fd41389dca Introduce two new helper macros to define the priority ranges used for
interactive timeshare threads (PRI_*_INTERACTIVE) and non-interactive
timeshare threads (PRI_*_BATCH) and use these instead of PRI_*_REALTIME
and PRI_*_TIMESHARE.  No functional change.

0d331f0eea Cleanup physical address and PTE types on MIPS.
1. Use vm_paddr_t for physical addresses.

There are a few places in the MIPS platform code where vm_offset_t is
used for physical addresses, change these to use vm_paddr_t:
- phys_avail[], physmem_desc[] arrays
- pmap_mapdev(), page_is_managed(), is_cacheable_mem() pmap_map() args
- local variables of various pmap functions

2. Change init_pte_prot() return from int to pt_entry_t, as this can be
64 bit when using 64 bit TLB entries.

3. Update printing of pt_entry_t and of vm_paddr_t to use 'j' format with
uintmax_t. This will be useful later if we plan to use 64bit phsical addr
on 32 bit n32 compilation.

30a663c808 Fix a brain fart. Since this file is shared between i386 and amd64, a
bus_size_t may be 32 or 64 bits.  Change the bounce_zone alignment field
to explicitly be 32 bits, as I can't really imagine a DMA device that
needs anything close to 2GB alignment of data.

31dade5c8b Fix several bugs in the ARP code related to improperly formatted
packets.

*) Reject requests with a protocol length not equal to 4.  This is IPv4
and there is no reason to accept anything else.

*) Reject packets that have a multicast source hardware address.

*) Drop requests where the hardware address length is not equal
to the hardware address length of the interface.

9d3b6a6cd6 Add type checking for static and dynamic sysctls using scalar types.
The code is turned off until the tree is fixed up so it compiles.
__FreeBSD_version was already bumped once today, so skip the bump, but
add an entry to UPDATING.

Note that __DESCR() is used in the SYSCTL_OID() macro and so is not
needed in macros that invoke it.  This use was inconsistent in the
file and I have made it consistent any lines already being changed.

3d4e888988 Update dialog to version 20100428. This changes the license under which
dialog is distributed from GPLv2 to LGPLv2 and introduces a number of new
features and a new and better libdialog API. The existing libdialog will
be kept temporarily as libodialog for compatibility purposes until sade,
sysinstall and tzsetup have been either updated or replaced.

__FreeBSD_version is now 900030.

c81547364f The message ring interrupt needs to be enabled for all cpus, not just
the ones which run the message ring handler.

Some bits of the interrupt mask are part of the status register which is
saved with the process context, and these bits are initialized from the
cpu on which the process is created. This means that all the processes
should have the same value for these interrupt mask bits, so that the
interrupt mask remains the same regardless of what thread is scheduled
on the cpu.

6881e65a56 For re(4) controllers that uses new jumbo frame scheme(RTL8168C/D/E),
limit maximum RX buffer size to RE_RX_DESC_BUFLEN instead of
blindly configuring it to 16KB. Due to lack of documentation, re(4)
didn't allow jumbo frame on these controllers. However it seems
controller is confused with jumbo frame such that it can DMA the
received frame to wrong address instead of splitting it into
multiple RX buffers. Of course, this caused panic.

Since re(4) does not support jumbo frames on these controllers,
make controller drop frame that is longer than RE_RX_DESC_BUFLEN
sized frame. Fortunately RTL810x controllers, which do not support
jumbo frame, have no such issues but this change also limited
maximum RX buffer size allowed to RTL810x controllers. Allowing
16KB RX buffer for controllers that have no such capability is
meaningless.

b8525325b4 - Retire some unused ithread priorities: PI_TTYHIGH, PI_TAPE, and
PI_DISKLOW.  While here, rename PI_TTYLOW to PI_TTY.
- Add a macro PI_SWI() that takes a SWI_* constant as an argument and
  returns the suitable thread priority.

a13f07741c Add new functions, fcu_fan_set_pwm and fcu_fan_get_pwm, to set and get
the pwm values. We can now set the fan's speed of a PWM controlled fan
with % numbers between 30 and 100 % instead of trying to model a
% number based on rpm.
The fcu chip offers both, the dutycycle and the rpm value of the PWM
controlled fans. I added the rpm value to the list of information
available via sysctl(8).

