b5eb64d6f0 Pre-KSE/M3 commit.
this is a low-functionality change that changes the kernel to access the main
thread of a process via the linked list of threads rather than
assuming that it is embedded in the process. It IS still embeded there
but remove all teh code that assumes that in preparation for the next commit
which will actually move it out.

156f4c8aea Fixes for alpha pmap on SMP machines:
- Create a private list of active pmaps rather than abusing the list of all
  processes when we need to look up pmaps.  The process list needs a sx lock
  and we can't be getting sx locks in the middle of cpu_switch()
  (pmap_activate() can call pmap_get_asn() from cpu_switch()).  Instead, we
  protect the list with a spinlock.  This also means the list is shorter
  since a pmap can be used by more than one process and we could (at least
  in thoery) dink with pmap's more than once, but now we only touch each
  pmap once when we have to update all of them.
- Wrap pmap_activate()'s code to get a new ASN in an explicit critical section
  so that when it is called while doing an exec() we can't get preempted.
- Replace splhigh() in pmap_growkernel() with a critical section to prevent
  preemption while we are adjusting the kernel page tables.
- Fixes abuse of PCPU_GET(), which doesn't return an L-value.
- Also adds some slight cleanups to the ASN handling by adding some macros
  instead of magic numbers in relation to the ASN and ASN generations.

199578d7e8 Compile osigreturn() unconditionally since it will always be needed on
some arches and the syscall table is machine-independent.  It was
(bogusly) conditional on COMPAT_43, so this usually makes no difference.

ia64: in addition:
- replace the bogus cloned comment before osigreturn() by a correct one.
  osigreturn() is just a stub fo ia64's.
- fix the formatting of cloned comment before sigreturn().
- fix the return code.  use nosys() instead of returning ENOSYS to get
  the same semantics as if the syscall is not in the syscall table.
  Generating SIGSYS is actually correct here.
- fix style bugs.

powerpc: copy the cleaned up ia64 stub.  This mainly fixes a bogus comment.

sparc64: copy the cleaned up the ia64 stub, since there was no stub before.

a91be5985c Linux/alpha uses the same BSDish return mechanism we do for
getpid, getuid, getgid and pipe, since they bootstrapped from
OSF/1 and never cleaned up.  Switch to the native syscalls
on alpha so that the above functions work

30e4a25f30 Allow alpha kernels to compile again- make sure opt_ddb.h is included
and the reference to db_regs is *extern* from alpha/include/db_machdep.h
(put it in alpha/alpha/machdep.c)- this avoids the problems we've had
about different 'common' sizes prohibiting the kernel from linking.

873822cd83 - Catch up printtrap() to KSE by using curthread and testing it against
NULL rather than curproc.  Without this, if we trap early before
  curthread is set, we recursively panic.
- In an SMP kernel, if we trap before curthread is set, then trap is going
  to recursively panic trying to bump td->td_md.md_kernnest.  The trap is
  fatal anyways, so to make debugging easier just call printtrap() to
  dump the trap info to the console and then halt.

844237b396 SMP Lock struct file, filedesc and the global file list.
Seigo Tanimura (tanimura) posted the initial delta.

I've polished it quite a bit reducing the need for locking and
adapting it for KSE.

Locks:

1 mutex in each filedesc
   protects all the fields.
   protects "struct file" initialization, while a struct file
     is being changed from &badfileops -> &pipeops or something
     the filedesc should be locked.

1 mutex in each struct file
   protects the refcount fields.
   doesn't protect anything else.
   the flags used for garbage collection have been moved to
     f_gcflag which was the FILLER short, this doesn't need
     locking because the garbage collection is a single threaded
     container.
  could likely be made to use a pool mutex.

1 sx lock for the global filelist.

b11ad14744 Use a spare slot in the machine context to identify the context format
and to inherently verify its validity.  Alpha signal frames and trap
frames are different; this field identifies which format the context
is

Set the machine context format (signal frame) before copying it out
when sending a signal.

