bdbf3440ce Revert r345706: the third time will be the charm
When a review is closed via Phabricator it updates the patch attached to the
review. I downloaded the raw patch from Phabricator, applied it, and repeated
my mistake from r345704 by accident mixing content from D19732 and D19738.

For my own personal sanity, I will try not to mix reviews like this in the
future.

760b1a815b Standardize -std=c++* as CXXSTD`
CXXSTD was added as the C++ analogue to CSTD.

CXXSTD defaults to `-std=c++11` with supporting compilers; `-std=gnu++98`,
otherwise for older versions of g++.

This change standardizes the CXXSTD variable, originally added to
googletest.test.inc.mk as part of r345203.

As part of this effort, convert all `CXXFLAGS+= -std=*` calls to use `CXXSTD`.

Notes:

This value is not sanity checked in bsd.sys.mk, however, given the two
most used C++ compilers on FreeBSD (clang++ and g++) support both modes, it is
likely to work with both toolchains. This method will be refined in the future
to support more variants of C++, as not all versions of clang++ and g++ (for
instance) support C++14, C++17, etc.

Any manual appending of `-std=*` to `CXXFLAGS` should be replaced with CXXSTD.
Example:

Before this commit:

9a41926bfb CXXSTD is the C++ analogue to CSTD.

100b034530 Make the "KB/s" field one character shorter (by reducing the precision)
and use the space to make the "tps" one character longer.

It makes the iostat(8) output a bit less messed up.

286258a9a0 makefs: Fix "time" mtree attribute handling
When processing mtree(5) MANIFEST files, makefs(8) previously threw an
error if it encountered an entry whose "time" attribute contained a
non-zero subsecond component (e.g. time=1551620152.987220000).

Update the handling logic to properly assign the subsecond component if
built with nanosecond support, or silently discard it otherwise.

Also, re-enable the time attribute for the kyua tests.

fa707ac77f trim(8): emit more user-friendly error message in verbose mode.
If underlying driver provides no TRIM/UNMAP support and operation fails
due to this reason, state it clearly in verbose mode (default)
instead of writing standard message that may be too cryptic for a user:

	trim: ioctl(DIOCGDELETE) failed: nda0: Operation not supported

Now it would write:

	trim: nda0: TRIM/UNMAP not supported by driver

But still use previous format including errno value for quiet mode.

Small candelete() function borrowed from diskinfo(8) code.
This function was committed by Alan Somers <asomers@FreeBSD.org>,
so give him some credit.

7bb1073842 Fix bhyve's NVMe Identify Namespace data
The NVMe Identify Namespace data structure's Number of LBA Formats
(NLBAF) field is a 0's based value (i.e. 0x0 means 1). Since the
emulation only supports a single format, set NLBAF to 0x0, not 1.

7f65491576 trim(8): add another safety net
It is quite easy make a mistake and run something like this:

	trim -f /dev/da0 -r rfile

This would trim the whole device then emit an error on non-existing file -r.

Add another check to prevent this while allowing this form still
for real object names beginning from dash:

	trim -f -- /dev/da0 -r rfile

9784899d58 Fix compilation of world with WITHOUT_{INET,INET6}_SUPPORT or both set.
Buildworld failed when both WITHOUT_INET6_SUPPORT and INET equivalent were set.
Fix netstat and syslogd by applying appropriate #ifdef INET/INET6 to make world
compile again.

ee74c3327a rtsol: Use vwarnx(3) to log messages to standard error.
This ensures that the program name is included in the output, which
makes it easy to identify the source of error messages printed
during boot.

7e4520854c Fix kldxref on PowerPC64
When using kldxref on kernel modules built with clang8 + lld8,
kldxref would be unable to find the modules metadata information,
because PowerPC64 was using the ef_nop.c implementation of
ef_reloc().

When GNU LD was used, it was also relocating the metadata section of
the .ko file. LLD does not do this, but only generate dynamic
relocations for it. With minor changes, ef_powerpc.c can now work
for PowerPC64 too.

