7df304f3e0 Stop linking against a direct-mapped virtual address and instead
use the PBVM. This eliminates the implied hardcoding of the
physical address at which the kernel needs to be loaded. Using the
PBVM makes it possible to load the kernel irrespective of the
physical memory organization and allows us to replicate kernel text
on NUMA machines.

While here, reduce the direct-mapped page size to the kernel's
page size so that we can support memory attributes better.

bb82622c3e Extend the rman(9) API to support altering an existing resource.
Specifically, these changes allow a resource to back a relocatable and
resizable resource such as the I/O window decoders in PCI-PCI bridges.
- rman_adjust_resource() can adjust the start and end address of an
existing resource. It only succeeds if the newly requested address
space is already free. It also supports shrinking a resource in
which case the freed space will be marked unallocated in the rman.
- rman_first_free_region() and rman_last_free_region() return the
start and end addresses for the first or last unallocated region in
an rman, respectively. This can be used to determine by how much
the resource backing an rman must be adjusted to accomodate an
allocation request that does not fit into the existing rman.

While here, document the rm_start and rm_end fields in struct rman,
rman_is_region_manager(), the bound argument to
rman_reserve_resource_bound(), and rman_init_from_resource().

b67d11bbcc Change rman_manage_region() to actually honor the rm_start and rm_end
constraints on the rman and reject attempts to manage a region that is out
of range.
- Fix various places that set rm_end incorrectly (to ~0 or ~0u instead of
  ~0ul).
- To preserve existing behavior, change rman_init() to set rm_start and
rm_end to allow managing the full range (0 to ~0ul) if they are not set by
the caller when rman_init() is called.

672dc4aea2 TCP reuses t_rxtshift to determine the backoff timer used for both the
persist state and the retransmit timer. However, the code that implements
"bad retransmit recovery" only checks t_rxtshift to see if an ACK has been
received in during the first retransmit timeout window. As a result, if
ticks has wrapped over to a negative value and a socket is in the persist
state, it can incorrectly treat an ACK from the remote peer as a
"bad retransmit recovery" and restore saved values such as snd_ssthresh and
snd_cwnd. However, if the socket has never had a retransmit timeout, then
these saved values will be zero, so snd_ssthresh and snd_cwnd will be set
to 0.

If the socket is in fast recovery (this can be caused by excessive
duplicate ACKs such as those fixed by 220794), then each ACK that arrives
triggers either NewReno or SACK partial ACK handling which clamps snd_cwnd
to be no larger than snd_ssthresh. In effect, the socket's send window
is permamently stuck at 0 even though the remote peer is advertising a
much larger window and pending data is only sent via TCP window probes
(so one byte every few seconds).

Fix this by adding a new TCP pcb flag (TF_PREVVALID) that indicates that
the various snd_*_prev fields in the pcb are valid and only perform
"bad retransmit recovery" if this flag is set in the pcb. The flag is set
on the first retransmit timeout that occurs and is cleared on subsequent
retransmit timeouts or when entering the persist state.

9f25ad52ce Introduce AR9130 (HOWL) WMAC support to the FreeBSD HAL.
The AR9130 is an AR9160/AR5416 family WMAC which is glued directly
to the AR913x SoC peripheral bus (APB) rather than via a PCI/PCIe
bridge.

The specifics:

* A new build option is required to use the AR9130 - AH_SUPPORT_AR9130.
This is needed due to the different location the RTC registers live
with this chip; hopefully this will be undone in the future.
This does currently mean that enabling this option will break non-AR9130
builds, so don't enable it unless you're specifically building an image
  for the AR913x SoC.

* Add the new probe, attach, EEPROM and PLL methods specific to Howl.

* Add a work-around to ah_eeprom_v14.c which disables some of the checks
for endian-ness and magic in the EEPROM image if an eepromdata block
is provided. This'll be fixed at a later stage by porting the ath9k
probe code and making sure it doesn't break in other setups (which
my previous attempt at this did.)

* Sprinkle Howl modifications throughput the interrupt path - it doesn't
implement the SYNC interrupt registers, so ignore those.

* Sprinkle Howl chip powerup/down throughout the reset path; the RTC methods
  were

* Sprinkle some other Howl workarounds in the reset path.

* Hard-code an alternative setup for the AR_CFG register for Howl, that
sets up things suitable for Big-Endian MIPS (which is the only platform
  this chip is glued to.)

This has been tested on the AR913x based TP-Link WR-1043nd mode, in
legacy, HT/20 and HT/40 modes.

Caveats:

* 2ghz has only been tested. I've not seen any 5ghz radios glued to this
chipset so I can't test it.

* AR5416_INTERRUPT_MITIGATION is not supported on the AR9130. At least,
it isn't implemented in ath9k. Please don't enable this.

* This hasn't been tested in MBSS mode or in RX/TX block-aggregation mode.