1ce407b675 Change the preemption code for software interrupt thread schedules and
mutex releases to not require flags for the cases when preemption is
not allowed:

The purpose of the MTX_NOSWITCH and SWI_NOSWITCH flags is to prevent
switching to a higher priority thread on mutex releease and swi schedule,
respectively when that switch is not safe.  Now that the critical section
API maintains a per-thread nesting count, the kernel can easily check
whether or not it should switch without relying on flags from the
programmer.  This fixes a few bugs in that all current callers of
swi_sched() used SWI_NOSWITCH, when in fact, only the ones called from
fast interrupt handlers and the swi_sched of softclock needed this flag.
Note that to ensure that swi_sched()'s in clock and fast interrupt
handlers do not switch, these handlers have to be explicitly wrapped
in critical_enter/exit pairs.  Presently, just wrapping the handlers is
sufficient, but in the future with the fully preemptive kernel, the
interrupt must be EOI'd before critical_exit() is called.  (critical_exit()
can switch due to a deferred preemption in a fully preemptive kernel.)

I've tested the changes to the interrupt code on i386 and alpha.  I have
not tested ia64, but the interrupt code is almost identical to the alpha
code, so I expect it will work fine.  PowerPC and ARM do not yet have
interrupt code in the tree so they shouldn't be broken.  Sparc64 is
broken, but that's been ok'd by jake and tmm who will be fixing the
interrupt code for sparc64 shortly.

2463f40fc3 Introduce a standard name for the lock protecting an interrupt controller
and it's associated state variables: icu_lock with the name "icu".  This
renames the imen_mtx for x86 SMP, but also uses the lock to protect
access to the 8259 PIC on x86 UP.  This also adds an appropriate lock to
the various Alpha chipsets which fixes problems with Alpha SMP machines
dropping interrupts with an SMP kernel.

a3b98398cb Modify the critical section API as follows:
- The MD functions critical_enter/exit are renamed to start with a cpu_
  prefix.
- MI wrapper functions critical_enter/exit maintain a per-thread nesting
  count and a per-thread critical section saved state set when entering
  a critical section while at nesting level 0 and restored when exiting
  to nesting level 0.  This moves the saved state out of spin mutexes so
  that interlocking spin mutexes works properly.
- Most low-level MD code that used critical_enter/exit now use
  cpu_critical_enter/exit.  MI code such as device drivers and spin
  mutexes use the MI wrappers.  Note that since the MI wrappers store
  the state in the current thread, they do not have any return values or
  arguments.
- mtx_intr_enable() is replaced with a constant CRITICAL_FORK which is
  assigned to curthread->td_savecrit during fork_exit().

05dfe86282 Small cleanups to the SMP code:
- Axe inlvtlb_ok as it was completely redundant with smp_active.
- Remove references to non-existent variable and non-existent file
  in i386/include/smp.h.
- Don't perform initializations local to each CPU while holding the
  ap boot lock on i386 while an AP bootstraps itself.
- Reorganize the AP startup code some to unify the latter half of the
  functions to bring an AP up.  Eventually this might be broken out into
  a MI function in subr_smp.c.

21b6b26912 Overhaul the per-CPU support a bit:
- The MI portions of struct globaldata have been consolidated into a MI
  struct pcpu.  The MD per-CPU data are specified via a macro defined in
  machine/pcpu.h.  A macro was chosen over a struct mdpcpu so that the
  interface would be cleaner (PCPU_GET(my_md_field) vs.
  PCPU_GET(md.md_my_md_field)).
- All references to globaldata are changed to pcpu instead.  In a UP kernel,
  this data was stored as global variables which is where the original name
  came from.  In an SMP world this data is per-CPU and ideally private to each
  CPU outside of the context of debuggers.  This also included combining
  machine/globaldata.h and machine/globals.h into machine/pcpu.h.
- The pointer to the thread using the FPU on i386 was renamed from
  npxthread to fpcurthread to be identical with other architectures.
- Make the show pcpu ddb command MI with a MD callout to display MD
  fields.
- The globaldata_register() function was renamed to pcpu_init() and now
  init's MI fields of a struct pcpu in addition to registering it with
  the internal array and list.
- A pcpu_destroy() function was added to remove a struct pcpu from the
  internal array and list.

95eb5d80c2 Merge rev 1.264 from i386/pmap.c (tegge via alfred):
Protect against an infinite loop when prefaulting pages.  This can
happen when the vm system maps past the end of an object or tries
to map a zero length object, the pmap layer misses the fact that
offsets wrap into negative numbers and we get stuck.