781e78182d Improve error handling: bail out if one of the files scheduled
to go to the FS image we are making cannot be read (e.g. EPERM).
Current behaviour when we issue waring but still proceeed and
return success is definitely not correct: masking out error
condition as well as making a slighly inconsistent FS where
attempt to access the file in question ends up in EBADF. See
linked DR for details.

1f641a35fd Fix several Coverity-detected issues in newsyslog.
- CID 1394815, CID 1305673: Dereference before null check - memory was
  allocated and the allocation checked for NULL with a call to errx()
  if it failed. Code below that was guaranteed that the pointer was
  non-NULL, but there was another check for NULL at the exit of the
  function (after the memory had already been referenced). Eliminate
  the useless NULL check.

- CID 1007452: Resource leak - Storage intended to be allocated and
  returned to the caller was never freed. This was the result of a
  regression in the function signature introduced in r208648 (2010)
  (thanks for that find, @cem!). Fixed by altering the function
  signature and passing the allocated memory to the caller as
  intended. This also fixes PR158794.

- CID 1008620: Logically dead code in newsyslog.c - This was a direct
  result of CID 1007452. Since the memory allocated as described there
  was not returned to the caller, a subsequent check for the memory
  having been allocated was dead code. Returning the memory
  re-animates the code that is the subject of this CID.

- CID 1006131: Unused value - in parsing a configuration file, a
  pointer to the end of the last field was saved, but not used after
  that. Rewrite to use the pointer value. This could have been fixed
  by avoiding the assignment altogether, but this solutions more
  closely follows the pattern used in the preceding code.

c7189f3694 Revert r344389 & r343906 - Fix Coverity errors in newsyslog
It was pointed out that a couple of the "memory leak" CIDs that I
fixed were arguably Coverity errors rather than errors in the
newsyslog code and the cure was worse than the disease. Revert both
changes. The first change, which included fixes for other Coverity
errors, will be re-worked to omit the troublesome changes and then
re-committed with the remaining fixes.

d09131e044 Connect the restored dumb and sc terminal emulators to the kernel build.
Add or fix options to control static and dynamic configuration.  Keep
the default of scteken, but default to statically configuring all available
emulators (now 3 instead of 1).

The dumb emulator is almost usable.  libedit and libreadline handle
dumb terminals perfectly for at least shell history.  less(1) works
as well as possible except on exit.  But curses programs make messes.
The dumb emulator has strange color support, with 2 dumb colors for
normal output but fancy colorization for the cursor, mouse pointer and
(with a non-dumb initial emulator) for low-level console output.

Using the sc emulator instead of the default of scteken fixes at least
the following bugs:
- NUL is a printing character in cons25 but not in teken
- teken doesn't support fixed colors for "reverse" video.
- The best versions of sc are about 10 times faster than scteken (for
  printing to the frame buffer).  This version is only about 5 times
  faster.

Fix configuration features:
- make SC_DFLT_TERM (for setting the initial emulator) a normal option.

Add configuration features:
- negative options SC_NO_TERM_* for omitting emulators in the static config.
  Modules for emulators might work, but I don't know of any
- vidcontrol -e shows the available emulators
- vidcontrol -E <emulator> sets the active emulator.

c0ca37d17b Fix the usage error I introduced in r344192.
Specifically, I put the new option line in the wrong place, and then fixed
up the rest without realizing it.  This puts the usage statement back to
what it was, with an additional line for the new -V option.

c6da8eb21f * Handle SIGPIPE in gssd
We've got some cases where the other end of gssd's AF_LOCAL socket gets
closed, resulting in an error (and SIGPIPE) when it tries to do I/O to it.
Closing without cleaning up means the next time nfsd starts up, it hangs,
unkillably; this allows gssd to handle that particular error.

* Limit the retry cound in gssd_syscall to 5.
The default is INT_MAX, which effectively means forever.  And it's an
uninterruptable RPC call, so it will never stop.

The two changes mitigate the problem.

2c3e626a0d Complete fix for CID 1007454, CID 1007453: Resource leak in newsyslog
The result of a strdup() was stored in a global variable and not freed
before program exit. This is a follow-up to r343906. That change
attempted to plug these resource leaks but managed to miss a code path
on which the leak still occurs. Plug the leak on that path, too.

