39ed7f250a Remove mbpool(9) now that it has no consumers.
mbpool existed to support NICs with memory interfaces and all remaining
comsumers were removed earlier this year with NATM.

b0900232b9 Revert "Unify boot1 with loader" change r324646
Back out the unification commit to boot1. There's some issues on the
arm and arm64 platforms that need to be addressed with code
changes. There's also a discussion on arch@ about the future of
boot1.efi vs just using loader.efi that needs to play out. So take a
pause on these changes until the arm issues can be fixed and it's
clear boot1.efi will survive into FreeBSD 12.

0bf94a089d embed_mfs: add error handling, usage
Ensure that we are called with two arguments, and that the output file
is writable. Also, if we cannot find the mfs section report the output
file name rather than "kernel", as this script may be used with other
than kernels.

b541ba195c cxgbe: delete now-redundant vnet decls
r324539 gathered some vnet decls into netinet/tcp_var.h, so that they
are now redundant in dev/cxgbe/tom/{t4_cpl_io.c,t4_ddp.c}. This triggers
gcc -Wredundant-decls.

fa00affd18 Fix a racy VI_DOOMED check in MNT_VNODE_FOREACH_ALL().
MNT_VNODE_FOREACH_ALL() is supposed to avoid returning doomed vnodes,
but the VI_DOOMED check it used was done without the vnode interlock
held, so it could race with a concurrent vgone().

14481f92c4 loader.mk: clean md.o even if MD_IMAGE_SIZE not defined
We don't normally provide special handling for optionally-included src
files, but md.o depends on both md.c and the value of ${MD_IMAGE_SIZE}.
Previously if one built with MD_IMAGE_SIZE, executed "make clean", and
then built with a different MD_IMAGE_SIZE md.o would not be rebuilt.

ad10496cf4 never retry oustanding requests when terminating iscsi session
CAM_REQ_ABORTED sounds natural for aborting outstanding requests when
tearing down a session, but that status actually causes eligible
requests to be tried again.  That's completely useless, so let's use
CAM_DEV_NOT_THERE instead.  Perhaps there is a better status, but this
should be good enough.  The change should affect only the session
termination.

20e9cab5fa iscsi: do not hold the global lock while tearing down a session
It should be sufficient to hold the lock just for removing the session
from the session list.  Everything else should be covered by the session
specific lock.

On top of that, at present we can get a deadlock caused by waiting on
the CAM SIM reference count while holding the global lock.  A specific
scenario involving ZFS is this:
- concurrent termination of two sessions, S1 and S2
- session S1 completed all I/Os and sleeps in CAM waiting for device
  close by ZFS;
- session S2 is also dead now, but can not forcefully complete
  outstanding requests by calling iscsi_session_cleanup() from
  iscsi_maintenance_thread_terminate(), since it can't get the same
  global sc_lock;
- as soon as there are unfinished requests, ZFS can not do
  spa_config_enter() as writer, and so can not close the device for
  session S1;
- deadlock.

f49c813c1d Use taskqueue(9) to do writes/commits to mirrored DSs concurrently.
When the NFSv4.1 pNFS client is using a Flexible File Layout specifying
mirrored Data Servers, it must do the writes and commits to all mirrors.
This patch modifies the client to use a taskqueue to perform these writes
and commits concurrently.
The number of threads can't be changed for taskqueue(9), so it is set
to 4 * mp_ncpus by default, but this can be overridden by setting the
sysctl vfs.nfs.pnfsiothreads.

e9445808a8 Re-evaluate thread' signal mask after ptracestop().
The stop drops process lock, which allows the signal mask to be
changed and our selected signal might become blocked, i.e. should be
returned to the process queue instead of delivery.

Also, for the existing check of the process no longer having an
attached debugger, we should not loose the signal, but requeue it.

0d8e04054e Properly reset the fields in clean_unrhdr.
In r324542 I neglected to reset the first and last fields of struct
unrhdr. This causes a tmpfs to fail the unr(9) consistency checks with
DIAGNOSTIC on. Fix this by resetting the fields by calling init_unrhdr.
While here, change a loop to use TAILQ_FOREACH_SAFE since it is more
readable and equally fast.

ca1f624517 Fix the pv_chunks pc_lru tailq handling in reclaim_pv_chunk().
For processing, reclaim_pv_chunk() removes the pv_chunk from the lru
list, which makes pc_lru linkage invalid.  Then the pmap lock is
released, which allows for other thread to free the last pv entry
allocated from the chunk and call free_pv_chunk(), which tries to
modify the invalid linkage.