7b67bd9f3d This patch is believed to fix a problem in the kernel rpc for
non-interruptible NFS mounts, where a kernel thread will seem
to be stuck sleeping on "rpccon". The msleep() in clnt_vc_create()

The patch changes the msleep() in clnt_vc_create() so it only sets

4309e17add This patch changes head so that the default NFS client is now the new
NFS client (which I guess is no longer experimental). The fstype "newnfs"
is now "nfs" and the regular/old NFS client is now fstype "oldnfs".
Although mounts via fstype "nfs" will usually work without userland
changes, an updated mount_nfs(8) binary is needed for kernels built with
"options NFSCL" but not "options NFSCLIENT". Updated mount_nfs(8) and
mount(8) binaries are needed to do mounts for fstype "oldnfs".
The GENERIC kernel configs have been changed to use options
NFSCL and NFSD (the new client and server) instead of NFSCLIENT and NFSSERVER.
For kernels being used on diskless NFS root systems, "options NFSCL"
must be in the kernel config.
Discussed on freebsd-fs@.

7126ba42f6 - Add the possibility to reuse the already last used timeout when patting
the watchdog, via the watchdog(9) interface.
For that, the WD_LASTVAL bitwise operation is used. It is mutually
exclusive with any explicit timout passing to the watchdogs.
The last timeout can be returned via the wdog_kern_last_timeout()
  KPI.
- Add the possibility to pat the watchdogs installed via the watchdog(9)
  interface from the kernel.
In order to do that the new KPI wdog_kern_pat() is offered and it does
  accept normalized nanoseconds or WD_LASTVAL.
- Avoid to pass WD_ACTIVE down in the watchdog handlers. All the control
bit processing should over to the upper layer functions and not passed
down to the handlers at all.

These changes are intended to be used in order to fix up the watchdog
tripping in situation when the userland is busted, but protection is still
wanted (examples: shutdown syncing / disk dumping).

d9ca1af7ed VFS sometimes is unable to inactivate a vnode when vnode use count
goes to zero. E.g., the vnode might be only shared-locked at the time of
vput() call. Such vnodes are kept in the hash, so they can be found later.

If ffs_valloc() allocated an inode that has its vnode cached in hash, and
still owing the inactivation, then vget() call from ffs_valloc() clears
VI_OWEINACT, and then the vnode is reused for the newly allocated inode.

The problem is, the vnode is not reclaimed before it is put to the new
use. ffs_valloc() recycles vnode vm object, but this is not enough.
In particular, at least v_vflag should be cleared, and several bits of
UFS state need to be removed.

It is very inconvenient to call vgone() at this point. Instead, move
some parts of ufs_reclaim() into helper function ufs_prepare_reclaim(),
and call the helper from VOP_RECLAIM and ffs_valloc().

6f5fe81e02 Fix a corner-case of interrupt handling which resulted in potentially
spurious (and fatal) interrupt errors.

One user reported seeing this:

Apr 22 18:04:24 ceres kernel: ar5416GetPendingInterrupts: fatal error,
  ISR_RAC 0x0 SYNC_CAUSE 0x2000

SYNC_CAUSE of 0x2000 is AR_INTR_SYNC_LOCAL_TIMEOUT which is a bus timeout;
this shouldn't cause HAL_INT_FATAL to be set.

After checking out ath9k, ath9k_ar9002_hw_get_isr() clears (*masked)
before continuing, regardless of whether any bits in the ISR registers
are set. So if AR_INTR_SYNC_CAUSE is set to something that isn't
treated as fatal, and AR_ISR isn't read or is read and is 0, then
(*masked) wouldn't be cleared. Thus any of the existing bits set
that were passed in would be preserved in the output.

The caller in if_ath - ath_intr() - wasn't setting the masked value
to 0 before calling ath_hal_getisr(), so anything that was present
in that uninitialised variable would be preserved in the case above
of AR_ISR=0, AR_INTR_SYNC_CAUSE != 0; and if the HAL_INT_FATAL bit
was set, a fatal condition would be interpreted and the chip was
reset.

This patch does the following:

* ath_intr() - set masked to 0 before calling ath_hal_getisr();
* ar5416GetPendingInterrupts() - clear (*masked) before processing
continues; so if the interrupt source is AR_INTR_SYNC_CAUSE
and it isn't fatal, the hardware isn't reset via returning
  HAL_INT_FATAL.

This doesn't fix any underlying errors which trigger
AR_INTR_SYNC_LOCAL_TIMEOUT - which is a bus timeout of some
sort - so that likely should be further investigated.

1e09ff3dc3 Instead of allocating memory for all the keys at device attach,
create reasonably large cache for the keys that is filled when
needed. The previous version was problematic for very large providers
(hundreds of terabytes or serval petabytes). Every terabyte of data
needs around 256kB for keys. Make the default cache limit big enough
to fit all the keys needed for 4TB providers, which will eat at most
1MB of memory.

While in_pseudo() etc. is often used in offloading feature support,
in_cksum() is mostly used to fix some broken hardware.

Keeping both around for the moment allows us to compile NIC drivers
even in an IPv6 only environment without the need to mangle them
with #ifdef INETs in a way they are not prepared for. This will
leave some dead code paths that will not be exercised for IPv6.