025816d9ce Add support for a virtual hostname to nfsd
Specifically, this allows (via "-V vhostname") telling nfsd what principal
to use, instead of the hostname.  This is used at iXsystems for fail-over in
HA systems.

11e67b92b5 In r340044 an attempt to quiet coverity warning cid 1357336
was incorrectly implemented leading to a possible double free.

It is possible for both the conditional free,
and the unconditional free added in r340044 to be done,
fix that by initializing uopt to NULL,
removing the conditional free,
and only using the unconditional free at the end.

60c81cf107 Add an example to pw.8 about how to add an existing user to a group.
Instead of using pw to modify group membership, users often edit
/etc/group by hand, which is discouraged.  Provide an example of
adding a user to the wheel group, which is a common use case.
I'm using a different user here as in the previous example as that
deleted the user (although the examples don't necessarily have to
be followed in order).

52467047aa Regularize the Netflix copyright
Use recent best practices for Copyright form at the top of
the license:
1. Remove all the All Rights Reserved clauses on our stuff. Where we
   piggybacked others, use a separate line to make things clear.
2. Use "Netflix, Inc." everywhere.
3. Use a single line for the copyright for grep friendliness.

fe74eaab7c Check element type before setting LEDs.
With r319610, sesutil started twiddling the bits of every SES device.
Not everything is a disk slot, there are also fan controllers, temperature
sensors, even power supplies, among other things controlled by SES.

Add a type check to make sure we are only operating on device slot and array
device slot elements.  Other type elements will be skipped, but it would be
simple to add additional cases for controlling the ident LEDs of other
element types (which are not necessarily the same bits).

Rather than doing raw bit manipulation of an unstructured byte array using
unnamed numeric constants, leverage existing code abstractions.

96329ce7e2 Silence Clang Scan warning about use of unitialized variable.
While the warning is a false positive, it is possible to clarify the code by
always initializing the variable. This does also allow to make the sending
of the "beep" control sequence depend on the validity of its parameters.

I have left the redundant assignment of 0 to the now initialized variables
in place since this makes the code simpler to understand and does not add
any run-time overhead (the compiler completely removes the "else if" test
and the assignments).

There was an embedded literal escape character in a string, which messes up
diplaying the source code on a terminal that interprets ANSI sequences. The
literal escape has been replaced by \e (non-standard, but supported by all
relevant compilers, and already used in other source files in base).

0fc0faf846 Add bluetooth-config script to simplify setting up bluetooth connections to
devices like mice, keyboards, bt-audio, ...

This script currently allows scanning for nearby devices, adds one to
/etc/bluetooth/hosts, adds an entry to hcsecd's conf and if it is a HID, add an
entry to bthidd's configs, as well.

e4e0dad424 cpucontrol: check for the supposed firmware file type and skip
non-regular entry.

do_update() skips entries with DT_DIR type.  This does not eliminate
other entries that might exist in the directory.  More, since NFS
might fill d_type with DT_UNKNOWN, dot and dotdot entries are not
skipped, then mmap(2) call failed for them when update microcode files
are located on NFS.

5c81ba5ade powerd(8): allow to force a method of battery state query
This change allows to determine power source via sysctl or /dev/apm
when devd(8) is running (used by default).

Based on patch from PR; other changes on top of it:

(if APM is enabled);

04e9edb544 Capsicumize rtsol(8) and rtsold(8).
These programs parse ND6 Router Advertisement messages; rtsold(8) has
required an SA, SA-14:20.rtsold, for a bug in this code.  Thus, they
are good candidates for sandboxing.

The approach taken is to run the main executable in capability mode
and use Casper services to provide functionality that cannot be
implemented within the sandbox.  In particular, several custom services
were required.

