SPDK poller function will return the different value for
the different status. Update the QoS poller for this change.

Especially for rxe. We're not waiting for the queue pairs to get
cleaned up after disconnect, so this test fails due to running
out of qpairs sometimes.

We were previously doing lots of checks in debug mode
to verify the validity of this field. Now we understand
how it works, so these checks are never going to hit
and are just making the code harder to read.

When we thought we could do error recovery we differentiated between
inactive and erro states. However, that's not possible so collapse
them back into one.

The specification states that opcode is not valid when the status
is not success. Instead, keep track of the operation type ourselves.

After some lengthy discussions with the RDMA experts, the only
way forward on an RDMA qp error is to disconnect it. The initiator
can create a new qp if it wants to later on.
Remove all of the error recovery code and disconnect the qp
any time an error is encountered.

Currently only the iscsi => scsi dependency is defined to
demonstrate how the framework behaves.
Eventually, lib/Makefile will be populated with more lines
of the form:
DEPDIRS-iscsi : scsi log conf event
This line will ensure that the iscsi subdirectory is built
after the specified dependency directories.
Then the lib/iscsi/Makefile also defines:
DEP_LIBNAMES = scsi log conf event
This line adds dependencies from the iscsi.so to the specified
libraries.
Ideally we could avoid this duplication by passing the DEPDIRS
value to the lower level Makefile, but that breaks down when we
start considering event/subsystems/* libraries. So for now we'll
need to duplicate it. Incidentally, DPDK also duplicates the
dependency information like this.
While here, remove use of $(MAKESUBDIRFLAGS) in spdk.subdirs.mk -
it was no longer used anywhere in the tree.

The condition that both raid_bdev and raid_bdev->base_bdev_info[i].bdev
are not NULL is already verified by matching it with base_bdev.
This is a preparation to the further refactoring in the next patch.

The crypto drivers are now always initialized at
bdev module init, so any subsequent attempts to
init them would immediately return with rc = 0.
Besides, from the code perspective, this call does
not have the proper error handling. To clean up
all of this, we remove the call entirely.

In early development of crypto we built the lirbary using
dpdkbuild/Makefile but later due to a DPDK change we needed
to move to a newer version of the ipsec library that is now
built and installed in pkgdep.sh btu we never removed it
from the makefile.
This patch removes the extra building of the ipsec module.

There is no guarantee that the first io_channel created for
a LUN will be the last one destroyed. So we need to attach
the poller to the LUN, not the io_channel.
While here, and related to this patch, remove the assert()
that the last channel destroyed must be on the LUN's
master thread. This assertion simply isn't true.