1d1ef54a74 When driver is not running, do not send DUMP command to controller
and just show old (cached) values. Controller will not respond to
the command unless MAC is enabled so DUMP request for down
interface caused request timeout.

eddf4ff009 Implement TSO on RealTek RTL8168/8111 C or later controllers.
RealTek changed TX descriptor format for later controllers so these
controllers require MSS configuration in different location of TX
descriptor. TSO is enabled by default for controllers that use new
descriptor format.
For old controllers, TSO is still disabled by default due to broken
frames under certain conditions but users can enable it.

769825141b Add IDs for HighPoint RocketRAID 64x controllers.
These controllers consist of two Marvell 88SE9128 6Gbps SATA chips and
PLX PCIe bridge. As result, they seem to be agree to work with ahci(4)
as usual HBAs. The only noticed issue is that RAID BIOS disables all
drive caches during boot, though `camcontrol cmd ...` is able to fix that.

Those who wants RAID functionality can still use closed proprietary driver
from HighPoint site.

03248b7340 Fix a bug in the client side krpc where it was, sometimes
erroneously, assumed that 4 bytes of data were in the first
mbuf of a list by replacing the bcopy() with m_copydata().
Also, replace the uses of m_pullup(), which can fail for
reasons other than not enough data, with m_copydata().
For the cases where it isn't known that there is enough
data in the mbuf list, check first via m_len and m_length().
This is believed to fix a problem reported by dpd at dpdtech.com
and george+freebsd at m5p.com.

61d3626b69 Allow AcpiOsInstallInterruptHandler() and AcpiOsRemoveInterruptHandler() to
install or remove non-SCI interrupt handlers per ACPI Component Architecture
User Guide and Programmer Reference.  ACPICA may install such interrupt
handler when a GPE block device is found, for example.  Add a wrapper for
ACPI_OSD_HANDLER, convert its return values to ours, and make it a filter.
Prefer KASSERT(9) over panic(9) as we have never seen those in reality.
Clean up some style(9) nits and add my copyright.

b398f2bdb6 Apply DMA address space restriction to controllers that have 4GB
DMA boundary bug and runs with PCI-X mode.  watchdog timeout was
observed on BCM5704 which lives behind certain PCI-X bridge(e.g.
AMD 8131 PCI-X bridge).  It's still not clear whether the root
cause came from that PCI-X bridge or not. The watchdog timeout
indicates the issue is in TX path. If the bridge reorders TX
mailbox write accesses it would generate all kinds of problems but
I'm not sure.  This should be revisited.

dafab26354 Backout r216973 and r216970. r216973 didn't solve watchdog timeout
issue seen on PCIX BCM5704 controller. r216970 fixed the issue but
the DMA address space restriction was applied to all bge(4)
controllers such that it caused unnecessary performance degradation
for controllers that have no such issues.

f9f16511ff o) Free mbufs in error cases.
o) Clear/acknowledge receive interrupt at end of thread.  This gives the
   management interfaces performance on the order of 100Mbps rather than
   the previous level of 10Mbps on my MR-730.

d46c373873 o) Expand the CIU driver to be aware of newly-allocated parts of the IRQ range.
o) Add 'octm', a trivial driver for the 10/100 management ports found on some
   Octeon systems.
o) Make the Simple Executive's management port helper routines compile on
   FreeBSD (namely by not doing math on void pointers.)
o) Add a cvmx_mgmt_port_sendm routine to the Simple Executive to send an mbuf
   so there is only one copy in the transmit path, rather than having to first
   copy the mbuf to an intermediate buffer and then copy that to the Simple
   Executive's transmit ring.
o) Properly work out MII addresses of management ports on the Lanner MR-730.
   XXX The MR-730 also needs some patches to the MII read/write routines, but
       this is sufficient for now.  Media detection will be fixed in the future
       when I can spend more time reading the vendor-supplied patches.

ef1dcae0cf Now that we correctly enable rx interrupts on all cores, performance has gotten
quite awful, because e.g. 4 packets will come in and get processed on 4
different cores at the same time, really battling with the TCP stack quite
painfully.  For now, just run one task at a time.