56c06d9e26 Merge rev 1.202 from i386/pmap.c (back in 1998 by John Dyson):
Make flushing dirty pages work correctly on filesystems that
unexpectedly do not complete writes even with sync I/O requests.
This should help the behavior of mmaped files when using
softupdates (and perhaps in other circumstances also.)

4aaee16141 As part of a general cleanup and reconvergence of related pmap code,

f8c48b0904 Introduce a boot environment variable (clock_compat_osf1) which can
be set to 1 to make FreeBSD and Tru64 coexist peacefully on a dual
boot system and not clobber each other's year in the TOY clock.
(Tru64 uses an offset 52 years higher than one would expect)

3df91ed4db o Add new header <sys/stdint.h>.
o Make <stdint.h> a symbolic link to <sys/stdint.h>.
o Move most of <sys/inttypes.h> into <sys/stdint.h>, as per C99.
o Remove <sys/inttypes.h>.
o Adjust includes in sys/types.h and boot/efi/include/ia64/efibind.h
  to reflect new location of integer types in <sys/stdint.h>.
o Remove previously symbolicly linked <inttypes.h>, instead create a
  new file.
o Add MD headers <machine/_inttypes.h> from NetBSD.
o Include <sys/stdint.h> in <inttypes.h>, as required by C99; and
  include <machine/_inttypes.h> in <inttypes.h>, to fill in the
  remaining requirements for <inttypes.h>.
o Add additional integer types in <machine/ansi.h> and
  <machine/limits.h> which are included via <sys/stdint.h>.

81577a9eef "Fix" a problem that got copied from alpha to ia64 and broke there.
When we truncate the msgbuf size because the last chunk is too small,
correctly terminate the phys_avail[] array - the VM system tests
the *end* for zero, not the start.  This leads the VM startup to
attempt to recreate a duplicate set of pages for all physical memory.

XXX the msgbuf handling is suspiciously different on i386 vs
alpha/ia64...

f5a4c275f5 Eliminate the prefix parameter to linux_emul_find(), which was always
linux_emul_path anyway.  Linux_emul_find() has interesting bugs in its
prefix handling (which luckily are not currently exploitable); this
commit is preliminary to an attempt at cleaning it up.

c55c6300c8 Detach the prom console when platform.cons_init is called. This seems
to avoid most of the double character kernel goop we've been having by having
both a prom console && a normal console.

Was not able to test with graphics head. Hope this doesn't break anything.

46e3f92a5d Add a per-thread ucred reference for syscalls and synchronous traps from
userland.  The per thread ucred reference is immutable and thus needs no
locks to be read.  However, until all the proc locking associated with
writes to p_ucred are completed, it is still not safe to use the per-thread
reference.

d9b89a94d8 - splhigh()/splx() -> critical_enter()/critical_exit()
- fix KV macro in t2_pci.c to include the sable_lynx_base variable
so that the T2 CSRs can be found on lynxes.   Current should be
bootable on lynxes now.

1d7ce16d9a Add support for "__gnuc_va_list". Some overly "smart" libraries assume
the existence of the __gnuc_va_list type[*] because our compiler is GCC.

[*] __gnuc_va_list is defined in the GCC ginclude/stdarg.h replacement
headerwhich we don't use.

defines it as a short.  Change that to an array of one short so that FreeBSD's

29c5c858fc Dissociate ptrace from procfs.
Until now, the ptrace syscall was implemented as a wrapper that called
various functions in procfs depending on which ptrace operation was
requested.  Most of these functions were themselves wrappers around
procfs_{read,write}_{,db,fp}regs(), with only some extra error checks,
which weren't necessary in the ptrace case anyway.

This commit moves procfs_rwmem() from procfs_mem.c into sys_process.c
(renaming it to proc_rwmem() in the process), and implements ptrace()
directly in terms of procfs_{read,write}_{,db,fp}regs() instead of
having it fake up a struct uio and then call procfs_do{,db,fp}regs().