Similarly, the chunk is inserted into the private tailq new_tail
temporary.  Again, free_pv_chunk() might be run and corrupt the
linkage for the new_tail after the pmap lock is dropped.

This is a consequence of r299788 elimination of pvh_global_lock, which
allowed for reclaim to run in parallel with other pmap calls which
free pv chunks.

As a fix, do not remove the chunk from pc_lru queue, use a marker to
remember the position in the queue iteration.  We can safely operate
on the chunks after the chunk's pmap is locked, we fetched the chunk
after the marker, and we checked that chunk pmap is same as we have
locked, because chunk removal from pc_lru requires both pv_chunk_mutex
and the pmap mutex owned.

Note that the fix lost an optimization which was present in the
previous algorithm.  Namely, new_tail requeueing rotated the pv chunks
list so that reclaim didn't scan the same pv chunks that couldn't be
freed (because they contained a wired and/or superpage mapping) on
every invocation.  An additional change is planned which would improve
this.

7f20726e4b Move common/Makefile.inc to sys/boot/loader.mk.
Makefile.inc has a specific meaning in the tree, and

fa271a5d09 Closer examination shows that nvme and CAM both normally zero-fill
allocations (for req and ccb, which ultimately contain the
nvme_cmd). As such, we can micro-optimize these routines. Add a
comment to this effect, and bzero the ccb used to make the requests
for the nda dump rotuine so it more closely matches a ccb allocated
with xpt_get_ccb().

57ef3db3a9 Fix the client IP address reported by nfsdumpstate for 64bit arch and NFSv4.1.
The client IP address was not being reported for some NFSv4 mounts by
nfsdumpstate. Upon investigation, two problems were found for mounts
using IPv4. One was that the code (originally written and tested on i386)
assumed that a "u_long" was a "uint32_t" and would exactly store an
IPv4 host address. Not correct for 64bit arches.
Also, for NFSv4.1 mounts, the field was not being filled in. This was
basically correct, because NFSv4.1 does not use a callback address.
However, it meant that nfsdumpstate could not report the client IP addr.
This patch should fix both of these issues.
For IPv6, the address will still not be reported. The original NFSv4 RFC
only specified IPv4 callback addresses. I think this has changed and, if so,
a future commit to fix reporting of IPv6 addresses will be needed.

80a2d1406f Fix the handling of parital and too short chunks.
Ensure that the current behaviour is consistent: stop processing
of the chunk, but finish the processing of the previous chunks.

This behaviour might be changed in a later commit to ABORT the
assoication due to a protocol violation, but changing this
is a separate issue.

03cea61b3a Add information needed by Linux libdrm 2.4.74 (shipped with CentOS 7.4).
Create a config file for PCI devices that exposes their configuration
space.  Only fields needed by libdrm are filled in (vendor, device,
revision, subvendor and subdevice).

Link /sys/class/drm/card%d/device to the PCI device directory.

11ce4d9f39 When a Linux program tries to access a /path the kernel tries
/compat/linux/path before /path.  Stop following symbolic links when
looking up /compat/linux/path so dead symbolic links aren't ignored.
This allows syscalls like readlink(2) and lstat(2) to work on such links.
And open(2) will return an error now instead of trying /path.

29431e54b9 Use nvme_ctrlr_poll instead of nvme_ctrlr_intx_handler since it is
more general and doesn't try to access registers that may be undefined
when the card is in MSIX mode.

This change, along with r324630, r324631, r324632, makes nda crash
dumps work again. Previously, they only worked on CPU 0 when the stack
garbage was just so.

fbed8df259 Explicitly set reserved fields and 'fuse' to 0. This prevents us from
acidentally sending bogus values in these fields, which some drives
may reject with an error or worse (undefined behavior).

This is especially needed for the ndadump routine which allocates the
cmd from stack garbage....

