919de96974 Fixed conflicts between <machine/console.h> and <machine/pcvt_ioctl.h>,
so that it is possible for kdump and truss to include them both.  Use
#defines from pcvt_ioctl.h in conflicting cases, since pcvt_ioctl.h
gives a hint about the bogus third arg to _IO().  Ifdef the common
typedefs.  Export `struct key_t' from pcvt_ioctl.h so pcvt_ioctl.h is
bug for bug compatible with console.h (now both are broken in C++
mode).

e736cd05cb This fix should help the panic problems in -current. There
were some errors in "interval" management.  Due to the
clustering mechanism, the code is necessarily complex and
error prone.

   code to correctly recognized his bt848 card.
3. Roger Hardiman <roger@cs.strath.ac.uk> submitted various fixes to smooth
   out the microcode and made all modes consistent.

4. Added supported for yuv12 so we know can capture raw streams and feed it
   to mpeg_encoder . The upshot is that we can now mpeg encode more and save
   nearly 100 percent of the disk requirements previously for programs such
   as fxtv first save the raw video image to disk then converted to a
   format suitable for mpeg_encode.

b8c7cb30ab Don't set even parity uselessly in the line control register (parity
is not enabled).

Saved a few bytes by reusing the line control register bits and not
loading the speed in the wrong (cpu) register.

bc3de552ad Moved declaration of `union nethostadr' outside of the KERNEL section,
to give pollution compatible with <nfs/nqfs.h>.  At least mount_nfs.c
previously had to #define KERNEL before including <nfs/nfs.h> to get
this pollution, but this gave other pollution.

Moved comment about NFSINT_SIGMASK to immediately before the code that
it applies to.

850047383e Forward declare more structs that are used in prototypes here - don't
depend on <sys/types.h> forward declaring common ones.

Added an underscore to `sin' in prototypes to avoid warnings for the
conflict with the ANSI sin().

ffbedc02de Removed ifdef for <sys/sockio.h> vs <sys/ioctl.h>. <sys/sockio.h> is
a BSD4.4Lite1 feature, not a FreeBSD feature.  <sys/ioctl.h> is a
compatibility misfeature.

Moved NPCI ifdef.  This file didn't compile if NPCI <= 0.  It shouldn't
be configured in that case, but it is easy to support (mis)configuration
of drivers without buses by generating null objects, and many drivers
do it.

Removed unused includes.

b163ce2f65 Fixed the search for free wd drives. The search was terminated
prematurely when there was a hole (for a cdrom or an unused
interface) in the sequence of wd drives.  This caused non-free
wd units to be probed as atapi drives.  There was no problem
provided the atapi probe failed correctly.

98f3fd4b49 Don't put "-I." in ${CFLAGS} here. bsd.kmod.mk now puts an absolute
path to the obj directory in ${CFLAGS}.  This is actually equivalent
to "-I." since bsd.kmod.mk also puts -I- in ${CFLAGS}.

021886ff87 Restrict the include search path using -nostdinc -I- in $(CFLAGS} as
in kernel Makefiles.  Nothing in /usr/include is used (provided
relative paths for sys/* and <machine> can be found), so there is
no need for the -I/usr/include kludge as in kernel Makefiles.

3398edcee9 Revert previous commit. Remove all ifp->if_* = 0 initializations,
since pkh made tunctl static in revision 1.17 these are already
guaranteed to be zero'd and tunattach will only be called once.

158f45f157 Add a new ioctl, SCSIOCGETDEVINFO, which takes a device ID and uses it
to look up information about that device.  Right now, all it does
is give back the dev_t for the device, if known, since that's all
I needed, but hopefully the SCSI mavens will come up with a more generally
useful structure.

c15541e7a7 contigalloc doesn't place the allocated page(s) into an object, and
now this breaks vm_page_wire (due to wired page accounting per object.)

This should fix a problem as described by Donald Maddox.

eaf13dd73a Change the busy page mgmt, so that when pages are freed, they
MUST be PG_BUSY.  It is bogus to free a page that isn't busy,
because it is in a state of being "unavailable" when being
freed.  The additional advantage is that the page_remove code
has a better cross-check that the page should be busy and
unavailable for other use.  There were some minor problems
with the collapse code, and this plugs those subtile "holes."