This gets performance up in most cases to where it was before the correctness
fixes that got interrupts to run on all cores (except in high-load TCP transmit
cases where all we're handling receive for is ACKs) and in some cases it's
better now.  What would be ideal would be to use a more advanced interrupt
mitigation strategy and possibly to use different workqueue groups per port for
multi-port systems, and so on, but this is a fine stopgap.

9a7c4f93a9 Modify readdirplus in the experimental NFS server in a
manner analogous to r216633 for the regular server. This
change busies the file system so that VFS_VGET() is
guaranteed to be using the correct mount point even
during a forced dismount attempt. Since nfsd_fhtovp() is
not called immediately before readdirplus, the patch is
actually a clone of pjd@'s nfs_serv.c.4.patch instead of
the one committed in r216633.

2801f33bda Change some variables from int to size_t. This is more accurate since
these variables represent sizes in one capacity or another.  There is
no reason to allow negative numbers.  Change userspace shared structure
elements that get used for the modified functions from int to uint32_t,
since it's not clear what userspace programs use these fields, and we
do not want to break binary compatibility.  This fixes a panic when
corrupt or bogus data is passed into the kernel.

a3f4c0274d Eliminate the counting of vm_page_pa_tryrelock calls. We really don't
need it anymore.  Moreover, its implementation had a type mismatch, a
long is not necessarily an uint64_t.  (This mismatch was hidden by
casting.)  Move the remaining two counters up a level in the sysctl
hierarchy.  There is no reason for them to be under the vm.pmap node.

6fbd535b5a Make sure to always do source address selection on
an unbound socket, regardless of any multicast options.
If an address is specified via a multicast option, then
let it override normal the source address selection.

This fixes a bug where source address selection was
not being performed when multicast options were present
but without an interface being specified.

7a8b4abb97 Revert r216805.
That revision is introducing a bug which is more visible than problems

4bdd0bc803 Rename PRIreg helper macro to PRIptr to better reflect its use. Registers
and pointers don't always have the same size, e.g. the __mips_n32 ABI
(ILP32) has 64 bit registers but 32 bit pointers.

On mips introduce PRIptr to fix the format specifier for (u)intptr_t.

Prefix PRI64 and PRIptr with underscores because macro names starting with
PRI[a-zX] are reserved for future use.

5cec74bed9 In rtld, read the initial stack access mode from AT_STACKPROT as set
by kernel, and parse PT_GNU_STACK phdr from linked and loaded dsos.

If the loaded dso requires executable stack, as specified by PF_X bit
of p_flags of PT_GNU_STACK phdr, but current stack protection does not
permit execution, the __pthread_map_stacks_exec symbol is looked up
and called. It should be implemented in libc or threading library and
change the protection mode of all thread stacks to be executable.

Provide a private interface _rtld_get_stack_prot() to export the stack
access mode as calculated by rtld.

90de4ffc1a In elf image activator, read and apply the stack protection mode from
PT_GNU_STACK program header, if present and enabled. Two new sysctls
are provided, kern.elf32.nxstack and kern.elf64.nxstack, that allow to
enable PT_GNU_STACK for ABIs of specified bitsize, if ABI decided to
support shared page.

Inform rtld about access mode of the stack initial mapping by
AT_STACKPROT aux vector.

At the moment, the default is disabled, waiting for the usermode
support bits.