1dbf52e7d9 Reduce traffic on vm_cnt.v_free_count
The variable is modified with the highly contended page free queue lock.
It unnecessarily shares a cacheline with purely read-only fields and is
re-read after the lock is dropped in the page allocation code making the
hold time longer.

Pad the variable just like the others and store the value as found with
the lock held instead of re-reading.

Provides a modest 1%-ish speed up in concurrent page faults.

9db0f8e76f Make the PHOLD in linux_wait_event_common() unconditional.
After some in-progress work is committed, this would otherwise be the only
instance of #if(n)def NO_SWAPPING in the tree. Moreover, the requisite
opt_vm.h include was missing, so the PHOLD/PRELE calls were always being
compiled in anyway.

41bf90bb78 Address two problems with sendfile(..., SF_NOCACHE) and apply one
"optimization".  First, sendfile(..., SF_NOCACHE) frees pages without
checking whether those pages are mapped.  This can leave the system
with mappings to free or repurposed pages.  Second, a page can be
busied between the time of the current busy test and acquiring the
object lock.  Essentially, the test performed before the object lock
is acquired can only be regarded as an optimization to short-circuit
further work on the page.  It cannot, however, be relied upon to prove
that it is safe to free the page.  Third, when sendfile(..., SF_NOCACHE)
was originally implemented, vm_page_deactivate_noreuse() did not yet
exist.  Use vm_page_deactivate_noreuse() instead of vm_page_deactivate(),
because it comes closer to freeing the page.

627ac5b4e3 Don't call selrecord() outside the select system call in the LinuxKPI, because
then td->td_sel is NULL and this will result in a segfault inside selrecord().
This happens when only using kqueue() to poll for read and write events.
If select() and kqueue() is mixed there won't be a segfault.

f92e3400bc remove process and jail directory machinations from dounmount
The manipulations done by mountcheckdirs() are not that useful during
the unmount, they can bring about unexpected security consequences.

Thic change effectively reverts the change in r73241.

The change also allows to simplify the handling of rootvnode global
variable.

565312d184 [ath] Begin using the replacement EDCA functions.
As part of ath10k and other chipset support, the EDCA stuff has to be moved
to potentially be per-VAP.  For hardware that doesn't support it (ie,
everything that we currently support) it can just fetch the "current"
global EDCA parameters for the NIC.

This is one of those parameters that is linked to the currently active
channel context / VAP in Linux mac80211 parlance.

48f95a360e [net80211] begin handling multiple hardware decap'ed A-MSDU in the RX path.
The duplicate detection code currently expects A-MSDU frames to be encaped -
they're decap'ed /after/ duplicate detection.

However for ath10k (and iwm hardware later on) the firmware supports
doing A-MSDU decap in hardware - which shows up as multiple frames with
the same sequence number and IV.

This is the first part of decap handling - if we see a stretch of A-MSDU
frames from the driver with the MORE bit set, then don't treat them
as duplicates.

This isn't 100% complete as crypto sequence number handling and "A-MSDU in
A-MPDU" needs handling, but it's a start.

This should be a glorified no-op for everyone.  Please tell me if it isn't.

5532aa9bb4 allow posix_fallocate in capability mode
posix_fallocate is logically equivalent to writing zero blocks to the
desired file size and there is no reason to prevent calling it in
capability mode. posix_fallocate already checked for the CAP_WRITE
right, so we merely need to list it in capabilities.conf.

a37f4eb659 Define prototype for exit and ensure references
Define a prototype for exit in stand.h. Provide a reference to exit in
a few conf.c files to ensure that its definition gets pulled in early.
Since exit() is a MD routine, it isn't defined in libsa. However,
libsa tends to be listed last and will soon have panic() in it which
calls exit(). The reference to exit early ensures that the MD exit is
available to satisfy linking for static libraries.

9aaf913e13 When unmounting a tmpfs, do not call free_unr.
tmpfs uses unr(9) to allocate inodes. Previously when unmounting it
would individually free the units when it freed each vnode. This is
unnecessary as we can use the newly-added unrhdr_clear function to clear
out the unr in onde go. This measurably reduces the time to unmount a
tmpfs with many files.

