Daniel Verkamp a3658ab042 Change doc link to new spdk.io/doc location

Also make sure all links point to the canonical www.spdk.io, not
spdk.io, to avoid extra redirects.

Building the Documentation

To convert the documentation into HTML, install Doxygen and mscgen and run make in the doc directory. The output will be located in doc/output/html.
