will better protect from
(unwittingly) dealing with stale handles/commands.

Fix the watchdog timeout code to better protect itself from mistakes.

If we run an abort on a putatively timed out command, the command
may in fact get completed, so check to make sure the command we're
timing it out is still around. If the abort succeeds, btw, the command
should get returned via a different path.

ac0797c3d1 GPIB overhaul, part #2: make the tnt4882 driver work with the newer
TNT5004 IC.  This involved a major rewrite of a number of things, as
this chip no longer supports the NAT7210 legacy mode but requires the
host to use the (more modern) FIFO mode.

In theory, this also ought to work on the older TNT4882C chip.  I'll
probably add this as optional support (perhaps by a device.hints flag)
later on.  By now, FIFO mode is *only* activates iff a TNT5004 chip
has been detected (where the old code didn't work at all), while
everything else is supposed to use the old code.

dc5fd2595c use u_char instead of u_int for short bitfields.
For our compiler the two constructs are completely equivalent, but
some compilers (including MSC and tcc) use the base type for alignment,
which in the cases touched here result in aligning the bitfields
to 32 bit instead of the 8 bit that is meant here.

Note that almost all other headers where small bitfields
are used have u_int8_t instead of u_int.

This may not be entirely correct either, but the existing check is

c2a5f1a57a - Check if_type of "addm <interface>" before setting the
interface's MTU to the if_bridge(4) interface.  This fixes a
  bug that MTU value of "addm <interface>" is used even when it
  is invalid for the if_bridge(4) member:

- Implement SIOCSIFMTU ioctl.  Changing the MTU is allowed only
  when all members have the same MTU value.

9dd3fbb0f2 Provide access to pcpu structures for SMP kernels.
The basic idea is to use a the same virtual address as a window onto
distinct physical memory locations - one per processor. The physical
address that you access through this mapping depends on which cpu you
are currently executing on. We can now use the same virtual address
on any processor to access its per-cpu area.

The details are:

- The virtual address for 'struct pcpu *pcpup' is obtained by
  stealing 2 pages worth of KVA in pmap_bootstrap().

- The mapping from the constant virtual address to a distinct
  physical page is done in cpu_pcpu_init() through a wired TLB entry.

- A side-effect of this is that we reserve 2 pages worth of memory
  for the pcpu but in reality it needs much less than that. The unused
  memory is now used as the boot stack for the BSP and APs.

Remove SMP-specific bits from locore.S. The plan is to use a separate
mpboot.S for AP bootstrap.

146f25649d Fix for kern/141646: when stacking pseudo drivers like
lagg and vlan the vlan attach/detach event is not being
handed down to em, this caused some init code not to run,
and thus VLANs did not work. Ultimately having the event
get propagated would be nice, but for now the solution is
to have HWFILTER off by default, when this is the case
VLANs will work, ifconfig can be used to turn it on and
then get HW tag filtering.

8429f8e8f4 Don't check the device ID. Instead, check the class, subclass and
programming I/F. New SoC designs have different device IDs, but
don't need special treatment. Consequently, we fail to probe and
attach for no other reason than not having added the device ID to
the code.

Bank on Freescale's sense of backward compatibility and assume
that if we find a host controller, we know how work with it.

This fixes detection of the PCI Express host controllers on
Freescale's QorIQ family of processors (P1, P2 and P4).

dec3772ee4 Add "maxfilesize" mount option for tmpfs to allow specifying the
maximum file size limit. Default is UINT64_MAX when the option is
not specified. It was useless to set the limit to the total amount of
memory and swap in the system.

Use tmpfs_mem_info() rather than get_swpgtotal() in tmpfs_mount() to
check if there is enough memory available.

Remove now unused get_swpgtotal().

f3d3bf8731 Add support for the AR9285 chipset, which is found on many netbooks
available today.

This card is a low power 802.11bgn that only does 11n rates up to MCS 7
(that's 65 Mbps in 20Mhz mode and 135 in 40Mhz mode).
802.11n is  not yet supported, but will be in the future.

The driver still has a problem regarding to the setting of txpower on
the card, so don't expect good performance yet. After fixing this
problem, an MFC is possible.

0eaed6efad For our memory re-mapping trick to work
interrupts must be disabled through the
page_zero's or copys etc. Note that the
temporary mapping used by panic's may
cause us pain since int's may not be disabled.
When we get dumps working we may have to revist
this. Note that with this fix the build got
much much further.. until it hung on disk IO (I
would imagine thats the rge/msgring driver acting
up).

e7017e7b1a Fix two of the extended memory hacks. The copy pages
routine in one place was setting the valid2 bit to
2 not 1. This meant the PTE was NOT valid and so
you would crash.

In Zero Page there was a incorrect setting of
the valid bit AFTER the actual zero (opps)..