It also moves the prototypes for procfs_{read,write}_{,db,fp}regs()
and proc_rwmem() from proc.h to ptrace.h, and marks all procfs files
except procfs_machdep.c as "optional procfs" instead of "standard".

aee394ff68 Stop using linux_getpgid(). The implementation at this time is
broken and fixing it only creates a duplicate of what is already
in the FreeBSD kernel. Therefore, map the syscall directly to
getpgid().

will be private to each CPU.

c727378338 Fix off by one error introduced by the use of the ifnet_byindex()
macro. The commit log clearly states that the index given to the
macro is one higher than previously used to index the array. This
wasn't represented in the code and resulted in kernel page faults.

Note ALL MODULES MUST BE RECOMPILED
make the kernel aware that there are smaller units of scheduling than the
process. (but only allow one thread per process at this time).
This is functionally equivalent to teh previousl -current except
that there is a thread associated with each process.

96b9a12bd2 Rip some well duplicated code out of cpu_wait() and cpu_exit() and move
it to the MI area.  KSE touched cpu_wait() which had the same change
replicated five ways for each platform.  Now it can just do it once.
The only MD parts seemed to be dealing with fpu state cleanup and things
like vm86 cleanup on x86.  The rest was identical.

XXX: ia64 and powerpc did not have cpu_throw(), so I've put a functional
stub in place.

774ebbef7e Fix some problems with the reference/modified tracking which I introduced
with the last change to the way the pmap_emulate_reference() works. This
should fix a number of memory corruption problems and also should stop the
mtimes of executables changing all the time.

df61d9eb64 Round of cleanups and enhancements. These include (in random order):
o  Introduce private types for use in linux syscalls for two reasons:
   1. establish type independence for ease in porting and,
   2. provide a visual queue as to which syscalls have proper
      prototypes to further cleanup the i386/alpha split.
   Linuxulator types are prefixed by 'l_'. void and char have not
   been "virtualized".

o  Provide dummy functions for all syscalls and remove dummy functions
   or implementations of truely obsolete syscalls.

o  Sanitize the shm*, sem* and msg* syscalls.

o  Make a first attempt to implement the linux_sysctl syscall. At this
   time it only returns one MIB (KERN_VERSION), but most importantly,
   it tells us when we need to add additional sysctls :-)

o  Bump the kenel version up to 2.4.2 (this is not the same as the
   KERN_VERSION MIB, BTW).

o  Implement new syscalls, of which most are specific to i386. Our
   syscall table is now up to date with Linux 2.4.2. Some highlights:
   -  Implement the 32-bit uid_t and gid_t bases syscalls.
   -  Implement a couple of 64-bit file size/offset bases syscalls.

o  Fix or improve numerous syscalls and prototypes.

o  Reduce style(9) violations while I'm here. Especially indentation
   inconsistencies within the same file are addressed. Re-indenting
   did not obfuscate actual changes to the extend that it could not
   be combined.

NOTE: I spend some time testing these changes and found that if there
      were regressions, they were not caused by these changes AFAICT.
      It was observed that installing a RH 7.1 runtime environment
      did make matters worse. Hangs and/or reboots have been observed
      with and without these changes, so when it failed to make life
      better in cases it doesn't look like it made it worse.

   the osf1 syscalls. This can only be done if the osfulator
   implements at least all syscalls used by the linuxulator. Remove
   dummy functions for syscalls that are now truely unimplemented.

o  Set the syscall namespace as follows: Mark a syscall as OSF1 if
   the Linux kernel has prefixed the syscall with 'osf_' and has
   provided special implementations for it. Otherwise mark the
   syscall as LINUX by default. Some of the LINUX syscalls remain
   marked as BSD or POSIX.

o  Rename syscalls so they match the names used in the Linux kernel.
   Also, provide more accurate prototypes. This generally improves
   cross-referencing and reduces head-scratching.

o  Fix the (g|s)etresuid syscalls. They mapped to (g|s)etresgid.

o  Sanitize the the shm*, sem* and msg* syscalls. Their prototypes
   were dictated by the way these syscalls were used in the i386
   code. That has been fixed. NOTE: linux_semctl now passes it's
   'arg' parameter by value and not by reference.

o  Fix prototype of linux_utime. It takes a struct timeval, not a
   struct utimbuf.

o  Fix the linux_sysfs syscall. It's index is not 255, but 254.

o  Implement the following syscalls:
     linux_sysctl

o  Add the following new syscalls:
     (g|s)etresgid
     linux_pivot_root (dummy)
     linux_mincore (dummy)
     linux_pciconfig_iobase (dummy)
     linux_getdents64

08e732a88b Remove the MPSAFE keyword from the parser for syscalls.master.
Instead introduce the [M] prefix to existing keywords.  e.g.
MSTD is the MP SAFE version of STD.  This is prepatory for a
massive Giant lock pushdown.  The old MPSAFE keyword made
syscalls.master too messy.