333dcaa498 Add clearing function for unr(9).
Previously before you could call unrhdr_delete you needed to
individually free every allocated unit. It is useful to be able to tear
down the unr without having to go through this process, as it is
significantly faster than freeing the individual units.

a7b8be82f0 hwpmc(4): Force sufficiently wide type for left shift
Ordinary input to this macro comes from pe_code, which is uint16_t.  Coverity
points out that shifting such a value discards the result of a 24 bit shift,

70e3b262d1 The th_bintime, th_microtime and th_nanotime members of the timehand
all cache the last system time (uptime + boottime).  Only the format
differs.  Do not re-calculate the bintime and simply use the value
used to calculate the microtime and nanotime.

Group all the updates under the relevant comment.  Remove obsoleted
XXX part.

63918d3848 Fix forced dismount when a pNFS mount is hung on a DS.
When a "pnfs" NFSv4.1 mount is hung because of an unresponsive DS,
a forced dismount wouldn't work, because the RPC socket for the DS
was not being closed. This patch fixes this.
This will only affect "pnfs" mounts where the pNFS server's DS
is unresponsive (crashed or network partitioned or...).
Found during testing of the pNFS server.

11738fb0f6 Do not check whether AcpiOsGetTimer() is called during boot.
From ACPICA 20170929, AcpiOsGetTimer() should be available early because
While() loop timeout mechanism was reimplemented with it.  Unfortunately,
it means AcpiLoadTables() may cause panic when a While() loop is executed.
After having lengthy discussions with ACPICA developers, I have concluded
that dummy timecounter is good enough for the purpose and it is the least
intrusive solution for now.  Also, they reminded me the ACPI specification
implies OS timer function should be available before loading tables.

7a158e826d Support the EFI Runtime Services on arm64. As with amd64 we use the 1:1
mapping. This uses the new common code shared with amd64.

The RTC should only be accessed via EFI. There is no locking around it as
the spec only has this as a requirement for the PC-AT CMOS device.

db76829b2b hyperv/hn: Workaround erroneous hash type observed on WS2016.
Background:
- UDP 4-tuple hash type is unconditionally enabled in Hyper-V on WS2016,
  which is _not_ affected by NDIS_OBJTYPE_RSS_PARAMS.
- Non-fragment UDP/IPv4 datagrams' hash type is delivered to VM as
  TCP_IPV4.

Currently this erroneous behavior only applies to WS2016/Windows10.

Force l3/l4 protocol check, if the RXed packet's hash type is TCP_IPV4,
and the Hyper-V is running on WS2016/Windows10.  If the RXed packet is
UDP datagram, adjust mbuf hash type to UDP_IPV4.

9c82bec42d Improvements to sendfile(2) mbuf free routine.
o Fall back to default m_ext free mech, using function pointer in
  m_ext_free, and remove sf_ext_free() called directly from mbuf code.
  Testing on modern CPUs showed no regression.
o Provide internally used flag EXT_FLAG_SYNC, to mark that I/O uses
  SF_SYNC flag.  Lack of the flag allows us not to dereference
  ext_arg2, saving from a cache line miss.
o Create function sendfile_free_page() that later will be used, for
  multi-page mbufs.  For now compiler will inline it into
  sendfile_free_mext().

07e87a1d55 In mb_dupcl() don't copy full m_ext, to avoid cache miss. Respectively,
in mb_free_ext() always use fields from the original refcount holding
mbuf (see. r296242) mbuf.  Cuts another cache miss from mb_free_ext().

However, treat EXT_EXTREF mbufs differently, since they are different -
they don't have a refcount holding mbuf.

Provide longer comments in m_ext declaration to explain this change
and change from r296242.

e8fd18f306 Shorten list of arguments to mbuf external storage freeing function.
All of these arguments are stored in m_ext, so there is no reason
to pass them in the argument list.  Not all functions need the second
argument, some don't even need the first one.  The second argument
lives in next cache line, so not dereferencing it is a performance
gain.  This was discovered in sendfile(2), which will be covered by
next commits.

The second goal of this commit is to bring even more flexibility
to m_ext mbufs, allowing to create more fields in m_ext, opaque to
the generic mbuf code, and potentially set and dereferenced by
subsystems.

History:
It appears that in the commit which introduced the code,
r165272, the array indexes of "sq_blocked[0]" and "td_name[i]"
were interchanged. In r180927 "td_name[i]" was corrected to
"td_name[0]", but "sq_blocked[0]" was left unchanged.