Also, the vfs_bio code wasn't checking correctly for PG_BUSY
pages.  I am going to develop a more consistant scheme for
grabbing pages, busy or otherwise.  For now, we are stuck
with the current morass.

c5b193bfba Retire LFS.
If you want to play with it, you can find the final version of the
code in the repository the tag LFS_RETIREMENT.

If somebody makes LFS work again, adding it back is certainly
desireable, but as it is now nobody seems to care much about it,
and it has suffered considerable bitrot since its somewhat haphazard
integration.

0621c31aaf Make the bounce buffer code a little more robust when space isn't
available.  If there isn't bounce space available, the bounce code
is disabled.  This will allow most large systems to run properly
when the bounce space is mistakenly allocated above 16MB.

c67d7e8aa2 Fix operation with the Iomega Zip 100 ATAPI.
All known versions of this drive (firmware 21.* and 23.*) will lock up
if presented with a read/write request of > 64 blocks.  In the presence
of such a unit, I/O requests of > 64 blocks are fragmented to avoid
this.

facf6a3fbe Check the status port after waiting for DRQ; some drives seem to be very
slow coming off the bus (eg. Iomega's ATAPI Zip).  Failure to do
this results in a false probe of an ATAPI device with garbage
data.

c3229e05a3 Improved connection establishment performance by doing local port lookups via
a hashed port list. In the new scheme, in_pcblookup() goes away and is
replaced by a new routine, in_pcblookup_local() for doing the local port
check. Note that this implementation is space inefficient in that the PCB
struct is now too large to fit into 128 bytes. I might deal with this in the
future by using the new zone allocator, but I wanted these changes to be
extensively tested in their current form first.

Also:
1) Fixed off-by-one errors in the port lookup loops in in_pcbbind().
2) Got rid of some unneeded rehashing. Adding a new routine, in_pcbinshash()
   to do the initialial hash insertion.
3) Renamed in_pcblookuphash() to in_pcblookup_hash() for easier readability.
4) Added a new routine, in_pcbremlists() to remove the PCB from the various
   hash lists.
5) Added/deleted comments where appropriate.
6) Removed unnecessary splnet() locking. In general, the PCB functions should
   be called at splnet()...there are unfortunately a few exceptions, however.
7) Reorganized a few structs for better cache line behavior.
8) Killed my TCP_ACK_HACK kludge. It may come back in a different form in
   the future, however.

These changes have been tested on wcarchive for more than a month. In tests
done here, connection establishment overhead is reduced by more than 50
times, thus getting rid of one of the major networking scalability problems.

Still to do: make tcp_fastimo/tcp_slowtimo scale well for systems with a
large number of connections. tcp_fastimo is easy; tcp_slowtimo is difficult.

WARNING: Anything that knows about inpcb and tcpcb structs will have to be
         recompiled; at the very least, this includes netstat(1).

4a2d2630c8 Generate symlinks to the "sys" and <machine> directories and put
them in the include path.  This fixes recent breakage of the syscons
LKMs and general brokenness of the include paths (headers under
/usr/include were used in many cases).

88a92ff10e Even though BIOS writer's guide recommends cpuid instruction of Cyrix
6x86MX CPU is enabled (BIOS should not disable it), some BIOS disables
it via CCR4.  In this case, cpu variable becomes CPU_486 and
identblue() is called.  Because Cyrix 6x86MX has MSR and doesn't have
MSR1002, wrmsr instruction generates general protection fault.

		vnode is already being rundown.
	Reassignbuf wasn't being called when needed under
		certain circumstances.

7b778b5e61 Make all file-system (MFS, FFS, NFS, LFS, DEVFS) related option new-style.
This introduce an xxxFS_BOOT for each of the rootable filesystems.
(Presently not required, but encouraged to allow a smooth move of option *FS
to opt_dontuse.h later.)

LFS is temporarily disabled, and will be re-enabled tomorrow.