- A Casper service is used to send Router Solicitation messages on a
  raw ICMP6 socket.  Initially I took the approach of creating a
  socket for each interface upon startup, and connect(2)ing it to
  the all-routers multicast group for the interface.  This permits
  the use of sendmsg(2) in capability mode, but only works if the
  interface's link is up when rtsol(d) starts.  So, instead, the
  rtsold.sendmsg service is used to transmit RS messages on behalf
  of the main process.  One could alternately define a service
  which simply creates and connects a socket for each destination
  address, and returns the socket to the sandboxed process.  However,
  to implement rtsold's -m option we also need to read the ND6 default
  router list, and this cannot be done in capability mode.
- rtsold may execute resolvconf(8) in response to RDNSS and DNSSL
  options in received RA messages.  A Casper service is used to
  fork and exec resolvconf(8), and to reap the child process.
- A service is used to determine whether a given interface's
  link-local address is useable (i.e., not duplicated or undergoing
  DAD).  This information is supplied by getifaddrs(3), which reads
  a sysctl not available in capability mode.  The SIOCGIFCONF socket
  ioctl provides equivalent information and can be used in capability
  mode, but I decided against it for now because of some limitations
  of that interface.

In addition to these new services, cap_syslog(3) is used to send
messages to syslogd.

fbac7e0ba2 Fix bhyve's NVMe Completion Queue entry values
The function which processes Admin commands was not returning the
Command Specific value in Completion Queue Entry, Dword 0 (CDW0). This
effects commands such as Set Features, Number of Queues which returns
the number of queues supported by the device in CDW0. In this case, the
host will only create 1 queue pair (Number of Queues is zero based).
This also masked a bug in the queue counting logic.

76e47b9420 Fix bhyve's NVMe queue bookkeeping
Many size / length parameters in NVMe are "0's based", meaning, a value
of 0x0 represents 1, 0x1 represents 2, etc.. While this leads to an
efficient encoding, it can lead to subtle bugs. With respect to queues,
these parameters include:
 - Maximum number of queue entries
 - Maximum number of queues
 - Number of Completion Queues
 - Number of Submission Queues

To be consistent, convert all 0's based values from the host to 1's
based value internally. Likewise, covert internal 1's based values to
0's based values when returned to the host. This fixes an off-by-one bug
when creating IO queues and simplifies some of the code. Note that this
bug is masked by another bug.

While in the neighborhood,
 - fix an erroneous queue ID check (checking CQ count when deleting SQ)
 - check for queue ID of 0x0 in a few places where this is illegal
 - clean up the Set Features, Number of Queues command and check for
   illegal values

5fa62a9335 Fix ESP generation when using a gmirror, and when booting from RO medium
When using a gmirror, entries in /dev can be removed. So instead of using
kern.disks, get the list of disks from "gpart status -sg" instead.

We assume that any 'efi' partition that can't be mounted as msdosfs should
be used as an ESP. However, the ESP on the CD/DVD can't be mounted read-write
and so was being treated as if unformatted. Try the mount as read-only
instead, to catch cases like this.

config(8)'s option handling has been written to allow duplicate options; if
the value changes, then the latest value is used and an informative message
is printed to stderr like so:

/usr/src/sys/amd64/conf/TEST: option "VERBOSE_SYSINIT" redefined from 0 to 1

Currently, this is only a possibility for cpu types, MAXUSERS, and
MACHINE_ARCH. Anything else duplicated in a config file will use the first
value set and error about duplicated options on subsequent appearances,
which is arguably unfriendly since one could specify:

include GENERIC
nooptions VERBOSE_SYSINIT
options VERBOSE_SYSINIT

to redefine the value later anyways.

db8b561345 Rework UEFI ESP generation
Currently, the installer uses pre-created 800KB FAT12 filesystems that
it dd's onto the ESP partition.
This changeset improves that by having the installer generate a FAT32
filesystem directly onto the ESP using newfs_msdos and then copying
loader.efi into /EFI/freebsd.
For live installs it then runs efibootmgr to add a FreeBSD boot entry
in the BIOS.

385e98080c Remove CTM from 13-CURRENT after the release of FreeBSD-12.0.
The removal (and creation of a port) has been pre-announced in UPDATING
1 month ago. Packages are available for all supported FreeBSD vesions.

I did not think that another entry in UPDATING is required to note the
actual removal.

No MFC is planned - CTM shall be kept in base for all releases up to 12.x.

the original virtio-scsi commit.  This change allows wanted device path to
be specified in either of two ways:
 -s 6,virtio-scsi,/dev/cam/ctl1.1
 -s 6,virtio-scsi,dev=/dev/cam/ctl2.3
If neither is specified, the default /dev/cam/ctl device is used.