03ca213761 The recent change to initialization of blists (r324420) relied on '-1'
appearing only where the code explicitly set it, but since much of the
data was not initialized, '-1' appeared other places too, and led to
panics.  Clear the allocated data before initializing nonzero values by
allocating with M_ZERO.

13a5d9112c Improvements to set_user_ldt().
Remove mtx_owned() checks from set_user_ldt().  Split the function
into _locked() version which requires the dt_lock spinlock owned, and
make set_user_ldt() a wrapper.  Add a comment in swtch.s noting that
the call to the new set_user_ldt() cannot recurse on dt_lock.

Remove #ifdef SMP block, the addend is always zero on UP.

Fix type of set_user_ldt_rv(), making it match the type used for
smb_rendezvous() callback, and remove the cast.  Use curproc.

8eefcd407b The blst_radix_init function has two purposes - to compute the number of
nodes to allocate for the blist, and to initialize them.  The computation
can be done much more quickly by identifying the terminating node, if any,
at every level of the tree and then summing the number of nodes at each
level that precedes the topmost terminator.  The initialization can also be
done quickly, since settings at the root mark the tree as all-allocated, and
only a few terminator nodes need to be marked in the rest of the tree.
Eliminate blst_radix_init, and perform its two functions more simply in
blist_create.

The allocation of the blist takes places in two pieces, but there's no good
reason to do so, when a single allocation is sufficient, and simpler.
Allocate the blist struct, and the array of nodes associated with it, with a
single allocation.

7e7ef41632 Fix imx6 hdmi init after r323553, which used a config_intrhook to defer the
attachment of i2c devices needed by hdmi.

The hdmi init also uses an intrhook callback to defer initialization, and if
the hdmi callback runs first, the i2c devices will not yet have registered
their device_t in association with their FDT phandle, which allows cross-
device references on FDT systems.

Now the hdmi deferred init checks for the i2c device registration, and if
it's not complete yet, it registers as an eventhandler watching for newbus
attach events.  When the i2c device eventually attaches, the hdmi driver
unregisters from watching further events, and continues with the hdmi init.
Because the function signatures for an intrhook callback and an event
handler callback are the same, a single function is used for both callbacks.

7f92689427 Add eventhandler notifications for newbus device attach/detach.
The detach case is slightly complicated by the fact that some in-kernel
consumers may want to know before a device detaches (so they can release
related resources, stop using the device, etc), but the detach can fail. So
there are pre- and post-detach notifications for those consumers who need to
handle all cases.

A couple salient comments from the review, they amount to some helpful
documentation about these events, but there's currently no good place for
such documentation...

Note that in the current newbus locking model, DETACH_BEGIN and
DETACH_COMPLETE/FAILED sequence of event handler invocation might interweave
with other attach/detach events arbitrarily. The handlers should be prepared
for such situations.

Also should note that detach may be called after the parent bus knows the
hardware has left the building. In-kernel consumers have to be prepared to
cope with this race.

fc09164658 Restore the ability to deregister an eventhandler from within the callback.
When the EVENTHANDLER(9) subsystem was created, it was a documented feature
that an eventhandler callback function could safely deregister itself. In
r200652 that feature was inadvertantly broken by adding drain-wait logic to
eventhandler_deregister(), so that it would be safe to unload a module upon
return from deregistering its event handlers.

There are now 145 callers of EVENTHANDLER_DEREGISTER(), and it's likely many
of them are depending on the drain-wait logic that has been in place for 8
years. So instead of creating a separate eventhandler_drain() and adding it
to some or all of those 145 call sites, this creates a separate
eventhandler_drain_nowait() function for the specific purpose of
deregistering a callback from within the running callback.

37244a84fd Replace an unnecessary call to vm_page_activate() by an assertion that
the page is already wired or queued.  Prior to the elimination of PG_CACHED
pages, vm_page_grab() might have returned a valid, previously PG_CACHED
page, in which case enqueueing the page was necessary.  Now, that can't
happen.  Moreover, activating the page is a dubious choice, since the page
is not being accessed.

095db7e6e7 random(4): Gather entropy from Pure sources
At initialization, hc_source_mask only includes non-Pure sources.

