d9ee777465 Tekram supplied driver for their DC390 and DC390T controllers.
These controllers are based on the AMD 53c974, and the driver
does only support those two cards, since it checks for a Tekram
specific configuration EEPROM.
This deficiency (TM) will be fixed soon ... :)

EISA slots to probe. This is mainly intended to allow installing the
system on an HP Netserver with an on-board AIC7xxx EISA SCSI
controller, that is sitting on EISA slot # 11.

Documentation updates explaining this hack will follow shortly.

Note that this can go away again as soon as the EISA device probing
is more intelligent about the address space clash with the PCI address
space.

2df62c4689 Implement closer-to POSIX mlock semantics. The major difference is
that we do allow mlock to span unallocated regions (of course, not
mlocking them.) We also allow mlocking of RO regions (which the old
code couldn't.) The restriction there is that once a RO region is
wired (mlocked), it cannot be debugged (or EVER written to.)

Under normal usage, the new mlock code will be a significant improvement
over our old stuff.

Other changes:

- add brackets to expansion of OUTB/W/L macro arguments.
- remove unused NCB structure element ns_async
- support sync. SCSI offset of 16 (instead of only 8) on 825A and 875
- correctly identify 53c810A and 53c825A chips
- preserve SCSI BIOS settings of PCI performance options
- remove (already disabled) support for NCR reset because of command timeout
- reverse order of reading of SCSI and DMA specific interrupt cause registers
- add definition of Tekram config EEPROM contents (not currently used)

24ca26f34f Significant performance improvement for mmap'ed files. This commit
makes MADV_SEQUENTIAL much more effective. I suggest that
we start using MADV_SEQUENTIAL on system utilities that mmap
their input files, and the I/O is predominantely sequential.
Below is a test with 'cmp' on two relatively large binary files,
where the files are so large that the caching is ineffective:

+ ls -l t1.xxx t2.xxx
-rw-r--r-- 1 root wheel 65598384 Dec 10 12:13 t1.xxx
-rw-r--r-- 1 root wheel 65598384 Dec 10 12:14 t2.xxx

+ time cmp t1.xxx t2.xxx
3.78user 0.70system 1:33.43elapsed 4%CPU

+ time cmpmadv t1.xxx t2.xxx
4.21user 1.05system 0:30.93elapsed 17%CPU

c579cf796a Alot of fixes from kazu:
1. All the suggestions earlier made by Bruce: renaming some symbols,
stricter error checking, removing redundant code, etc.

2. The `psm' driver preserves the default counter resolution and
report rate, whatever they are after reset. (Based on reports and
suggestion from Nate and Rob Bolin).

3. The `psm' driver now does not check the so-called sync. bit in the
first byte of the data packet by default, so that the tapping feature
of ALPUS GlidePoint works (based on reports from Louis Mamakos). I
tested the code with ALPUS Desktop GlidePoint (M/N GP101) and found
no problem; tapping worked. It appears ALPUS produces several models
of GlidePoint. I hope the other models are OK too.

The check code can still be activated by defining the PSM_CHECKSYNC
option in the config file. (The bit checking slightly reduces, if not
completely eliminates, weird mouse behavior cased by unsynchronized
mouse data packets. It also helps us to detect if the mouse interrupt
can ever be lost. But, well, if there are devices which cannot be
supported this way...)

4. The `psm' driver does not include the protocol emulation code by
default. The code can still be compiled in if the PSM_EMULATION option
is specified in the config file. Louis Mamakos suggests the emulation
code is putting too much in the kernel, and `moused' works well.
I will think about this later and decide if the entire emulation
code should be removed.

5. And, of course, the fix in `scprobe()' from Bruce to cure the
UserConfig problem. My code in `kbdio.c' is slightly different from
his patch, but has the same effect. There still is a possibility that
`scprobe()' gets confused, if, for whatever reasons, the user holds
down a key for very long time during the boot process. But we cannot
cope with everything, can we?

95e3c79022 This update adds the support for != 512 byte sector SCSI devices to

Currently sectors of size 512, 1024 & 2048 are supported, the only
restriction beeing in fdisk, which hunts for the sectorsize of
the device.
This is based on patches to od.c and the other system files by

1f92d15e77 Cosmetic code cleanup from Matt's latest driver.

b) Change [Dd][Cc]21.4. to just 21.4., seems Dec has done this to all
of the drivers for all OS's. (Did they get in trouble with someone?)
[The few that remain can either not be eliminated, or are waiting for
additional driver functional changes that will remove them.]

c) Move some code from dc21040.h into the driver, later a whole block of that
code and more will move to devar.h, but for now this makes it easier
    to study diffs.

d) Add a big bold comment to the README.de file about it not reflecting

Note that these are all cosmetic changes and should be no functional
change in the driver whatsoever. If _anyone_ spots a problem introduced
by this please let me know ASAP!

808b088926 Implement a new totally dynamic (up to MAXPHYS) buffer kva allocation
scheme. Additionally, add the capability for checking for unexpected
kernel page faults. The maximum amount of kva space for buffers hasn't
been decreased from where it is, but it will now be possible to do so.

This scheme manages the kva space similar to the buffers themselves. If
there isn't enough kva space because of usage or fragementation, buffers
will be reclaimed until a buffer allocation is successful. This scheme
should be very resistant to fragmentation problems until/if the LFS code
is fixed and uses the bogus buffer locking scheme -- but a 'fixed' LFS
is not likely to use such a scheme.

Now there should be NO problem allocating buffers up to MAXPHYS.

cab983994c Conditionally enable the busfree interrupt to catch unexpected busfrees.
Immediate SCBs, since they always send messages that tell the target to
transition to bus free now rely on the busfree interrupt instead of the
IMMEDDONE sequencer interrupt that was generated before.