2d8acc0f4a VM level code cleanups.
1)	Start using TSM.
	Struct procs continue to point to upages structure, after being freed.
	Struct vmspace continues to point to pte object and kva space for kstack.
	u_map is now superfluous.
2)	vm_map's don't need to be reference counted.  They always exist either
	in the kernel or in a vmspace.  The vmspaces are managed by reference
	counts.
3)	Remove the "wired" vm_map nonsense.
4)	No need to keep a cache of kernel stack kva's.
5)	Get rid of strange looking ++var, and change to var++.
6)	Change more data structures to use our "zone" allocator.  Added
	struct proc, struct vmspace and struct vnode.  This saves a significant
	amount of kva space and physical memory.  Additionally, this enables
	TSM for the zone managed memory.
7)	Keep ioopt disabled for now.
8)	Remove the now bogus "single use" map concept.
9)	Use generation counts or id's for data structures residing in TSM, where
	it allows us to avoid unneeded restart overhead during traversals, where
	blocking might occur.
10)	Account better for memory deficits, so the pageout daemon will be able
	to make enough memory available (experimental.)
11)	Fix some vnode locking problems. (From Tor, I think.)
12)	Add a check in ufs_lookup, to avoid lots of unneeded calls to bcmp.
	(experimental.)
13)	Significantly shrink, cleanup, and make slightly faster the vm_fault.c
	code.  Use generation counts, get rid of unneded collpase operations,
	and clean up the cluster code.
14)	Make vm_zone more suitable for TSM.

This commit is partially as a result of discussions and contributions from
other people, including DG, Tor Egge, PHK, and probably others that I
have forgotten to attribute (so let me know, if I forgot.)

This is not the infamous, final cleanup of the vnode stuff, but a necessary
step.  Vnode mgmt should be correct, but things might still change, and
there is still some missing stuff (like ioopt, and physical backing of
non-merged cache files, debugging of layering concepts.)

764d8cef56 A more complete fix for the "land" attack, removing the "quick fix" from
rev 1.66.  This fix contains both belt and suspenders.

Belt: ignore packets where src == dst and srcport == dstport in TCPS_LISTEN.
 These packets can only legitimately occur when connecting a socket to itself,
 which doesn't go through TCPS_LISTEN (it goes CLOSED->SYN_SENT->SYN_RCVD->
 ESTABLISHED).  This prevents the "standard" "land" attack, although doesn't
 prevent the multi-homed variation.

Suspenders: send a RST in response to a SYN/ACK in SYN_RECEIVED state.
 The only packets we should get in SYN_RECEIVED are
 1. A retransmitted SYN, or
 2. An ack of our SYN/ACK.
 The "land" attack depends on us accepting our own SYN/ACK as an ACK;
 in SYN_RECEIVED state; this should prevent all "land" attacks.

We also move up the sequence number check for the ACK in SYN_RECEIVED.
 This neither helps nor hurts with respect to the "land" attack, but
 puts more of the validation checking in one spot.

a9b9e46550 - Support for multiple PD6832 controllers. Each found 6832 is assigned
and initializes the next two ports in order starting at 03e0.  This
  also patches pcic_p.h to reduce the I/O ports mapped from 4 to 2.

3ae6029808 - Move SETKLOC and CLEARKLOCK macros into uion.h.
- Set UN_ULOCK in union_lock() when UN_KLOCK is set.  Caller expects
  that vnode is locked correctly, and may call another function which
  expects locked vnode and may unlock the vnode.
- Do not assume the behavior of inside functions in FreeBSD's
  vfs_suber.c is same as 4.4BSD-Lite2.  Vnode may be locked in
  vget() even though flag is zero.  (Locked vnode is, of course,
  unlocked before returning from vget.)

Move sigjmp_buf and jmp_buf structure definitions to machine/setjmp.h
so that i386 can continue to use int as the basic register type and
alpha can use long. Bruce was concerned about possible differing
alignment. I've left the definition of _JBLEN in machine/setjmp.h
even though Bruce's example used the number directly. I don't know if
any other code relies on _JBLEN, so I left it to avoid potential
breakage.

ffbb164e19 Set p_retval for the correct process in getpriority(). This fixes
a null pointer panic when the pointer for the incorrect process is
NULL.  getpriority() was broken in rev.1.27.  Rev.1.28 broke the
warning instead of fixing the problem.

95f67de2e3 The removal of a page from the free queue in vm_page_zero_idle was

4722175765 Tie up some loose ends in vnode/object management. Remove an unneeded
config option in pmap.  Fix a problem with faulting in pages.  Clean-up
some loose ends in swap pager memory management.

The system should be much more stable, but all subtile bugs aren't fixed yet.

aaf862068b Bring in IDE ATAPI floppy support.
This is Junichi's v1.0 driver.

NOTE: Major device numbers have been changed to avoid conflict with other
      FreeBSD 3.0 devices.  The new numbers should be considered "official."
      This driver is still considered "beta" quality, although we have been
      playing with it.  Please submit bugs to junichi and myself.