The patch changes source registration to enable the registered source in the
hc_source_mask bitmask. This mask governs which sources are harvested.

This patch also disallows userspace from disabling such sources.

709939a7b7 namecache: factor out ~MAKEENTRY lookups from the common path
Lookups of the sort are rare compared to regular ones and succesfull ones
result in removing entries from the cache.

In the current code buckets are rlocked and a trylock dance is performed,
which can fail and cause a restart. Fixing it will require a little bit
of surgery and in order to keep the code maintaineable the 2 cases have
to split.

The later fields of the harvest_event structure are predictable and provide
little value to the entropy pool.  Only feed in the relatively high entropy
counter and explicit entropy buffer to increase measured input entropy.

a4730cdb82 Improve i386_get_ldt().
Provide consistent snapshot of the requested descriptors by preventing
other threads from modifying LDT while we fetch the data, lock dt_lock
around the read.  Copy the data into intermediate buffer, which is
copied out after the lock is dropped.

Comparing with the amd64 version, the read is done byte by byte, since
there is no atomic 64bit read (cmpxchg8b method is too heavy comparing
with the avoided issues).

Improve overflow checking for the descriptors range calculations and
remove unneeded casts.  Use unsigned types for sizes.

Allow zero num argument to i386_get_ldt() and i386_set_ldt().  This
case is handled naturally by the code flow.

31329820e5 Oversight on armv7 bulk commit: Make MACHINE_ARCH be armv7 for new
enough processors. This isn't ideal, because one could still compile
MACHINE_ARCH armv6, but with armv7 options enabled. We don't normally
do that, and it's a bit of an edge case so accept the less than ideal
solution here in the absence of something better.

094fc1ed0f Tag all armv7 kernels as such in their machine config line.
Transition all boards that support arm cortex CPUs to armv7. This
leaves two armv6 kernels in the tree. RPI-B, which uses the BCM2835
which has a ARM1176 core, and VERSATILEPB, which is a qemu board setup
around the time RPI-B went in. Copy std.armv6 to std.armv7, even
though that duplicates a lot of stuff. More work needs to be done to
sort out the duplication.

Copy all the places we do armv6 and add armv7 as basically an
alias. clang appears to generate code for armv7 by default. armv7 hard
float isn't supported by the the in-tree gcc, so it hasn't been
updated to have a new default.

Support armv7 as a new valid MACHINE_ARCH (and by extension
TARGET_ARCH).

b949cc41d1 Add Flex File Layout support to the NFSv4.1 pNFS client.
This patch adds support for the Flexible File Layout to the pNFS client.
Although the patch is rather large, it should only affect NFS mounts
using the "pnfs" option against pNFS servers that do not support File
Layout.
There are still a couple of things missing from the Flexible File Layout
client implementation:
- The code does not yet do a LayoutReturn with I/O error stats when
  I/O error(s) occur when attempting to do I/O on a DS.
  This will be fixed in a future commit, since it is important for the
  MDS to know that I/O on a DS is failing.
- The current code does writes and commits to mirror DSs serially.
  Making them happen concurrently will be done in a future commit,
  after discussion on freebsd-current@ on the best way to do this.
- The code does not handle NFSv4.0 DSs. Since there is no extant pNFS
  server that implements NFSv4.0 DSs and NFSv4.1 DSs makes more sense
  now, I don't intend to implement this until there is a need for it.

d07e22cdd8 locks: take the number of readers into account when waiting
Previous code would always spin once before checking the lock. But a lock
with e.g. 6 readers is not going to become free in the duration of once spin
even if they start draining immediately.

Conservatively perform one for each reader.

Note that the total number of allowed spins is still extremely small and is
subject to change later.

e284fa0d3f bnxt: fix intermittent VLAN issues
bnxt_init() is invokes Function Reset (bnxt_hwrm_func_reset) and thus FW
configuration for VLANs get erased.  To fix this, in bnxt_init(), Invoking
HWRM to configure VLANs (bnxt_hwrm_cfa_l2_set_rx_mask).