Begin comments MP-Safe procedures with the comment:
/*
 * MPSAFE
 */
This comments means that the procedure may be called without
Giant held (The procedure itself may still need to obtain
Giant temporarily to do its thing).

sv_prepsyscall() is now MP SAFE and assumed to be MP SAFE
sv_transtrap() is now MP SAFE and assumed to be MP SAFE

ktrsyscall() and ktrsysret() are now MP SAFE (Giant Pushdown)
trapsignal() is now MP SAFE (Giant Pushdown)

Places which used to do the if (mtx_owned(&Giant)) mtx_unlock(&Giant)
test in syscall[2]() in */*/trap.c now do not.  Instead they
explicitly unlock Giant if they previously obtained it, and then
assert that it is no longer held to catch broken system calls.

Rebuild syscall tables.

a45063618a o Remove some GCCisms in src/powerpc/include/endian.h.
o Unify <machine/endian.h>'s across all architectures.
o Make bswapXX() functions use a different spelling of u_int16_t and
  friends to reduce namespace pollution.  The bswapXX() functions
  don't actually exist, but we'll probably import these at some
  point.  Atleast one driver (if_de) depends on bswapXX() for big
  endian cases.
o Deprecate byteorder(3) prototypes from <sys/types.h>, these are
  now prototyped indirectly in <arpa/inet.h>.
o Deprecate in_addr_t and in_port_t typedefs in <sys/types.h>, these
  are now typedef'd in <arpa/inet.h>.
o Change byteorder(3) prototypes to use standards compliant uint32_t
  (spelled __uint32_t to reduce namespace pollution).
o Document new preferred headers and standards compliance.

0f92f56bab Teach bus_dmamem_free() about contigfree(). This is a bit of a hack,
but it's better than the buggy behavior we have now. If we contigmalloc()
buffers in bus_dmamem_alloc(), then we must configfree() them in
bus_dmamem_free(). Trying to free() them is wrong, and will cause
a panic (at least, it does on the alpha.)

I tripped over this when trying to kldunload my busdma-ified if_rl
driver.

6b7f805e47 Fix yet another "unconditionally free()ing memory without even checking
to see if it was malloc()ed first" bug. In bus_dmamap_create(), one of
two things can happen: either we need to allocate a special map due to
some manner of bounce buffering requirement, or we can DMA a buffer
in place. On the x86 platform, the "in place" case results in
bus_dmamap_create() returning a dmamap of NULL. The bus_dmamap_destroy()
routine later checks for NULL and won't bother free()ing the map if
it detects this condition.

But on the alpha, we don't use NULL, we use a statically allocated map
called nobounce_dmamap(). Unfortunately, bus_dmamap_destroy() does not
handle the condition where we attempt to destroy such a map: it tries
to free() the dmamap, which causes a panic.

Fix: test that map != &nobounce_dmamap before trying to free() it.

With this fix, my busdma-ified if_sis driver works on the alpha. I'm
a bit alarmed that I'm the first person ever to trip over this bug, since
we have been using busdma on the alpha for a while, and since it sort
of screams out "Hi! I'm a bug! Booga-booga!" when you look at it.

(Somewhere, somebody will say: "But Bill, why don't you just not bother
destroying the maps in this case." Because the API is supposed to be
a) symetrical and b) opaque to the caller. I can't know whether it's safe
to skip the bus_dmamap_destroy() step or not without sticking my fingers
into unsafe places, which is what I wanted to avoid in the first place.)

fed209ef95 In prom_halt(), set the halt restart flags on the current CPU, not the
boot CPU.  This was the reason reboots on SMP systems could result in
weird hangs.  Unlike the x86, we do not need to switch back to the boot
CPU in order to reboot the machine.  See Section 3.4.5 of Part III
(Console Interface Architecture) from the Alpha Architecture Reference
Manual (aka the Brown Book) for more info.