Add pmap_invalidate_cache_pages() method on x86. It flushes the CPU
cache for the set of pages, which are not neccessary mapped. Since its
supposed use is to prepare the move of the pages ownership to a device
that does not snoop all CPU accesses to the main memory (read GPU in
GMCH), do not rely on CPU self-snoop feature.

amd64 implementation takes advantage of the direct map. On i386,
extract the helper pmap_flush_page() from pmap_page_set_memattr(), and
use it to make a temporary mapping of the flushed page.

be8b35eda7 Add checks for MNTK_UNMOUNTF at the beginning of three
functions, so that threads don't get stuck in them during
a forced dismount. nfs_sync/VFS_SYNC() needs this, since it is
called by dounmount() before VFS_UNMOUNT(). The nfscl_nget()
case makes sure that a thread doing an VOP_OPEN() or
VOP_ADVLOCK() call doesn't get blocked before attempting
the RPC. Attempting RPCs don't block, since they all
fail once a forced dismount is in progress.
The third one at the beginning of nfsrpc_close()
is done so threads don't get blocked while doing VOP_INACTIVE()
as the vnodes are cleared out.
With these three changes plus a change to the umount(1)
command so that it doesn't do "sync()" for the forced case
seem to make forced dismounts work for the experimental NFS
client.

336d023b2e Make in_proto.c dependent on either inet or inet6.
While it does not provide any functionality for IPv6, it provides
the sysctl nodes for net.inet.* that a lot of functionality shared
between IPv4 and IPv6 depends on. We cannot change these anymore
without breaking a lot of management and tuning.

In case of IPv6 only, we compile out everything but the sysctl node
declarations.

a4646b9313 Allow using CMSG_NXTHDR with -Wcast-align.
If various checks are omitted, the CMSG_NXTHDR macro expands to
(struct cmsghdr *)((char *)(cmsg) + \
  _ALIGN(((struct cmsghdr *)(cmsg))->cmsg_len))

Although there is no alignment problem (assuming cmsg is properly aligned
and _ALIGN is correct), this violates -Wcast-align on strict-alignment
architectures. Therefore an intermediate cast to void * is appropriate here.

There is no workaround other than not using -Wcast-align.

have many thousands of entries in rmans, making scrolling through the
output of "show all rman" impractical. Individual rmans can then be viewed
including their contents with "show rman 0xaddr" as usual.

8d169381e4 Improve SATA Asynchronous Notification feature support in CAM:
- make SATA SIMs announce capabilities to handle SDB with Notification bit;
 - make PMP driver honor this SIMs capability;
 - make SATA XPT to negotiate and enable this feature for ATAPI devices.

This feature allows supporting SATA ATAPI devices to inform system about
some events happened, that may require attention. In my case this allows
LG GH22LS50 SATA DVR-RW drive to report tray open/close events. Events
reported to CAM in form of AC_SCSI_AEN async. Further they could be used
as a hints for checking device status and reporting media change to upper
layers, for example, via spoiling mechanism of GEOM.

This is bit dirty and likely should be revised at a later date,

and allowing developers to do "tricky stuff" as they desire.
For now, this works.

c772d0204f Port over a TX gain fix from ath9k specific to the AR9285 (Kite) and AR9271.
Note: this HAL currently only supports the AR9285.

From Linux ath9k:

The problem is that when the attenuation is increased,
the rate will start to drop from MCS7 -> MCS6, and finally
will see MCS1 -> CCK_11Mbps. When the rate is changed b/w
CCK and OFDM, it will use register desired_scale to calculate
how much tx gain need to change.

The output power with the same tx gain for CCK and OFDM modulated
signals are different. This difference is constant for AR9280
but not AR9285/AR9271. It has different PA architecture
a constant. So it should be calibrated against this PA
characteristic.

The driver has to read the calibrated values from EEPROM and set
the tx power registers accordingly.

27afdbaa51 Introduce geom_map, a GEOM provider designed for use by
embedded flash stores.

Some devices - notably those with uboot - don't have an
explicit partition table (eg like Redboot's FIS.)
geom_map thus provides an easy way to export the hard-coded
flash layout as geom providers for use by filesystems and
other tools.

It also includes a "search" function which allows for
dynamic creation of partition layouts where the device only
has a single hard-coded partition. For example, if
there is a "kernel+rootfs" partition, a single image can
be created which appends the rootfs after the kernel with
an appropriate search string. geom_map can be told to
search for said search string and create a partition
beginning after it.

4ac80906c3 Fix a long standing SUJ performance problem:
- Keep a hash of indirect blocks that have recently been freed and are
   still referenced in the journal.
 - Lookup blocks in this hash before forcing a new block write to wait on
the journal entry to hit the disk. This is only necessary to avoid
confusion between old identities as indirects and new identities as
   file blocks.
 - Don't free jseg structures until the journal has written a record that
invalidates it. This keeps the indirect block information around for
as long as is required to be safe.
 - Force an empty journal block write when required to flush out stale
journal data that is simply waiting for the oldest valid sequence
   number to advance beyond it.