While there, remove per-queue CTL device opening, which makes no sense at
this point.

99fa47de81 Fix several iov handling bugs in bhyve virtio-scsi backend.
- buf_to_iov() does not use buflen parameter, allowing out of bound read.
 - buf_to_iov() leaks memory if seek argument > 0.
 - iov_to_buf() doesn't need to reallocate buffer for every segment.
 - there is no point to use size_t for iov counts, int is more then enough.
 - some iov function arguments can be constified.
 - pci_vtscsi_request_handle() used truncate_iov() incorrectly, allowing
   getting out of buffer and possibly corrupting data.
 - pci_vtscsi_controlq_notify() written returned status at wrong offset.
 - pci_vtscsi_controlq_notify() leaked one buffer per event.

fb14e73cb4 Normally when an attempt is made to mount a UFS/FFS filesystem whose
superblock has a check-hash error, an error message noting the
superblock check-hash failure is printed and the mount fails. The
administrator then runs fsck to repair the filesystem and when
successful, the filesystem can once again be mounted.

This approach fails if the filesystem in question is a root filesystem
from which you are trying to boot. Here, the loader fails when trying
to access the filesystem to get the kernel to boot. So it is necessary
to allow the loader to ignore the superblock check-hash error and make
a best effort to read the kernel. The filesystem may be suffiently
corrupted that the read attempt fails, but there is no harm in trying
since the loader makes no attempt to write to the filesystem.

Once the kernel is loaded and starts to run, it attempts to mount its
root filesystem. Once again, failure means that it breaks to its prompt
to ask where to get its root filesystem. Unless you have an alternate
root filesystem, you are stuck.

Since the root filesystem is initially mounted read-only, it is
safe to make an attempt to mount the root filesystem with the failed
superblock check-hash. Thus, when asked to mount a root filesystem
with a failed superblock check-hash, the kernel prints a warning
message that the root filesystem superblock check-hash needs repair,
but notes that it is ignoring the error and proceeding. It does
mark the filesystem as needing an fsck which prevents it from being
enabled for writing until fsck has been run on it. The net effect
is that the reboot fails to single user, but at least at that point
the administrator has the tools at hand to fix the problem.

b307954481 In hardened systems, where the security.bsd.unprivileged_proc_debug sysctl
node is set, allow setting security.bsd.unprivileged_proc_debug per-jail.
In part, this is needed to create jails in which the Address Sanitizer
(ASAN) fully works as ASAN utilizes libkvm to inspect the virtual address
space. Instead of having to allow unprivileged process debugging for the
entire system, allow setting it on a per-jail basis.

The sysctl node is still security.bsd.unprivileged_proc_debug and the
jail(8) param is allow.unprivileged_proc_debug. The sysctl code is now a
sysctl proc rather than a sysctl int. This allows us to determine setting
the flag for the corresponding jail (or prison0).

As part of the change, the dynamic allow.* API needed to be modified to
take into account pr_allow flags which may now be disabled in prison0.
This prevents conflicts with new pr_allow flags (like that of vmm(4)) that
are added (and removed) dynamically.

Also teach the jail creation KPI to allow differences for certain pr_allow
flags between the parent and child jail. This can happen when unprivileged
process debugging is disabled in the parent prison, but enabled in the
child.

ab478b0185 Prevent periodic/etc/weekly/340.noid from descending into root directories

1e1a4743ba jail(8): introduce new command option -e to exhibit
a list of configured non-wildcard jails with their parameters,

The option -e takes separator argument that is used
to separate printed parameters. It will be used with following

9165316ff6 newsyslog.conf: Restrict included files in default config to [!.]*.conf
The new default config will only include files from the following
directories which end with '.conf' and do not beginning with a '.'
character:
- /etc/newsyslog.conf.d/
- /usr/local/etc/newsyslog.conf.d/

This matches the syslog.conf(5) functionality, and also prevents '.sample' or
'.pkgnew' files being included. This is important for ports which install files
in /usr/local/etc/newsyslog.conf.d/ and also for pkgbase.