4a89454dcd - Close races with signals and other AST's being triggered while we are in
the process of exiting the kernel.  The ast() function now loops as long
  as the PS_ASTPENDING or PS_NEEDRESCHED flags are set.  It returns with
  preemption disabled so that any further AST's that arrive via an
  interrupt will be delayed until the low-level MD code returns to user
  mode.
- Use u_int's to store the tick counts for profiling purposes so that we
  do not need sched_lock just to read p_sticks.  This also closes a
  problem where the call to addupc_task() could screw up the arithmetic
  due to non-atomic reads of p_sticks.
- Axe need_proftick(), aston(), astoff(), astpending(), need_resched(),
  clear_resched(), and resched_wanted() in favor of direct bit operations
  on p_sflag.
- Fix up locking with sched_lock some.  In addupc_intr(), use sched_lock
  to ensure pr_addr and pr_ticks are updated atomically with setting
  PS_OWEUPC.  In ast() we clear pr_ticks atomically with clearing
  PS_OWEUPC.  We also do not grab the lock just to test a flag.
- Simplify the handling of Giant in ast() slightly.

bb5c43c4b8 Zap 'ptrace(PT_READ_U, ...)' and 'ptrace(PT_WRITE_U, ...)' since they
are a really nasty interface that should have been killed long ago
when 'ptrace(PT_[SG]ETREGS' etc came along.  The entity that they
operate on (struct user) will not be around much longer since it
is part-per-process and part-per-thread in a post-KSE world.

gdb does not actually use this except for the obscure 'info udot'
command which does a hexdump of as much of the child's 'struct user'
as it can get.  It carries its own #defines so it doesn't break
compiles.

21b80f4133 Use a machine dependent type, Elf_Hashelt, for the elements of the elf
dynamic symbol table buckets and chains.  The sparc64 toolchain uses 32
bit .hash entries, unlike other 64 bits architectures (alpha), which use
64 bit entries.

0caeab3ccd - Do not handle the per-CPU containers in mbuf code as though the cpuids
were indices in a dense array. The cpuids are a sparse set and treat
  them as such, setting up containers only for CPUs activated during
  mb_init().

- Fix netstat(1) and systat(1) to treat the per-CPU stats area as a sparse
  map, in accordance with the above.

This allows us to properly boot with certain CPUs disactivated. However, if
we later decide to re-activate said CPUs, we will barf until we decide to
implement CPU spinon/spinoff callback hooks to allow for said CPUs' per-CPU
containers to get configured on their activation.

f8df84f8c5 Simplify the implementation of pmap_emulate_reference(). The new version
simply manipulates the pte which faulted instead of traversing the mapping
list for that page. This makes it possible to complete the trap without
needing locks and incidentally improves the accuracy of some statistics
used by the VM system.

This commit adds driver support for 3Com 3cR990 10/100 ethernet
adapters based on the Typhoon I and Typhoon II chipsets. This is actually
a port of the OpenBSD driver with many hacks by me.

No Virginia, there isn't any support for the hardware crypto yet. However
there is support for TCP/IP checksum offload and VLANs.

Special thanks go to Jason Wright, Aaron Campbell and Theo de Raadt for
squeezing enough info out of 3Com to get this written, and for doing
most of the hard work.

Manual page is included. Compiled as a module and included in GENERIC.

c37f9bf14e Turn off preemption on the Alpha for the time being. Peter and JHB aren't
sure when things got so bad (JHB says preemption worked just fine for months
before the AlbertVM commit).  Even post DillionVM locking commit, Miatas
(DEC Personal Workstations) are very fragile -- not making it thru a world
build.  With this patch it does.

Those hacking on SMPng will want to locally back out this commit.  The rest
of us will want to run with it until the SMPng guys figure out the problem(s).

59f5dd5135 Fix reboot hangs that have happened with multiple processors
on Alpha 4100s.

Basically, if you're halting or you're rebooting, you should
tell all other processors to halt first. Define IPI_HALT- IPI_STOP
is not what we want for this purpose, which will call prom_halt(0)
on receipt.

The processor running the halt or reboot wil send an IPI_HALT to all
other processors, delay a bit, then continue to do what what it was
planning on doing (prom_halt({0|1})).