1c0054d261 Fix "taskqgroup_attach: setaffinity failed: 3" with iflib drivers
Improved logging added in r323879 exposed an error during
attach. We need the irq, not the rid to work correctly. em uses
shared irqs, so it will use the same irq for TX as RX. bnxt does
not use shared irqs, or TX irqs at all, so there's no need to set
the TX irq affinity.

a6d4b1dc48 Ensure that after sucessfull i386_set_ldt() call, other threads can
use LDT segments immediately.

If the i386_set_ldt() call created a first LDT descriptor (and
consequently created the LDT) for our address space, LDTR is currently
loaded only on the CPU executing the syscall.  Other CPUs executing
threads sharing the address space, would only load LDTR after context
switch.

Uncomment set_user_ldt_rv() and call it on all CPUs.  Remove critical
section inside set_user_ldt(), it is not needed in the context of call
from smp_rendezvous().

Set md_ldt after md_ldt_sd is initialized using the same code sequence
as in user_ldt_free().  Do the whole initialization in a critical
section, to not race with the context switching while we set LDT.

78d58cb6bc Avoid a race betweem freeing LDT and context switches.
cpu_switch.S uses curproc->p_md.md_ldt value as the flag indicating
presence of the process LDT.  The flag is checked and then ldt segment
descriptor is copied into the CPU' GDT slot.

Disallow context switches around clearing of the curproc LDT state by
performing the cleanup in critical section.  Ensure that the md_ldt
flag is cleared before md_ldt_sd descriptor content is destroyed by
inserting fence between the operations.

We depend on the x86 memory model strong ordering guarantees, in
particular, that cpu_switch.S observes the writes to md_ldt and
md_ldt_sd in the expected order.

693593b6f0 sysctl-s in a module should be accessible only when the module is initialized
A sysctl can have a custom handler that may access data that is initialized
via SYSINIT(9) or via a module event handler (also invoked via SYSINIT).
Thus, it is not safe to allow access to the module's sysctl-s until
the initialization is performed.  Likewise, we should not allow access
to teh sysctl-s after the module is uninitialized.
The latter is easy to achieve by properly ordering linker_file_unregister_sysctls
and linker_file_sysuninit.
The former is not as easy for two reasons:
- the initialization may depend on tunables which get set when sysctl-s are
  registered, so we need to set the tunables before running sysinit-s
- the initialization may try to dynamically add more sysctl-s under statically
  defined sysctl nodes
So, this change splits the sysctl setup into two phases.  In the first phase
the sysctl-s are registered as before but they are disabled and hidden from
consumers.  In the second phase, done after sysinit-s, normal access to the
sysctl-s is enabled.

The change should affect only dynamic module loading and unloading after
the system boot-up.  Nothing changes for sysctl-s compiled into the kernel
and sysctl-s in preloaded modules.

287c718f32 Improve amd64_get_ldt().
Provide consistent snapshot of the requested descriptors by preventing
other threads from modifying LDT while we fetch the data, lock dt_lock
around the read.  Copy the data into intermediate buffer, which is
copied out after the lock is dropped.

Use guaranteed atomic (aligned volatile) reads of the descriptors to
use same-size atomic as CPU update to set A bit in the descriptor type
field.

Improve overflow checking for the descriptors range calculations and
remove unneeded casts.

a4bc304da5 remove heuristic error detection from ddi_strto*()
Zero, <TYPE>_MIN and <TYPE>_MAX values can result from valid conversions.
They don't necessarily imply any error.
Since we do not have any reliable error signaling from libkern's strto*(),
it's better to always assume success rather than to report an error when
there is none.

a58679a93b Complete r323772 on amd64.
Compilers are allowed to combine plain reads into group operations,
e.g. 64bit element copies of one array into another can be
legitimately optimized back to a memcpy() call, which r323772 tried to
prevent.

Qualify accesses to LDT descriptors with volatile dereference to
ensure that each write indeed occurs.  After that, our usual claim of
native-size aligned writes being atomic applies.

This is equivalent to atomic_store(memory_order_relaxed) C11 accesses,
but our machine/atomic.h does not provide corresponding primitive.

a1fc6a8c49 On amd64, mark the set_user_ldt() function as static.
On i386, the function is used from the context switch code and needs

37afe7dfd2 Reduce default max_ldt_segment value to 512.
This makes the LDT to use only one page with default settings,
avoiding the need to find contigous 2 pages in KVA.  It seems that
most users are fine even with 512 segments.