Rearrange some code in the message out loop to give ATN a little more time
to drop before we ACK the last byte.

Use SPIORDY instead of REQINIT when snooping for a tag message on a reconnect.
This is done for the same reasons we use SPIORDY in the inb functions.

When going into BITBUCKET mode, turn off HDMAEN in the DFCNTRL register so
that we can "not care" what the value of HCNT is. If HCNT is 0, BITBUCKET
mode won't transfer any data if HDMAEN is set. Seeing as we don't want the
transfer to even think about touching the host, this seems more sane anyway.

6e58261e4c Change the way Automatic PIO is performed in the mesgin and mesgout phases.
SPIORDY should go active on any REQ of the bus, so testing for REQINIT is
not necessary. It also seems that testing for SPIORDY is more robust then
REQINIT since SPIORDY comes active after REQINIT and PHASEMIS seems to take
some time to come true after REQ is asserted if the phase has changed. Of
course, none of this is documented.

b9ad39602c Fix UserConfig w/syscons.
The 'getchar' function in syscons (sccngetc) is used by UserConfig to
get keyboard input from the user. When it was modified to use the
shared keyboard port routines it used the port passed in during the
probe routine. Since the probe routine was not yet called, the port was
set to 0, which is obviously not going to work.

Pre-initialize sc_port to IO_KBD which is really a kludge, but it's how
the previous driver did it's job.

5421f0d2eb Be even more careful in how we manipulate the QOUTQCNT variable. Now we
do reset it from the QOUTCNT register inside a pause/unpause. This now happens
once per command complete interrupt in the paging case (one interrupt can be
for multiple completed commands). I may introduce a counter and do a lazy
update in the future, similar to what is done with the QINCNT.

Enhance the QUEUE FULL condition handling so that the number of openings will
be reduced. This has become more important now that the driver is faster.
This code really belongs in the gerneric SCSI layer, as will be the case once
3.0 gets the code from the 'SCSI' branch.

Add some #if 0'd out trace code I've been using to help debug sequencer
problems.

Fix the SCB paging problem that I was seeing. This was only on my 7850
controller and stems from the fact that its QINFIFO can only handle 3bit
SCB identifiers. This means that you can only have 8 transactions open at
a time with the current paging scheme to these controllers. The code added
to enforce this is generic in that it tests for the number of relevent bits
that the QINFIFO can store and adjusts the max accordingly. It may be possible
to come up with a scheme that allows for more than 8 commands at a time, but
I don't know that it is worth the effort simply to fix a low end card. The
aic7880 still can do 255.

This problem may be related to what Andrey was seeing since I don't have n
aic7770 rev E chip here to test on, but as soon as someone probes one of these
cards with this new code, the dmesg output will tell the whole story.

We encountered an interesting situation where the superblock for
a file system got written to disk with the "fs_fmod" flag set to
one. It appears that this flag is normally supposed to be cleared
during ffs_sync(), but we experienced a crash, or some other weird
occurrence that left it on the disk set to 1.

Later this partition was mounted read-only... and the fs_fmod
field was never cleared, causing ffs_sync() to panic "rofs mod"
when trying to unmount that filesystem (ffs_vfsops.c: line 790).

fix:
set this bit to 0 when you load the superblock from disk.

UserConfig that will allow older/quirky hardware (most notably older IBM
ThinkPad laptops) to work with the standard boot kernel.

da55dbb36d Only access the specific interrupt status registers if required.
This follows more closely the suggestions in the latest NCR docs, and has
been running on my system for weeks with no problem. It does improve the
quality of diagnostic messages and does allow to better understand the
sequence of events in case of an error.

This should go into 2.2 and 2.1.6.

9225c9cae2 Removed gratuitous differences between ext2_readwrite.c and ufs_readwrite.c.
This fixes several bugs and one missing feature:
- cluster_read() was needlessly used for reading files of size exactly 1
  block.
- EFAULT errors for read didn't terminate the loop. This was probably
  harmless.
- IO_VMIO handling was missing near line 275. I don't know what this does.
- B_CLUSTEROK was only set if (doclusterwrite) nead line 293. This was
harmless, if only because another bug prevents doclusterwrite from being
  0.
- MNT_NOATIME wasn't implemented.

This should be in 2.2, of course.

857450d0ac Don't switch from fast interrupt handlers to normal interrupt
handlers if interrupts are nested more than a few (3) deep. This
only reduces the maximum nesting level by 1 with the standard
drivers unless there is a related bug somewhere, but can't hurt
much (the worst case is returning to hoggish interrupt handler like
wdintr(), but such interrupt handlers hurt anyway).

Fixed a previously harmless race incrementing the interrupt nesting
level.

This should be in 2.1.6 and 2.2.

69de44e25c Count only hardware interrupts in cnt.v_intr, so that the individual
hardware interrupt counts add up to the total. Previously, software
interrupts generated by splz() were counted in the total. These
software interrupts seem to be very rare - there have apparently been
0 of them on freefall among the last 352448857 interrupts.

7abcc0e65a Improve the queuing algorithms used by NFS' asynchronous i/o. The
existing mechanism uses a global queue for some buffers and the
vp->b_dirtyblkhd queue for others. This turns sequential writes into
randomly ordered writes to the server, affecting both read and write
performance. The existing mechanism also copes badly with hung
servers, tending to block accesses to other servers when all the iods
are waiting for a hung server.

The new mechanism uses a queue for each mount point. All asynchronous
i/o goes through this queue which preserves the ordering of requests.
A simple mechanism ensures that the iods are shared out fairly between
active mount points. This removes the sysctl variable vfs.nfs.dwrite
since the new queueing mechanism removes the old delayed write code
completely.

This should go into the 2.2 branch.