06eb8de1e1 Create shared (readonly) page. Each ABI may specify the use of page by
setting SV_SHP flag and providing pointer to the vm object and mapping
address. Provide simple allocator to carve space in the page, tailored
to put the code with alignment restrictions.

Enable shared page use for amd64, both native and 32bit FreeBSD
binaries.  Page is private mapped at the top of the user address
space, moving a start of the stack one page down. Move signal
trampoline code from the top of the stack to the shared page.

89281909e1 On mixed 32/64 bit architectures (mips, powerpc) use __LP64__ rather than
architecture macros (__mips_n64, __powerpc64__) when 64 bit types (and
corresponding macros) are different from 32 bit. [1]

Correct the type of INT64_MIN, INT64_MAX and UINT64_MAX.

Define (U)INTMAX_C as an alias for (U)INT64_C matching the type definition
for (u)intmax_t. Do this on all architectures for consistency.

61d89c0b21 On 32 bit architectures define (u)int64_t as (unsigned) long long instead
of (unsigned) int __attribute__((__mode__(__DI__))). This aligns better
with macros such as (U)INT64_C, (U)INT64_MAX, etc. which assume (u)int64_t
has type (unsigned) long long.

The mode attribute was used because long long wasn't standardised until
C99. Nowadays compilers should support long long and use of the mode
attribute is discouraged according to GCC Internals documentation.

The type definition has to be marked with __extension__ to support
compilation with "-std=c89 -pedantic".

af03e997ba Fix types of some values in machine/_limits.h.
On some architectures UCHAR_MAX and USHRT_MAX had type unsigned int.
However, lacking integer suffixes for types smaller than int, their type
should correspond to that of an object of type unsigned char (or short)
when used in an expression with objects of type int. In that case unsigned
char (short) are promoted to int (i.e. signed) so the type of UCHAR_MAX and
USHRT_MAX should also be int.

Where MIN/MAX constants implicitly have the correct type the suffix has
been removed.

While here, correct some comments.

69d18cbe4f - Move sched_fork() later in fork() after the various sections of the new
thread and proc have been copied and zeroed from the old thread and
  proc.  Otherwise attempts to modify thread or process data in sched_fork()
  could be undone.
- Don't copy td_{base,}_user_pri from the old thread to the new thread in
  sched_fork_thread() in ULE.  This is already done courtesy the bcopy()
  of the thread copy region.
- Always initialize the real priority (td_priority) of new threads to the
  new thread's base priority (td_base_pri) to avoid bogusly inheriting a
  borrowed priority from the parent thread.

c2aa4fc0eb Import support for the Sony Playstation 3 using the OtherOS feature
available on firmwares 3.15 and earlier.

Caveats: Support for the internal SATA controller is currently missing,
as is support for framebuffer resolutions other than 720x480. These
deficiencies will be remedied soon.

aea1c06730 Modify the experimental NFS server so that it calls
vn_start_write() with a non-NULL vp. That way it will
find the correct mount point mp and use that mp for the
subsequent vn_finished_write() call. Also, it should fail
without crashing if the mount point is being forced dismounted
because vn_start_write() will set the mp NULL via VOP_GETWRITEMOUNT().

af6310a666 Fix page fault that occurred when trying to initialize preloaded kernel module,
the dependency of which was preloaded, but failed to initialize.  Previously,
kernel dereferenced NULL pointer returned by modlist_lookup2(); now, when this
happens, we unload the dependent module.  Since the depended_files list is
sorted in dependency order, this properly propagates, unloading modules that
depend on failed ones.

From the user point of view, this prevents the kernel from panicing when
trying to boot kernel compiled without KDTRACE_HOOKS with dtraceall_load="YES"
in /boot/loader.conf.