2d388a1550 Hints processing is FUBAR'ed right now.
By default, we will end up with a duplicate set of hints if people have
a properly populated /boot/device.hints.  So for now, remove the hints
here until Peter revisits the new hints processing from mid-June that
broke Alpha booting.

93369f554a Reorg vm_page.c into vm_page.c, vm_pageq.c, and vm_contig.c (for contigmalloc).
Also removed some spl's and added some VM mutexes, but they are not actually
used yet, so this commit does not really make any operational changes
to the system.

vm_page.c relates to vm_page_t manipulation, including high level deactivation,
activation, etc...  vm_pageq.c relates to finding free pages and aquiring
exclusive access to a page queue (exclusivity part not yet implemented).
And the world still builds... :-)

e028603b7e With Alfred's permission, remove vm_mtx in favor of a fine-grained approach
(this commit is just the first stage).  Also add various GIANT_ macros to
formalize the removal of Giant, making it easy to test in a more piecemeal
fashion. These macros will allow us to test fine-grained locks to a degree
before removing Giant, and also after, and to remove Giant in a piecemeal
fashion via sysctl's on those subsystems which the authors believe can
operate without Giant.

79f0cebcfb Really do proper keepme files in the compile directories. Use
.cvsignore file for [A-Za-z]* to keep these directories around rather
than waste a file on .keepme.  This should also make people's built
trees place nice with CVS.

fc8bd4c1c0 Updates and tweaks to the new trace functionality:
- Use db_printf() instead of printf().
- Clean up decode_syscall() to use regular if-then-else rather than goto's.
- Use the same method of parsing PID's for per-process traces as the x86
  code does: that is, if the address passed in is not a valid kernel
  address, treat it is a decimal pid.
- If the pid of the current process is specified, fall back to using the
  "default" parameters for the trace as curproc's pcb is not valid at this
  point.

7a5df26ad4 initialize mp_ncpus in cpu_mp_probe() so that if the probe fails and
cpu_mp_start() is never called, mp_ncpus will have a non-zero value.
This prevents systat from dying with an arithmatic exception caused
by a divide-by-zero error on UP alphas running a GENERIC kernel.

f10fa038c1 With this commit, I hereby pronounce gensetdefs past its use-by date.
Replace the a.out emulation of 'struct linker_set' with something
a little more flexible.  <sys/linker_set.h> now provides macros for
accessing elements and completely hides the implementation.

The linker_set.h macros have been on the back burner in various
forms since 1998 and has ideas and code from Mike Smith (SET_FOREACH()),
John Polstra (ELF clue) and myself (cleaned up API and the conversion
of the rest of the kernel to use it).

The macros declare a strongly typed set.  They return elements with the
type that you declare the set with, rather than a generic void *.

For ELF, we use the magic ld symbols (__start_<setname> and
__stop_<setname>).  Thanks to Richard Henderson <rth@redhat.com> for the
trick about how to force ld to provide them for kld's.

For a.out, we use the old linker_set struct.

NOTE: the item lists are no longer null terminated.  This is why
the code impact is high in certain areas.

The runtime linker has a new method to find the linker set
boundaries depending on which backend format is in use.

linker sets are still module/kld unfriendly and should never be used
for anything that may be modular one day.

97157b4d0e Resurrect platform.pci_intr_map() and essentially undo the effects of
the interface conversion to platform.pci_intr_route().  I've left the
platform.pci_intr_route() function pointer in place, as well as
alpha_pci_route_interrupt(), but no platform currently implements it.

To work around the removal of alpha_platform_assign_pciintr(cfg);
from the pci probe code, I've hooked in calls to platform.pci_intr_map()
in pcib_read_config (similar to the x86 APIC_IO ifdef in pci_cfgregread)
for every chipset that has a platform which needs it.

While here, I've removed the interupt mapping/routing code from the
AS2x00 platform because its not required (it has never been present in
-stable).

