89eb2dfeeb Make this manual page reflect the sources better. 
- A #include of <sys/mutex.h> is no longer needed to use sx(9) (since
  2001/05/01).
- Use of the SX_SYSINIT() macro requires inclusion of '<sys/kernel.h>'