766cc93241 Correct an 8-year-old typo which reliably leads to typo after typo today:
The macros here for generating coprocessor 0 accessors are named like:

	MIPS_RDRW32_COP0

That macro would produce mips_rd_<register>() and mips_wr_<register>()
inlines to access the specified register by name from C.  The problem is that
the R and the W were swapped in the macros originally; it was meant to be named
RDWR because it generated mips_rd_* and mips_wr_* functions, but was instead
spelled RDRW, which nobody should be expected to get right by anything other
than copy and paste.

It's too many consonants in a row to keep straight anyway, so just prefer e.g.:

	MIPS_RW32_COP0

While here, add a missing #undef.

72ce649bd8 Partially revert change made in r212061. r212061 relied on
bus_dma(9)'s capability which honors boundary restrictions of DMA
tag for dynamic buffers. However it seems this does not work well
and it triggered watchodg timeouts on controller that has the
hardware bug. It's not clear whether there is still another
hardware bug not mentioned in errata. This should be revisited
since this change shall make use of bounce buffers which in turn
reduces performance a lot on systems that have more than 4GB
memory.

7d0e430fef Remove OCTEON1-32. Since moving to the Cavium SDK, we've lost our support for
running an o32 kernel safely, and would have to add interrupt disabling and
reenabling to a bunch of macros in the Simple Executive sources to support it.

The only reason one would run an o32 kernel on Octeon would be to run o32 world,
which is better worked towards by adding o32 binary compatibility to n64 kernels
along with, eventually, supporting multilib systems so o32 binaries can run
alongside n32 and n64 ones.

1b29205c00 Add hamfisted locking to the Xen/PV pmap code: Only allow one thread to
be in {pmap_pinit, pmap_copy, pmap_release} at a time.

This reduces the rate of panics when running 'make index' from ~0.6/hour
to ~0.02/hour (p < 10^-30).

At a later date this locking will be removed, and for this reason, it is
wrapped in #ifdef HAMFISTED_LOCKING; this temporary hack is being put in
place with the intention of shipping somewhat-stable Xen bits in FreeBSD
8.2-RELEASE.

005e768447 Adjust the critical section protecting _xen_flush_queue to cover the
entire range where the page mapping request queue needs to be atomically
examined and modified.

Oddly, while this doesn't seem to affect the overall rate of panics
(running 'make index' on EC2 t1.micro instances, there are 0.6 +/- 0.1
panics per hour, both before and after this change), it eliminates
vm_fault from panic backtraces, leaving only backtraces going through
vmspace_fork.

30073aa256 Extend the section in which interrupts are disabled in the TLB demap
functions, otherwise if we get preempted after checking whether a certain
pmap is active on the current CPU but before disabling interrupts we might
operate on an outdated state as the pmap might have been deactivated in
the meantime. As the same issue may arises when the TLB demap function is
interrupted by a TLB demap IPI, just entering a critical section before
the check isn't sufficient so we have to fully disable interrupts instead.

688b83a0a6 There can be more than 0x20000000 swap meta blocks allocated if a swap-backed
md(4) device is used. Don't panic when deallocating such a device if swap

8229c0275d o) When trying to determine whether the pcpu pointer is a managed address, check
not just that it is greater than the minimal kernel virtual address, but also
   that it is less than the maximal kernel virtual address.  On n64 kernels, the
   pcpup comes out of a direct-mapped address that, with an unsigned compare, is
   rather greater than the minimal kernel virtual address.
o) Turn the panic if interrupts are disabled in cpu_idle into a KASSERT since on
   other architectures it's behind INVARIANTS anyway.
o) Add a check that not all interrupts are masked, too.
o) Add cpu_idleclock() and cpu_activeclock() use to cpu_idle as is done on other
   architectures.

Add and export constants of array sizes of jail parameters as compiled into
the kernel.
This is the least intrusive way to allow kvm to read the (sparse) arrays

475bb93831 Fix endianness bug introduced in r205091.
After controller updates control word in a RX LE, driver converts
it to host byte order. The checksum value in the control word is
stored in big endian form by controller. r205091 didn't account for
the host byte order conversion such that the checksum value was
incorrectly interpreted on big endian architectures which in turn
made all TCP/UDP frames dropped. Make RX checksum offload work
on any architectures by swapping the checksum value.

969f8f9046 Try to catch a possible divide-by-zero as early as possible if "mtu" is 0
(also test for negative MTUs if checking it anyway).
An MTU of 0 is arguably a bug elsewhere, but this at least gives us some
more debugging hints.

558c7e035b Move the increment operation under the lock and split the condition
variable into two so that we can see on which one we are waiting.
This might also more properly propagate the update of the
flowclean_cycles flag and avoid "hangs" people were seeing.

0440193beb Make i386_set_ldt work on i386/XEN, step 5/5.
When cleaning up a thread, reset its LDT to the default LDT.

Note: Casting the LDT pointer to an int and storing it in pc_currentldt is
wildly bogus, but is harmless since pc_currentldt is a write-only variable.

0f106c39c9 Make i386_set_ldt work on i386/XEN, step 4/5.
Use xen_update_descriptor to update the LDT rather than bcopy.  Under Xen,
pages used for holding LDTs must be read-only, so we can't make the change
ourselves.

Ths obvious alternative of "remap the page read-write, make the change, then
map it read-only again" doesn't work since Xen won't allow an LDT page to be
remapped as R/W.  An arguably better solution is used by NetBSD: They don't
modify LDTs in-place at all, but instead copy the entire LDT, modify the new
version, then atomically swap.

Synchronize reality with comment: The user_ldt_alloc function is supposed to
return with dt_lock held.  Due to broken locking in i386/xen/pmap.c, we drop
dt_lock during the call to pmap_map_readonly and then pick it up again; this
can be removed once the Xen pmap locking is fixed.

76aea5c53e Make i386_set_ldt work on i386/XEN, step 1/5.
Lock the vm page queue mutex around calls to pte_store.  As with many other
uses of the vm page queue mutex in i386/xen/pmap.c, this is bogus and needs
to be replaced at some future date by a spin lock dedicated to protecting
the queue of pending xen page mapping hypervisor calls.  (But for now, bogus
locking is better than a panic.)

8d62f29422 The mwl's HAL manages an array of MWL_MBSS_MAX VAPs where the first 8 are
supposed to be APs and the later 24 are pre-configured as STAs. A wrong
condition during initialization is responsible for not configuring the last
8 array members. This is results in being able to create more than 8,
possible uninitialized, AP-VAPs.

e90b6f07f7 Add driver for DM&P Vortex86 RDC R6040 Fast Ethernet.
The controller is commonly found on DM&P Vortex86 x86 SoC.  The
driver supports all hardware features except flow control.  The
flow control was intentionally disabled due to silicon bug.

DM&P Electronics, Inc. provided all necessary information including
sample board to write driver and answered many questions I had.

modification of memory which was already free'd and eventually in:

64276929de Handle missing jremrefs when a directory is renamed overtop of
another, deleting it.  If the directory is removed, UFS always need to
remove the .. ref, even if the ultimate ref on the parent would not
change. The new directory must have a new journal entry for that ref.
Otherwise journal processing would not properly account for the
parent's reference since it will belong to a removed directory entry.

Change ufs_rename()'s dotdot rename section to always
setup_dotdot_link(). In the tip != NULL case SUJ needs the newref dependency
allocated via setup_dotdot_link().

Stop setting isrmdir to 2 for newdirrem() in softdep_setup_remove().
Remove the isdirrem > 1 checks from newdirrem().

3e36dcd710 Fix the LINUX_SOUND_MIXER_INFO ioctl to return success after the
information is set to FreeBSD.  It had been falling through to the end
of linux_ioctl_sound() and returning ENOIOCTL.  Noticed when running the
Linux ALSA amixer tool.

Add a LINUX_SOUND_MIXER_READ_CAPS ioctl which is used by the Skype
v2.1.0.81 binary.