Tested on: UP1000, Miata(GL), XP1000, AS2100, AS500

- move the sysctl code to kern_intr.c
- do not use INTRCNT_COUNT, but rather eintrcnt - intrcnt to determine

e575dde359 finally fix intr routing on alphas such as the as500 after months of
breakage:

- call PCIB_ROUTE_INTERRUPT() regardless of how valid the intline looks.
  Some alphas leave garbage in the intline and leave the intr mapping
  to OS platform support routines that map slots/buses to intlines
- Down in the alpha pci code, first try platform.pci_intr_route() and
  if it doesn't exist or returns garbage, just read the intline out of
  config space.

tested on AS500 (garbage in intline) and UP1000 (PC-like, intline is valid)

Note that a nice little hack like the APIC_IO section of pci_cfgregread()
is not workable.  This is because the calling interface for
alpha_pci_route_interrupt() requires us to figure out the bus/slot/etc
from a device_t.  At pci_read_device() time, we don't have a device_t
for the bus/slot/func in question.

f504530d9f o Merge contents of struct pcred into struct ucred. Specifically, add the
real uid, saved uid, real gid, and saved gid to ucred, as well as the
  pcred->pc_uidinfo, which was associated with the real uid, only rename
  it to cr_ruidinfo so as not to conflict with cr_uidinfo, which
  corresponds to the effective uid.
o Remove p_cred from struct proc; add p_ucred to struct proc, replacing
  original macro that pointed.
  p->p_ucred to p->p_cred->pc_ucred.
o Universally update code so that it makes use of ucred instead of pcred,
  p->p_ucred instead of p->p_pcred, cr_ruidinfo instead of p_uidinfo,
  cr_{r,sv}{u,g}id instead of p_*, etc.
o Remove pcred0 and its initialization from init_main.c; initialize
  cr_ruidinfo there.
o Restruction many credential modification chunks to always crdup while
  we figure out locking and optimizations; generally speaking, this
  means moving to a structure like this:
        newcred = crdup(oldcred);
        ...
        p->p_ucred = newcred;
        crfree(oldcred);
  It's not race-free, but better than nothing.  There are also races
  in sys_process.c, all inter-process authorization, fork, exec, and
  exit.
o Remove sigio->sio_ruid since sigio->sio_ucred now contains the ruid;
  remove comments indicating that the old arrangement was a problem.
o Restructure exec1() a little to use newcred/oldcred arrangement, and
  use improved uid management primitives.
o Clean up exit1() so as to do less work in credential cleanup due to
  pcred removal.
o Clean up fork1() so as to do less work in credential cleanup and
  allocation.
o Clean up ktrcanset() to take into account changes, and move to using
  suser_xxx() instead of performing a direct uid==0 comparision.
o Improve commenting in various kern_prot.c credential modification
  calls to better document current behavior.  In a couple of places,
  current behavior is a little questionable and we need to check
  POSIX.1 to make sure it's "right".  More commenting work still
  remains to be done.
o Update credential management calls, such as crfree(), to take into
  account new ruidinfo reference.
o Modify or add the following uid and gid helper routines:
      change_euid()
      change_egid()
      change_ruid()
      change_rgid()
      change_svuid()
      change_svgid()
  In each case, the call now acts on a credential not a process, and as
  such no longer requires more complicated process locking/etc.  They
  now assume the caller will do any necessary allocation of an
  exclusive credential reference.  Each is commented to document its
  reference requirements.
o CANSIGIO() is simplified to require only credentials, not processes
  and pcreds.
o Remove lots of (p_pcred==NULL) checks.
o Add an XXX to authorization code in nfs_lock.c, since it's
  questionable, and needs to be considered carefully.
o Simplify posix4 authorization code to require only credentials, not
  processes and pcreds.  Note that this authorization, as well as
  CANSIGIO(), needs to be updated to use the p_cansignal() and
  p_cansched() centralized authorization routines, as they currently
  do not take into account some desirable restrictions that are handled
  by the centralized routines, as well as being inconsistent with other
  similar authorization instances.
o Update libkvm to take these changes into account.

2434332d91 Make sure that all resource allocation is handled in the pcib device, not
the chipset. This is already how the multi-hose systems handle resource
allocation and it fixes a bug where dense and bwx memory allocations were
not handled properly.

c87fc95350 Lock the procfs functions for doing a single step and reading/writing
registers better.  Hold sched_lock not only for checking the flag but
also while performing the actual operation to ensure the process doesn't
get swapped out by another CPU while we the operation is being performed.