201100c58b Initial implementation of draft-ietf-6man-ipv6only-flag.
This change defines the RA "6" (IPv6-Only) flag which routers
may advertise, kernel logic to check if all routers on a link
have the flag set and accordingly update a per-interface flag.

If all routers agree that it is an IPv6-only link, ether_output_frame(),
based on the interface flag, will filter out all ETHERTYPE_IP/ARP
frames, drop them, and return EAFNOSUPPORT to upper layers.

The change also updates ndp to show the "6" flag, ifconfig to
display the IPV6_ONLY nd6 flag if set, and rtadvd to allow
announcing the flag.

Further changes to tcpdump (contrib code) are availble and will
be upstreamed.

Tested the code (slightly earlier version) with 2 FreeBSD
IPv6 routers, a FreeBSD laptop on ethernet as well as wifi,
and with Win10 and OSX clients (which did not fall over with
the "6" flag set but not understood).

We may also want to (a) implement and RX filter, and (b) over
time enahnce user space to, say, stop dhclient from running
when the interface flag is set.  Also we might want to start
IPv6 before IPv4 in the future.

All the code is hidden under the EXPERIMENTAL option and not
compiled by default as the draft is a work-in-progress and
we cannot rely on the fact that IANA will assign the bits
as requested by the draft and hence they may change.

97820530b1 Fix dialog autosizing to accomodate for hline
dialog will conditionally ignore the --hline option if not enough space
was available to accomodate for the text width. Traditionally the width
of the widget had to be 10 wider than the text. Recent updates to dialog
have changed the requirement to be at least 12 wider than the hline text
else the hline text is not rendered at the bottom of the widget.

b25a469f94 rcorder(8): add support for /etc/rc.resume, so it calls "rcorder -k resume"
and runs scripts containing "KEYWORD: resume" with single "resume" argument.

Working example is the port sysutils/cpupdate that defines
extra_commands="resume" to reload CPU microcode cleared
by suspend/resume sequence.

9edef078cc cxgbetool(8): Add a subaction (tcbrss <n>) that can be used with "pass"
action to distribute traffic using the half of the VI's RSS indirection
table.

The value specified should either be the start of the VI's RSS slice
(available at dev.<ifname>.<inst>.rss_base since r339700) or the
midpoint (rss_base + rss_size/2).  The traffic that hits the filter will
use the first or second half of the indirection table respectively.
The indirection table can be populated in different ways to achieve
different kinds of traffic/load distributions.  For example, r339749
allows a netmap interface to have half the rx queues in the first half
of the table and the rest in the other.

e08bb1091c Implenent --fromfile to read variable values when printing variables
So

./efivar --fromfile Boot0001.bin --print --load-option

will take the value from Boot0001.bin file and then decode it as if it
were a load-option. This is useful for debugging handling of such
variables that may be hanging the boot for some people.

8883128b8e Allow the bhyve VNC server to listen on IPv6 for incoming connections.
Alternatively to IPv4 address:port this will allow to listen on IPv6
link-local (incl. scope), a specific address, or ::.  Addresses have
to be given in RFC2732 format so that [::]:port parsing will work.

This patch also starts to introduce WITH_INET/INET6_SUPPORT to bhyve.

fb72b618c5 In bhyve's fbuf emulation improve the overall "usage" message and
for the vga option, rather than printing the entire option string,
only print vga (as we do for everything else).

c1cdf6a42f Remove mse(4) from tree
Remove mse and all support for bus and inport devices from the tree.
Data from nycbug's dmesg database shows the last sighting of this
driver was in 4.10 on only one machine.

8e1887fa1a pwd_mkdb: retire -B and -L endianness options
Legacy v3 db support was retired in r333133, and it was v3 support that
required the -B and -L options.  The options were retained temporarily,
but now that stable/12 has branched they can be removed.

618703b57b Fix a dead store.
We would fail to clear DNS search list configuration if a router
stopped specifying the DNSSL RA option.  I suspect that the bug
was mostly harmless, as the RDNSS and DNSSL options are typically used
together and omitting the RDNSS option would have the same effect.