Hopefully this will fix the 0xc0000000 crashes
that I have been seeing (unless of course there are
other problems with these old hacks of mine to get
to memory above 512Meg)

83c5d981ac MFp4: Large set of CAM inprovements.
- Unify bus reset/probe sequence. Whenever bus attached at boot or later,
CAM will automatically reset and scan it. It allows to remove duplicate
code from many drivers.
- Any bus, attached before CAM completed it's boot-time initialization,
will equally join to the process, delaying boot if needed.
- New kern.cam.boot_delay loader tunable should help controllers that
are still unable to register their buses in time (such as slow USB/
PCCard/ CardBus devices), by adding one more event to wait on boot.
- To allow synchronization between different CAM levels, concept of
requests priorities was extended. Priorities now split between several
"run levels". Device can be freezed at specified level, allowing higher
priority requests to pass. For example, no payload requests allowed,
until PMP driver enable port. ATA XPT negotiate transfer parameters,
periph driver configure caching and so on.
- Frozen requests are no more counted by request allocation scheduler.
It fixes deadlocks, when frozen low priority payload requests occupying
slots, required by higher levels to manage theit execution.
- Two last changes were holding proper ATA reinitialization and error
recovery implementation. Now it is done: SATA controllers and Port
Multipliers now implement automatic hot-plug and should correctly
recover from timeouts and bus resets.
- Improve SCSI error recovery for devices on buses without automatic sense
reporting, such as ATAPI or USB. For example, it allows CAM to wait, while
CD drive loads disk, instead of immediately return error status.
- Decapitalize diagnostic messages and make them more readable and sensible.
- Teach PMP driver to limit maximum speed on fan-out ports.
- Make boot wait for PMP scan completes, and make rescan more reliable.
- Fix pass driver, to return CCB to user level in case of error.
- Increase number of retries in cd driver, as device may return several UAs.

376a67cf64 - Zero the MSI/MSI-X queue argument, otherwise mtx_init(9) can panic
indicating an already initialized lock.
- Check for an empty MSI/MSI-X queue entry before asserting that we have

  MSI-X queue interrupts which may happen in case of using an interrupt
  handler rather than a filter.

b3021b932a Optimize pmap_demote_pde() by using the new KPTmap to access a kernel
page table page instead of creating a temporary mapping to it.

Set the PG_G bit on the page table entries that implement the KPTmap.

Locore initializes the unused portions of the NKPT kernel page table
pages that it allocates to zero.  So, pmap_bootstrap() needn't zero
the page table entries referenced by CMAP1 and CMAP3.

Simplify pmap_set_pg().

dbf6fb7226 Do not leave dirty cache lines behind if bus_dmamap_sync was called
to invalidate memory chunk that starts or ends in the middle of
cache line.

This was responsible for one half of the problem preventing umass
to work reliably on some MIPS32 platforms. USBng needs to stop
sharing cache lines between DMA-able memory and other structures
to cure the other half.

651ff543f8 Fix a race that can occur when nfs nfsiod threads are being created.
Without this patch it was possible for a different thread that calls
nfs_asyncio() to snitch a newly created nfsiod thread that was
intended for another caller of nfs_asyncio(), because the nfs_iod_mtx
mutex was unlocked while the new nfsiod thread was created. This patch
labels the newly created nfsiod, so that it is not taken by another
caller of nfs_asyncio(). This is believed to fix the problem reported
on the freebsd-stable email list under the subject:
FreeBSD NFS client/Linux NFS server issue.

9a6a6ecb3a acpi_hpet: correctly get number of timers/comparators in a timer block
Also, account for a quirk of AMD/ATI HPET which reports number of timers
instead of id of the last timer as manadated by the specification.
Currently this has no effect on functionality but in the future we may
make actual use of the HPET timers, not only of its timecounter.

a69ed8dfb3 Update the 1G drivers, shared code sync with Intel,
igb now has a queue notion that has a single interrupt
with an RX/TX pair, this will reduce the total interrupts
seen on a system. Both em and igb have a new watchdog
method. igb has fixes from Pyun Yong-Hyeon that have
improved stability, thank you :)

I wish to MFC this for 7.3 asap, please test if able.

6268666c1b Add support for SATA part of Marvell 88SE912x controllers to ahci(4).
Limit early revisions from 6Gb/s to 3Gb/s by default, or they negotiate
only 1.5Gbps, when 3Gb/s devices connected.

Add dummy driver for PATA part of these controllers, preventing generic
driver attach them. It causes system freeze when SATA controller used after
PATA was touched.

61f7c762de Create the "cfecons" tty directly using tty_makedev(). It is not clear what
the intention of having two ttys pointing to the same cfe console device was.

Also we were not initializing the output[] array passed in as input to
tty_makedev() so one name of the ttys was garbage.

Fix the code that calls cfe_write() to deal with the case where only a partial
buffer is written out.

cfe_cngetc() needs to return if there is no character available as input.
If we don't do this then the cfe_timeout() function will spin forever
because cfe_cngetc() will only ever return if there is valid input.

d87b96dd84 Install the XTLB exception handler for Sibyte processors.
This is a workaround for the fact that the CFE is compiled as a 64-bit
application and therefore sets the SR_KX bit every time we call into
it (for e.g. console).

A TLB miss for any address above 0xc0000000 with the SR_KX bit set will
end up at the XTLB exception vector. We workaround this by copying the
standard TLB handler at the XTLB exception vector.

49396cced3 Fix a problem seen when a new process was returning to userland
through fork_trampoline.

This was caused because we were clearing the SR_INT_IE and setting
SR_EXL bits of the status register at the same time. This meant
that if an interrupt happened while this MTC0 was making its way
through the pipeline the exception processing would see the
status register with SR_EXL bit set. This in turn would mean that
the COP_0_EXC_PC would not be updated so the return from exception
would be to an incorrect address.

It is easy to verify this fix by a program that forks in a loop
and the child just exits:
